Although type 2 diabetes is a disease often associated with aging, the global prevalence of early-onset diabetes has been increasing due to man’s sedentary lifestyle, low-physical activity, obesity, and some non-modifiable risk factors. Many studies have found that individuals with early-onset type 2 diabetes were at higher risk of developing vascular complications than those with late-onset diabetes. Individuals with early-onset diabetes are usually unwilling to visit hospital and have more confidence in their health, which results in poor glycemic control and the delayed detection of diabetes-related complications. Few studies have focused on the treatment and prevention of complications in specific population of individuals with early-onset type 2 diabetes. Therefore, focusing on this particular population is critical for the government and academic societies. Screening for T2DM is imminent for young adults with a family history of diabetes, obesity, markers of insulin resistance, or alcohol consumption. More data are definitely required to establish a reasonable risk model to screen for early-onset diabetes.

Clinical efficacy and safety of Traditional Chinese Medicine combined with Western Medicine in patients with diabetic acute ischemic stroke

OBJECTIVE： To observe the clinical efficacy and safety of Traditional Chinese Medicine （TCM） com- bined with Western Medicine （WM） in patients with diabetic acute ischemic stroke. METHODS： Ninety patients with diabetic acute ischemic stroke were randomly divided into a treat- ment group and a control group. The 45 patients in the treatment group were given standardized treat- ment with TCM combined with WM. They received corresponding oral Chinese decoctions three times daily, according to their TCM syndromes, along with basic western medical treatment. The 45 patients in the control group were given non-stan- dardized treatment with TCM combined with WM. They received an oral Chinese decoction for promo- tion of blood circulation to inhibit hemostasis, regardless of their TCM syndromes, along with ba- sic western medical treatment. The treatments lasted for 4 weeks. Scores were evaluated on the National Institutes of Health Stroke Scale （NIHSS） score, activity of daily life （ADL） scores, and TCM symptoms before treatment and 2 and 4 weeks after treatment. RESULTS： Analysis of variance for repeated mea- surements showed that there were significant dif- ferences in NIHSS and ADL score before and after treatment in both groups （P〈0.05）. There were also significant differences between the scores at 2 and 4 weeks after treatment. There were signifi- cant differences in TCM syndrome scores before and after treatment in both groups （P〈0.05）. There were also significant differences between the scores at 2 and 4 weeks after treatment. The X2 test showed no statistically significant difference in the incidence of adverse reactions between the two groups （P〉0.05）. CONCLUSION： Standardized treatment was supe- rior to non-standardized treatment for clinical efficacy of TCM combined with WM in patients with diabetic acute ischemic stroke, and the superiority was more obvious in improving neural dysfunction, ADL score, and TCM symptoms. OBJECTIVE： To investigated the role of Mailuoning in the prevention of highglucosemediated cell apoptosis in human umbilical vein endothelial cells （HUVEC） and the mechanisms involved. METHODS： MTT assay was used to investigate cell viability, western blot was used to investigate pro tein expression, and flow cytometric detection technology was used to detect cell apoptosis. RESULTS： Exposure of HUVEC to high glucose （50 mM） significantly suppressed cell viability and increased cell apoptosis compared with normal glu cose （11 mM）（all P〈0.05）. However, Mailuoning pre vented highglucoseinduced HUVEC apoptosis in dosedependent manner. Further studies indicated that Mailuoning suppressed highglucoseinduced p38 mitogenactivated protein kinase phosphoryla tion, but had no effect on extracellular signalregu lated kinase 1/2 and Akt phosphorylation. CONCLUSION： Mailuoning can prevent highglu coseinduced HUVEC apoptosis by suppressing p38 activation.

Euglycemic diabetic ketoacidosis(DKA)is an acute life-threatening metabolic emergency characterized by ketoacidosis and relatively lower blood glucose(less than 11 mmol/L).The absence of hyperglycemia is a conundrum for physicians in the emergency department and intensive care units;it may delay diagnosis and treatment causing worse *** DKA is an uncommon diagnosis but can occur in patients with type 1 or type 2 diabetes *** the addition of sodium/glucose cotransporter-2 inhibitors in diabetes mellitus management,euglycemic DKA incidence has *** other causes of euglycemic DKA include pregnancy,fasting,bariatric surgery,gastroparesis,insulin pump failure,cocaine intoxication,chronic liver disease and glycogen storage *** pathophysiology of euglycemic DKA involves a relative or absolute carbohydrate deficit,milder degree of insulin deficiency or resistance and increased glucagon/insulin *** DKA is a diagnosis of exclusion and should be considered in the differential diagnosis of a sick patient with a history of diabetes mellitus despite lower blood glucose or absent urine *** diagnostic workup includes arterial blood gas for metabolic acidosis,serum ketones and exclusion of other causes of high anion gap metabolic *** DKA treatment is on the same principles as for DKA with correction of dehydration,electrolytes deficit and insulin *** dextrosecontaining fluids should accompany intravenous insulin to correct metabolic acidosis,ketonemia and to avoid hypoglycemia.

It is true that a primary goal of diabetes early diagnosis and treatment is quality of life(QoL). The term QoL is still confusing but it is agreed that it composes of four components:The physical component, mental, cogitative component, psychological and social component. Many articles have been written addressing those four components. During the last five years 15500 articles and reviews have been written addressing diabetes and coronary arterial disease, 16100 addressing diabetes and renal function, 28900 addressing diabetes and retinopathy, 16800 addressing diabetic foot ulcers and other 26300 addressing diabetic neuropathy. Moreover 17200 articles are dealing with diabetic sexual dysfunction, 24500 with the correlation of diabetes and depression 17500 about diabetes and dementia, only 1 about diabetes and family functioning and 1950000 about diabetes and QoL, indicating the worldwide interest. In order to confront this metabolic anomaly and its consequences, researchers developed numerous generic and disease specific psychometric tools. With the aid of those psychometric tools the scientific community has started to realize the gruesome effect of diabetes on patients' lives. Diabetic's QoL becomes worse when complications start to develop or comorbidities coexist. Dominant amongst complications, in health-related quality of life(HRQoL) lowering, but not related to risk factors(genetic, the weight of birth, or others) is coronary arterial disease followed by renal failure, blindness, and the combination of micro-and macrovascular complications and in some studies by sexual dysfunction. Moreover many are the comorbidities which deteriorate further the effect of diabetes in a patient life. Among them obesity, hypertension, dyslipidemia, depression, arthritis are the most common.
