Does endoscopic sclerotherapy in filarial chyluria affect renal function and morphology? A prospective study using dimercaptosuccinic acid renal scan

Objective:To look for change in relative renal function and document renal scarring following endoscopic renal pelvic instillation sclerotherapy(RPIS)in patients with chyluria by dimercaptosuccinic acid(DMSA)renal ***:A prospective study was performed between November 2015 and September *** patients with biochemically documented chyluria who underwent RPIS using either 1%-silver nitrate or 0.1%-povidine iodine were *** received either 3-,6-or *** renal scan was performed before and 2e3 months after ***:Of the 34 patients,22 were *** age was 41.08±16.64 years(range,15-70 years).Thirty-two patients(94.1%)responded to therapy while two did not respond even after *** follow-up was 8.94±3.70 *** mean relative renal function(preinstillation)of normal kidney was 50.76%±3.55%while that of affected renal unit(side of instillation)was 49.20%±3.44%(range,43.0%-61.0%).After instillation therapy,the mean relative renal function of normal side was 52.26%±3.57%while that of affected renal unit was 47.50%±3.56%(range,41.0%-54.0%).The relative renal function did not change>5%from the baseline value in any patient except one(in which the differential function increased paradoxically by 12%).Two patients developed renal scar in post-instillation renal ***:Endoscopic sclerotherapy in chyluria is safe and *** relative renal function does not deteriorate by more than 5%.There is a small risk of development of renal *** studies involving larger number of patients are needed to answer this dilemma.

A comparison of retroperitoneoscopic and open surgical renal pedicle lymphatic disconnection for the treatment of serious filarial chyluria

Currently the most effective clinical management for serious intractable chyluria is renal pedicle lymphatic disconnection, which generally consists of nephrolympholysis, renal hilar lymphatic vessel stripping and ureterolympholysis. In most conventional open surgical procedures, a lumbar incision is used and perinephrium and adipose capsule of kidney cut open, resulting in a heavy injury and long interval of postoperative recovery. The laparoscopic technique, with minimal invasion, less complications and rapid postoperative recovery, is being utilized more and more as the alternative surgical technique for urological disease.

Retroperitoneoscopic renal pedical lymphatic disconnection in the treatment of chyluria

chyluria is a disease that may severely debilitate a patient＇s health condition. Some investigations have indicated that in patients with chyluria, part of the uptake fat and protein leaks from the ***, recurrent chyluria may result in malnutrition. In addition, with nearly one half of the body＇s the urine, depressed regulatory lymphocytes lost through functions of the lymphatic system may cause illnesses following immune deficiency. Therefore, it is imperative to provide effective and active medical care for chyluria patients. The purpose of surgical treatment is to eliminate lymphatic fistula and decrease intralymphatic pressure. We recently performed retroperitoneoscopic renal pedical lymphatic disconnection for 78 cases of chyluria patients in our hospital between April 2003 and June 2007. We herein summarize our experience as follows.

MRI as Non-Invasive Modality of Choice for Diagnosis of Cause in Case of chyluria

chyluria is a urological manifestation of lymphatic system abnormality as a result of some abnormal communication between these two systems. Although chyluria as such is rare, filariasis is most common cause in Asian countries. This parasitic tropical debilitating disease is known for chronicity, recurrence and resultant complications. This article provides importance of Magnetic Resonance Imaging (MRI), as a non-invasive modality, in the diagnosis of exact etiopathogenesis of chyluria along with clinical symptoms, other investigations required and brief about the management protocols. On MRI, abnormally dilated lymphatics are visualized as meshwork of multiple tubular, tortuous, fluid-filled structures in the retroperitoneum. We report a case of 20 year young adult who presented with history of passing whitish urine on and off since 8 months. MRI helped in the detection of the site and level of lymphatic obstruction and resultant lympho-urinary fistula.
