Shenfu Injection(参附注射液) Suppresses Inflammation by Targeting Haptoglobin and Pentraxin 3 in Rats with chronic Ischemic heart failure

Objective： To investigate the regulatory effects of Shenfu Injection （SFI, 参附注射液） on hemodynamic parameters and serum proteins in rats with post-infarction chronic heart failure （CHF）. Methods： Forty-five healthy Wistar rats were randomized into three groups： sham, heart failure （model） and SFI group. The CHF was induced by left coronary artery ligation. Seven days after the surgical operation, animals in the sham group and the model group received saline （6.2 mL/kg/d）, while animals in the SFI group received SFI （6.2 mL/kg.d） intraperitoneally. Four weeks later, cardiac hemodynamic parameters were measured via the carotid route. The expression of serum proteins was analyzed by two-dimensional electrophoresis and matrixassisted laser desorption/ionization time-of-flight mass spectrometer （MALDI-TOF MS）. Results： Recording of hemodynamic parameters showed that left ventricular systolic pressure （LVSP）, maximum rate of left ventricular pressure （＋dp/dtmax） rise, and maximum rate of left ventricular pressure （-dp/dtmax） decrease, while the left ventricular end diastolic pressure （LVEDP） rose in the model group compared to those in the sham group （P〈0.05）. The results of the MALDI-TOF MS indicated that haptoglobin （HP）, pentraxin 3 （PTX3） and alpha-1- antitrypsin were up-regulated, while serum albumin and 40S ribosomal protein were down-regulated in the model group （P〈0.05）. Compared with the model group, LVSP, ＋dp/dtmax and -dp/dtmax were higher, while LVEDP was lower in the SFI group （P〈0.05）. Expression levels of HP and PTX3 were lower than in the model group （P〈0.05）. Conclusion： SFI could improve hemodynamic function and decrease inflammatory reactions in the pathophysiology of CHF. The serum proteins HP and PTX3 could be potential biomarkers for chronic ischemic heart failure, and they could also be the serum protein targets of SFI.

Efficacy of regional renal nerve blockade in patients with chronic refractory heart failure

Background Increased renal sympathetic nerve activity can result in diuretic resistance in patients with chronic congestive heart failure. We investigated the effect of regional renal nerve blockade on the patients with chronic refractory heart failure and diuretic resistance. Methods Eighteen patients with chronic refractory heart failure were enrolled （mean age （64＋11） years）. The patients were randomly divided into two groups （renal nerve blockade group and standard therapy group, n=9 each）. Renal nerve blockade was performed by percutaneous injection of local anaesthetic under computed tomographic guidance. heart rate, mean arterial blood pressure, plasma and urine electrolytes, neurohormones, factional excretion of sodium （FENa）, 24-hour urine volume were monitored at baseline and the first 24 hours after therapy. Dyspnea and oedema were also evaluated. The major adverse cardiovascular events （MACE）, plasma brain natriuretic peptide （BNP） level and left ventricular ejection fraction （LVEF） were compared between the two groups during the 3-12 months follow-up period. Results No complication was observed during the acute phase of renal nerve blockade. After renal nerve blockade, the 24-hour urine volume and FENa were significantly increased, while the level of plasma rennin, angiotensin II, aldosterone BNP and atrial natriuretic peptide as well as dyspnea and oedema were significantly reduced in renal nerve blockade group compared with baseline and standard therapy group. During three to 12 months of follow-up, the rate of MACE and plasma BNP level were significantly lower, while LVEF was significantly higher in renal nerve blockade group than those in standard therapy group. Conclusion Regional renal nerve blockade may be a safe and effective treatment for patients with chronic refractory heart failure.

Analysis of the Diagnostic Value of Echocardiography in Patients with chronic heart failure

Objective:This study aims to analyze the diagnostic value of echocardiography in patients with chronic heart ***:30 patients with chronic heart failure admitted to our hospital during January 2021 to January 2023 were selected as the observation group,and 30 healthy people who received physical examination in our hospital during the same period were selected as the reference *** groups received echocardiography,and the results of the ultrasound examination were compared,and the subjects in the observation group were grouped according to the New York heart Association(NYHA)cardiac function grading *** results of echocardiography in patients with chronic heart failure of different cardiac function grades were ***:The left ventricular ejection fraction(LVEF)and ratio of maximum blood flow in the early diastole(E)to movement speed(A)(E/A)in the observation group were lower than those in the reference group,while the left ventricular posterior wall thickness(LVPWd),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),left atrial diameter(LAD),and interventricular septal thickness(IVS)were higher than those of the reference group(Pchronic heart failure continued to decrease,while the levels of LVPWd,LVEDD,LVESD,LAD,and IVS continued to increase,and there were significant differences among the groups(Pchronic heart failure can accurately evaluate the changes of cardiac shape and cardiac function,and provide a reliable basis for clinical diagnosis with a high application value.

Clinical Research of Zhenwu Decoction Combined with Tingli Dazao Xiefei Decoction in the Treatment of chronic heart failure with Edema Syndrome due to Yang Deficiency

[Objectives]The paper was to investigate the clinical effects of Zhenwu Decoction combined with Tingli Dazao Xiefei Decoction in the treatment of chronic heart failure with edema syndrome due to yang deficiency.[Methods]Eighty patients with chronic heart failure were randomly divided into 2 groups(control group and treatment group),with 40 cases in each *** patients in the control group were treated with conventional western medicine for 14 d,and the patients in the treatment group were treated with Zhenwu Decoction and Tingli Dazhao Xiefeng Decoction on the basis of the control group for 14 *** curative effect of traditional Chinese medicine(TCM)syndrome score,left ventricular ejection fraction(LVEF),plasma N-terminal brain natruretic peptide precursor(NT-proBNP)and New York heart Association(NYHA)functional class were compared between the two groups before and after *** living with heart failure questionnaire in the principle of Chinese civilization and traditional medicine characteristics(C-MLHF)at admission and 1 month after discharge were compared between the two groups.[Results]After treatment,the total effective rate of TCM syndrome score in the treatment group was 82.50%,and that in the control group was 67.50%.The curative effect of TCM syndrome score in the treatment group was better than that in the control group(P<0.05).After treatment the LVEF in both groups was increased compared with that before treatment,and the NT-proBNP level was decreased compared with that before treatment(P<0.05),and those in the treatment group were better than those in the control *** total effective rate of cardiac function in the treatment group was 87.50%,and that in the control group was 65.00%.The improvement of cardiac function in the treatment group was better than that in the control group(P<0.05).One month after discharge,C-MLHF scores in both groups were decreased compared with those before treatment,and that in the treatment group was l

Value of N-Terminal Pro B-Type Natriuretic Peptide,High-Sensitivity C-Reactive Protein,and Homocysteine Levels in Predicting Cardiovascular Events in chronic heart failure Patients After Discharge

Objective:To investigate the value of N-terminal pro B-type natriuretic peptide(NT-proBNP),high-sensitivity C-reactive protein(hs-CRP),and homocysteine(Hcy)levels in predicting cardiovascular events(CV)in patients with chronic heart failure(CHF).Methods:A total of 63 patients with CHF admitted to our hospital between June 2019 and July 2021 were *** NT-proBNP,hs-CRP,and Hcy levels were detected at discharge,and a 12-month follow-up was done after their discharge to collect clinical *** collected data were inclusive of data from 21 CHF patients with cardiovascular disease and 42 CHF patients without cardiovascular *** effect of NT-proBNP,hs-CRP,and Hcy levels on the occurrence of CV was ***:The levels of NT-proBNP,hs-CRP,and Hcy in the group with cardiovascular disease were significantly higher than those in the group without cardiovascular disease(P<0.05);the levels of serum NT-proBNP,hs-CRP,and Hcy at discharge had certain value in predicting short-term CV in CHF patients(P<0.05).Conclusion:NT-proBNP,hs-CRP,and Hcy levels can be used to predict CV in CHF patients,thus having clinical application value.

Aims:Increased indiscriminate use of pulmonary artery hypertension targeted drugs has been observed in patients with pulmonary hypertension(PH)secondary to heart *** performed a meta-analysis to evaluate the chronic effects of using phosphodiesterase 5(PDE5)inhibitors to treat patients with PH secondary to chronic systolic heart *** and Results:PubMed,EMBASE,and the Cochrane Library were searched through October 2013 for randomized controlled trials(RCTs)assessing PDE5inhibitor treatments in PH patients secondary to chronic heart *** RCTs involving 206 chronic systolic heart failure patients with PH complications were *** was used in all *** treatment resulted in fewer hospital admissions compared with the placebo treatment(3.15%versus 12.20%;risk ratio,0.29;95%confidence interval,0.11-0.77).Various hemodynamic parameters were improved with additional sildenafil treatment,including reduced mean pulmonary artery pressure(weighted mean differences,[WMD],-5.71 mmHg,pheart rate and blood *** exercise capacity improved(Oxygen consumption at peak exercise,WMD,3.20 ml/min-1/kg-1,pchronic systolic heart failure.

Application and Promotion of Whole-Process Capacity Management Model for CHF Patients Led by Specialist Nurses in “heart failure Center Alliance Unit”

Objective: To implement the whole-process capacity management model led by specialist nurses, improve the capacity management behavior of medical staff, and build a standardized, standardized and operable CHF capacity management system. Methods: According to the evidence pyramid principle and search strategy, 2 evidence-based nursing backbone completed literature search in both Chinese and English, and finally included 7 literatures. Results: Around the three key links of capacity assessment, monitoring and management, stakeholders were invited to evaluate each evidence according to the FAME principle, that is, the feasibility, suitability, effectiveness and clinical significance of evidence. Finally, 11 best evidences were obtained and 5 clinical review indicators of the cost project were transformed. This study formulated the competence management plan for CHF patients based on the current situation, established competence load evaluation criteria for CHF patients, and determined the target “dry weight” value for CHF patients. Conclusion: The whole-course volume management model of CHF patients guided by specialist nurses should be established and applied and promoted in the “heart failure Center Alliance unit”, so as to improve the capacity management ability of medical staff for CHF patients, enhance the self-management ability of CHF patients, improve the capacity management behavior and health outcomes, and effectively reduce the hospitalization rate and mortality rate of CHF patients in the region.
