Many patients with left ventricular systolic dysfunction have concomitant mitral regurgitation(MR). Their symptoms and prognosis worsen with increasing severity of MR. Percutaneous Mitra Clip® can be used safely to reduce the severity of MR even in patients with advanced heart failure and is associated with improved symptoms, quality of life and exercise tolerance. However, a few patients with very poor left ventricular systolic function may experience significant haemodynamic disturbance in the peri-procedural period. We present three such patients, highlighting some of the potential problems encountered and discuss their possible pathophysiological mechanisms and safety measures.

＜正＞Objective:To discuss the potential role of medicinal herbs,especially those with effect of strengthening "qi",in the treatment of chronic heart failure（CHF） via modulating myocardial substrate ***:the relationships among heart failure,myocardial energetic metabolism and herbal medicine were analyzed in detail through reviewing and summarizing the accumulating knowledge and recent findings on myocardial metabolism,heart failure and herbal ***:either energy lack or abnormal energetic metabolism is one of the main causes to the initiation and development of heart *** studies suggest that the cardiac function in the patients with CHF could be improved by inhibiting the energy production from metabolism of fatty acid and enhancing the energy production from glucose *** concept of "qi" in Chinese medicine is very close to the energy in western *** decoction of Chinese medicine containing herbal medicine with effect of strengthening "qi" and the herbal medicine themselves and their components were evidenced to be effective in improvement of heart failure and regulation of both lipid and glucose metabolism;more importantly,all these herbal medicine contain the multi-sugar which can be metabolized into single unit of sugar which may be metabolized as a substrate by myocardium and potentially produce the cardioprotective ***,we may find out a novel way to explain why the herbal medicine with effect of strengthening "qi" can improve cardiac function in patients with *** other words,regulating myocardial metabolism is one of the mechanisms underlying the cardioprotection produced by herbal medicine with effect of strengthening "qi" in the treatment of heart failure.

Alcoholic cardiomyopathy(ACM)is a dilated cardiomyopathy induced by excessive alcohol consumption that causes progressive cardiac dysfunction and chamber dilatation,leading to arrhythmias,heart failure,and *** transplantation is the final measure for end-stage *** this case,we used multimodal imaging for ACM diagnosis in a patient with refractory heart *** patient received a heart transplant with a good prognosis,and his pathological results after the transplant confirmed our suspected ***-stage ACM may lead to refractory heart failure and multimodal imaging may play an important role in the diagnosis,prognosis prediction,and follow-up study of suspected ACM.

Cardiac disease（CD） and Alzheimer’s disease（AD） are main threats to public *** from epidemiology and clinico-pathology have shown that CD may be causal in the progress of AD,but the underlying mechanisms are largely *** with myocardial infarction /congestive heart failure were tested for the effects of heart failure（HF） on neuropsychiatric and cognitive *** male or female mice were subjected to the surgery of left anterior descending artery（LAD） *** 1 to 2 weeks,high-frequency echocardiography was performed to exam the cardiac structure and *** with heart failure exhibited anxiety-like or depression-like *** months after surgery,mice with chronic heart failure（CHF） displayed the impairment of learning to some extent in the Morris Water Maze *** expressions of selected molecules involved in the metabolism of p-amyloid,apoptosis and inflammation were altered in CHF mice,p-amyloid accumulation,degenerating neurons and gliosis could not be detected in mice with a 3-month or 6-month course of heart *** findings give a clear connection that chronic heart failure moderately increases the risk of cognitive impairment and alters the cerebral metabolism of *** relationship between CHF and neurodegenerative diseases is ongoing.

目的:通过建立阿霉素诱导的慢性心衰大鼠模型,观察参苈加颗粒对慢性心衰大鼠心功能、心肌组织病理形态改变、24h尿量和尿离子含量的影响;观察其对慢性心衰大鼠AVP、AQP-2的含量的影响,从改善心功能和减轻水潴留的角度探讨参苈加颗粒的抗心衰作用。方法:选取135只雄性Wistar大鼠,体重200±20g,预饲养1周,每只称重后随机选取1 5只作为空白组,其余120只腹腔注射2mg/ml浓度的ADR溶液,建立慢性心力衰竭动物模型,每只大鼠按1.5mg/kg的剂量腹腔注射,每周2次,共7周。空白组注射等体积生理盐水。末次给药2周后,将造模成功的大鼠随机分成6组。共7组,为空白组、模型组、芪苈强心组(中药对照组)、呋塞米组(西药对照组)及参苈加颗粒低剂量组、参苈加颗粒中剂量组、参苈加颗粒高剂量组,每组1 5只,分组后,将实验大鼠定额灌胃3ml/只/次,3次/天,共28天。每次按下列剂量:芪苈强心组(0.09g/kg)、呋塞米组(6.09mg/kg),参苈加颗粒低剂量(1.07g/kg)、参苈加颗粒中剂量组(2.1g/kg)、参苈加颗粒高剂量(4.26g/kg)。空白组不采取措施,模型组给予同等体积的生理盐水灌胃,3次/天,共28天。各组大鼠的心功能指标用心脏彩色超声检测,大鼠心肌病理学检测用HE染色法测定、用透射电镜观察,大鼠尿液离子水平用全自动生化分析仪检测,各组大鼠AVP、AQP-2的含量均使用ELISA试剂盒进行检测。结果:1.和模型组相比,各治疗组大鼠AVP和AQP-2含量均降低,但以参苈加中剂量组降低最为明显。通过实验发现慢性心衰大鼠AVP和AQP-2的水平变化趋势一致,随心衰的进展而升高,随药物治疗而降低。2.和模型组相比,各治疗组大鼠一般情况和心功能均有改善,左心射血分数增高,心肌组织损伤减轻,以参苈加中剂量组改善最为明显。3.各治疗组大鼠24h尿量及各尿离子含量增高,其中呋塞米组尿量及各尿离子含量远高于空白组,而参苈加各剂量组尿量与各尿离子含量明显增多,但均低于空白组。结论:1.参苈加颗粒能显著降低慢性心衰大鼠AVP及AQP-2的水平,改善心功能,提高左心射血分数,减轻心肌病理改变,改善水潴留,减轻慢性心衰的症状体征,尤以参苈加中剂量组的治疗效果最为显著。2.参苈加颗粒治疗慢性心衰的机制,与其能抑制机体AVP的分泌,通过V2受体下调肾脏AQP-2的表达有关。3.参苈加颗粒治疗慢性心衰,和西药呋塞米相比,可减少因体液离子过度丢失导致的电解质紊乱和其他不良反应。

Shenfu decoction（SFD） can be used to treat patients with sign of Yangqi decline or Yang exhaustion related to chronic heart failure（CHF）.We conducted a gas chromatography with time-of-flight mass spectrometer（GC/TOF-MS）-based metabolomics study to increase the understanding of CHF and assess the efficacies and mechanisms of SFD in treating CHF induced by coronary artery ligation in *** on unsupervised principal component analysis,there was a clear separation between the CHF and sham surgery group,which revealed that CHF disturbed the metabolism of endogenous substances and significantly altered the urine metabolite *** SFD treatment,the metabolomics profile found in CHF was significantly reversed,shifting much closer to normal controls and sham surgery group,indicating that SFD has therapeutic effects in CHF,which is in accordance with the hemodynamic assay *** pathway analysis demonstrated that several pathways including fatty acid biosynthesis,fatty acid elongation,steroid biosynthesis,galactose metabolism,and amino acid metabolism were significantly altered in CHF ***,we may infer that SFD shows therapeutic efficacy in CHF by restoring these disturbed metabolic pathways,especially those related to energy *** study offers new methodologies for increasing the understanding of CHF and systematically characterizing the efficacies and mechanisms of SFD in treating CHF.
