Glucocorticoid reduction induced chorea in pediatric-onset systemic lupus erythematosus:A case report

BACKGROUND Pediatric-onset systemic lupus erythematosus(SLE)is typically more severe than adult-onset SLE,with a higher incidence of nervous system *** is a relatively rare neurological complication reported in 2.4%-7%of SLE *** particular,chorea induced by glucocorticoid dose reduction is even ***,we report the case of a girl with SLE,who developed chorea during the process of glucocorticoid therapy *** SUMMARY We describe a 14-year-old girl who was diagnosed with *** was treated with methylprednisolone and rituximab,and her symptoms *** the second day after the methylprednisolone dose was reduced according to the treatment guidelines,the patient developed *** condition improved after adjusting her glucocorticoid *** This case is a reminder that extra attention to chorea is required in SLE patients during glucocorticoid dose reduction.

We report a rare case about chorea in a Chinese adult after pulmonary thromboendarterectomy （PTE） with deep hypothermia and circulatory arrest.A 41-year-old male was admitted to our hospital because of breathing difficulties for about three years and intermittent hemoptysis for about two *** examination revealed mild cyanotic lips,lower extremity *** was classified as grade III heart failure according the classification system of the New York Heart Association （NYHA）.

Hemichorea Associated with Non-ketotic Hyperglycemia:A Case Report and Review of the Literature

Hemichorea associated with non-ketotic hyperglycemia(HC-NH)is a rare *** purpose of this case report is to introduce a patient with HC-NH and provide a schedule of examination and follow-up *** also reviewed the current understanding of pathophysiology and treatment and how to apply it to our *** case involved a 37-year-old Asian diabetic man who had a 9-day history of losing movement in left limbs and *** initial blood glucose level was 10.13 mmol/***1c was 13.6%.Before admission,head CT scan showed suspicious small pieces of left brainstem with slightly high-density shadow and right putamen nucleus with high-density *** the day of admission,head MRI showed punctate T1WI low signal shadow,T2WI high and low mixed signal shadow,T1WI high signal shadow and T2WI low signal shadow of right putamen *** case findings were consistent with his displayed motor pattern and with the HC-NH *** control of the blood glucose levels alleviates his choreiform *** endocrinology follow-up 6 months after discharge found that his symptoms did not recur after the outpatient’s medication compliance was ***-NH is a rare manifestation of poor diabetes control,but it should also be noticed by *** recognition and gradual treatment of elevated blood glucose levels seem to completely alleviate choreiform symptoms.

γ-Aminobutyric acid（GABA）,plays a key role in all stages of life,also is considered the main inhibitory *** activates two kind of membrane receptors known as GABAA and GABAB,the first one is responsible to render tonic inhibition by pentameric receptors containing α4-6,β3,δ,or ρ1-3 subunits,they are located at perisynaptic and/or in extrasynaptic *** biophysical properties of GABAA tonic inhibition have been related with cellular protection against excitotoxic injury and cell death in presence of excessive *** this basis,GABAA tonic inhibition has been proposed as a potential target for therapeutic intervention of Huntington＇s ***＇s disease is a neurodegenerative disorder caused by a genetic mutation of the huntingtin *** experimental studies of Huntington＇s disease mouse models have been developed,such as R6/1,R6/2,Hdh Q92,Hdh Q150,as well as *** all of them,some key experimental reports are focused on *** neostriatum is considered as the most important connection between cerebral cortex and basal ganglia structures,its cytology display two pathways called direct and indirect constituted by medium sized spiny neurons expressing dopamine D1 and D2 receptors respectively,they display strong expression of many types of GABAA receptors,including tonic *** studies about of GABAA tonic subunits and Huntington＇s disease into the neostriatum are rising in recent years,suggesting interesting changes in their expression and localization which can be used as a strategy to delay the cellular damage caused by the imbalance between excitation and inhibition,a hallmark of Huntington＇s disease.
