Characterization of growth and survival of mice displaying early onset hypertonic symptoms is critical as these animals are important for research investigating mechanisms and treatments of pediatric conditions associated with hypertonia,such as cerebral ***,most animal models of cerebral palsy reproduce risk factors for developing this condition,with most failing to develop the physical symptoms or failing to survive in the postnatal *** ***-Glrb spa/J(Gly receptor mutation)transgenic mouse(spa mouse),displays symptoms of early onset hypertonia,though little has been reported on growth and survival,with no reports of growth and survival since genotyping became *** found that the majority of spa mice display symptoms by *** mice surviving to weaning,only^9%were spa *** weaning age,spa mice had significantly lower weights than their heterozygote and wild-type *** mice that died after weaning and prior to use in experiments or being culled,48%were spa *** poor growth and decreased survival of spa mice across multiple developmental and adult ages resembled the varied survival rates observed in humans with mild or severe cerebral *** understanding of the expected survival of these mice is helpful for planning breeding and animal numbers for *** to the symptoms and timing of symptom onset,spa mice will be valuable in uncovering mechanisms and long-term effects of early onset hypertonia in order to move toward interventions for these conditions.

BACKGROUND： In the process of early screening and interventions to high risk infants with brain damage, the occasion and choosing methods of interventions and the combined application of different interventions are still at the exploratory phase. OBJECTIVE： To observe the efficacy of early intervention using Vojta and Bobath combined treatment in high risk infants with brain damage, and investigate the effect of early rehabilitation on the prognosis. DESIGN： A randomized controlled comparative observation. SETTING： Daqing Oil Field General Hospital of Heilongjiang Province. PARTICIPANTS： Eighty-four high risk infants younger than l year were selected from the Department of Pediatrics, Daqing Oil Field General Hospital of Heilongjiang Province from October 2005 to October 2006, including 52 boys （62%） and 32 girls （38%）. The treatment started at the age of 0 - 3 months in II cases （13%）, 4 - 6 months in 28 cases （33%）, 7 - 9 months in 35 cases （42%）, and 10 - 12 months in 10 cases （1 2%）. Infants with at least two of the followings were enrolled, including 7 Vojta abnormal postural reflexes, slow or disorder of motor development, increase of muscular tension, postural abnormality, primary reflection residual and CT/MRI abnormalities. Informed consents were obtained from their guardians. The 84 infants were randomly divided into treatment group （n =42） and control group （n =42）. METHODS： All the children were intravenously injected with cerebroprotein hydrolysate injection or cattle encephalon glycoside and ignotin injection, 10 times as a course for 2 - 5 courses; Besides, the infants in the treatment group also received early rehabilitative training of Vojta and Bobath combined treatment, once a day, 40 minutes per time, 5 times a week followed by a 2-day rest, 1 month as a course, and totally 2 - 5 courses. Bone Mineral Density and Vitamin D Status in Children with Chronic Neurological Syndromes—Clinical Observations

Background: Some of the risk factors for osteoporosis population include: chronic immobilisation, insufficient dietary supply of calcium and vitamin D, decreased physical activity and long-term pharmacological treatment (glucocorticoids, anticonvulsant drugs). In disabled children and adolescents, the negative impact of these factors may cumulate to considerably impair the quality of life. Objectives: The aim of our study was to assess the vitamin D status and bone mineral density in children with chronic neurological syndromes. Material and Methods: A total of 34 children between 3 and 18 years of age were examined: 9 children with muscular dystrophy, 17 with cerebral palsy and 8 with lumbar myelomeningocele. All the subjects underwent the following assessments: measurement of the concentration of the hepatic metabolite of vitamin D and total body less head and/or lumbar spine densitometry by dual energy X-ray absorptiometry (DEXA). Low bone mass or low bone mineral density was diagnosed if the Z-score value was found to be equal to or below −2.0. Results: Indications for the above tests were chronic immobilisation or motor activity restriction, and—in 10/34 children—femoral or vertebral fracture. Vitamin D deficiency (81%). Low bone mass on densitometry was demonstrated in 27/34 and osteoporosis in 10/34 subjects (Z-score ≤−2.0). Conclusion: Bone densitometry should be included in the standard of care for children with chronic neurological syndromes, and early detection of low bone mass should be an indication for treatment with calcium and vitamin D.

Objectives: Halliwick-based hydrotherapy has become one of the most widespread approaches in the water environment for people with disabilities. The Swimming with Independent Measure (SWIM) test was developed by Peackok (1993) based on the need to create specific aquatic assessment tests for people with disabilities. The purpose of our study was its translation into Greek language and the investigation of its metric properties. 目的脑性瘫痪(cerebral palsy)简称脑瘫,脑瘫的医学康复是至今尚未解决的难题。国际上有许多先进的康复训练(Rehabilitation-training)方法,但大部分脑瘫患儿经长期康复训练也难以治愈或完全生活自理。为了探讨针灸在脑瘫康复中脑的影像学改变,我们采用通督醒神针刺法治疗脑瘫患儿50例,并进行随机对照研究。以探讨通督醒神针刺法在脑瘫康复中的脑功能代偿作用。方法将2～7岁脑瘫患儿100例随机分为治疗组(50例)和对照组(50例),治疗组确50例,男32例、女18例,2～3岁26例,3～7岁24例,伴有智力低下者36例。头颅CT检查有脑萎缩和大脑额、颞叶发育不良等病变者39例。对照组男40例,女10例,2～3岁22例,3～7岁28例。伴有智力低下者39例。头颅CT检查有脑萎缩和大脑额、颞叶发育不良病变者43例。治疗组采用通督醒神针刺法,通督补肾组选穴:督脉穴13针配伍涌泉、阳陵泉、足三里、三阴交。和醒神头针(神庭透百会,百会透脑户和四神聪)。每周针刺2次,每针灸10次,休息日15天,针灸30次1个疗程。配合康复训练。对照组单纯用康复训练。康复训练主要用物理治疗(physitherapyPT),作业治疗(Ocupunctionaltherapy OT)。选用鲍巴斯(Bobath)法,以抑性手法抑制脑瘫患儿的异常姿势和异常姿势反射。促通脑瘫患儿的颈立直、坐位立直、站位立直和动态平衡。每天训练1～2小时,90天为一个疗程。观察两组临床疗效以及头颅CT、SPECT脑部病变恢复情况。疗效评定标准:显效:治疗后DQ提高15,头颅SPECT、C T复查脑萎缩、脑软化灶好转或正常;有效:治疗后DQ提高10,头颅SPECT、脑C T复查脑发育不良有好转;无效:治疗后DQ提高小于10,伴有异常的头颅C T和SPECT复查未见好转或正常。结果治疗组总有效率为84%,显著高于对照组总有效率的52%。治疗后发育商(DQ)治疗显著组高于对照组(P<0.01)。治疗组,治疗后头颅CT检查恢复正常9例,2例脑室傍软化灶恢复正常,7例脑皮层发育不良及轻度萎缩者恢复正常。头颅CT恢复率33.33%(13/39),对照组治疗后头颅CT的2例脑发育不良者恢复正常。CT恢复率4.65%(2/43),治疗组较对照组显著增高,X=11.257 P<0.001。治疗组治疗前28例脑部SPECT脑血流量灌注不足、神经元功能活性减低。治疗后27例恢复正常(占96.43%)。对照组治疗前29例脑部SPECT脑血流量灌流量不足、神经元功能活性减低,治疗后恢复正常6例(占20.69%)。SPECT恢复正常率治疗组高于对照组,X=33.5191 P<0.001。结论,本项影像学(CT/SPECT)研究结果表明,通督醒神针刺法可促进脑瘫患儿脑细胞功能重建。促进大脑的语言功能、运动功能、认知功能的代偿。针刺组的语言商,大动作发育商,精细动作发育商,个人社交和社会适应发育商治疗后较治疗前显著提高。现代医学研究证实针刺足三里、三阴交穴位中央后回及额下回、岛叶、顶下小叶、颞中回等脑部激活。针刺足三里、伏兔穴fMRI观察下丘脑、海马回、额回的神经生理兴奋平均血氧水平依赖(BolD)信号强度增加,针刺阳陵泉,额、颞叶、基底节、桥脑等部位平均BolD信号强度增强。针刺头皮运动区SPECT检查患者的局部脑血流灌注和脑细胞功能活动获得改善。针刺头穴百会与曲鬓可以升高或降低双侧大脑的运动功能区域中葡萄糖的代谢,从而诱导与运动相关的某些区域神经组织兴奋,补偿或协助受损的神经网络,促进运动功能重建。本研究表明针刺在脑瘫患儿中的多重修复作用,可能促进了神经胶质细胞的生长过程,使神经胶质细胞朝着有利于中枢神经细胞对神经冲动实施正常整合和调控的方向生长,有利于中枢神经元细胞逐步代偿受累肢体受损的中枢神经元细胞功能。
