High sensitive c-reactive protein, adiponectin, and urine albumin excretion rate in chinese coronary artery disease patients with different glucose tolerance status

Background Serum high sensitive c-reactive protein （hs-cRP）, adiponectin levels and urine albumin excretion rate （UAER） are probably associated with inflammation and atherosclerosis. The aim of this study was to determine the three markers in coronary artery disease （cAD） subjects with different glucose tolerance status in a chinese population and further explore the levels of the three markers in these subjects and the possible association of these markers with cAD risk factors and the severity of cAD as well. Methods A total of 242 subjects with angiographically documented cAD were recruited, and then assigned to three groups： the normal glucose tolerance （NGT）＋ cAD group, including 100 cAD patients with NGT; the impaired glucose tolerance （IGT）＋ cAD group, 40 cAD patients with IGT; the type 2 diabetes mellitus （T2DM）＋ cAD group, 102 cAD patients with T2DM. Serum hs-cRP, adiponectin levels as well as UAER were measured in all subjects. Results Serum hs-cRP levels were increased in the T2DM ＋ cAD group compared with the NGT ＋ cAD group （4.71±2.59） vs （3.60±2.46） mg/L, P=0.037. Serum adiponectin levels were gradually decreased from the NGT ＋ cAD to IGT ＋ cAD to T2DM ＋ cAD groups, （5.99±1.84）, （5.82±1.72） and （4.65±1.71） mg/L, P=0.002 and 0.040 for NGT ＋ cAD and IGT ＋ cAD groups vs T2DM ＋ cAD group, respectively. While the UAER was gradually increased from the NGT ＋ cAD to IGT ＋ cAD to T2DM ＋ cAD groups, （6.42±2.51）, （6.89±2.94） and （15.03±4.22） μg/min （P 〈0.001） for NGT ＋ cAD and IGT ＋ cAD groups vs T2DM ＋ cAD group. Multiple linear stepwise regression analysis showed that waist-hip ratio （WHR） and low density lipoprotein cholesterol （LDL-c） were the significant determinants of serum hs-cRP levels; triglyceride （TG）, high density lipoprotein cholesterol （HDL-c）, age, WHR, T2DM, 2-hour serum insulin （2hINS）, sex, and apolipoprotein B were the significant determinants of serum adiponectin levels; and systolic blood pressure （SBP）, T2DM and hemoglobin A1c （HbA1c） we

Increased plasma c-reactive protein level predicts rapid progression of non-target atherosclerotic lesions in patients with stable angina after stenting

Background Although the role of c-reactive protein （cRP） in predicting rapid progression of atherosclerotic lesions has been intensively studied in unstable coronary artery disease, the data from patients with stable angina （SA） are largely absent. The present study evaluated a middle-size patient cohort who underwent percutaneous coronary intervention （PcI） with stent implantation and follow-up coronary angiography （cAG） and tested the hypothesis that increased plasma level of high-sensitive cRP would indicate rapid progression of de novo non-target coronary artery lesions in chinese patients with *** The study population comprised of 311 consecutive patients with chronic SA who underwent coronary stent implantation on initial admission and angiographic follow-up （（8.5±1.2） months）. Rapid angiographic progression of non-target lesion was angiographically assessed and the patients were classified into two groups according to whether the progression existed or not. Increased serum levels of c-reactive protein and matrix metalloproteinase-9 in obstructive sleep apnea syndrome

Background Obstructive sleep apnea syndrome （OSAS）, characterized by intermittent hypoxia/reoxygenation （IHR）, has been identified as an independent risk factor for cardiovascular diseases （cVD）. The cVD biomarkers associated with OSAS have not been thoroughly investigated. Methods Fifty-one men with OSAS recently diagnosed by polysomnography were classified into two groups according to the severity of apnea： moderate to severe OSAS group （n=- 28） and mild OSAS group （n=- 23）. Twenty-five obese men, of comparable age and body mass index （BMI）, without OSAS were chosen as control subjects. Serum metabolic variables, c-reactive protein （cRP） and matrix metalloproteinase-9 （MMP-9） were measured. Spearman correlation and regression analysis were performed. Results Serum concentrations of cRP and MMP-9 were significantly higher in 51 OSAS patients than in 25 control subjects. Levels of cRP and MMP-9 were significantly higher in patients with moderate to severe OSAS than in patients with mild OSAS or in obese control subjects. A positive correlation was found between levels of cRP and MMP-9 in OSAS patients. Regression analysis showed that after adjusting for age and BMI, apnea/hypopnea index （AHI） significantly correlated with serum concentrations of cRP and MMP-9 in patients with OSAS. conclusions AHI, mirroring the frequency of IHR, was a predictor of enhanced circulating cVD biomarkers MMP-9 and cRP. Our data support the theory that IHR contributes to the upregulation of the inflammatory factors in OSAS patients.

Univariate and multivariate analysis of risk factors for severe clostridium difficile-associated diarrhoea: Importance of co-morbidity and serum c-reactive protein

AIM: To investigate risk factors for severe clostridium difficile associated diarrhoea (cDAD) in hospitalised patients. METHODS: We analysed risk factors for severe cDAD (associated with systemic signs of hypovolemia) in 124 hospitalised patients by retrospective chart review. RESULTS: Severe cDAD was present in 27 patients (22%). Statistical analysis showed a significant association with a higher 30-d mortality (33% vs 4%, P ceeding 14 d (74% vs 52%, P = 0.048). charlson co-morbidity score (OR 1.29 for 1 point increment, P c-reactive protein at diagnosis (OR 1.15 for 10 mg/L increment, P ctors of severe cDAD. cONcLUSION: Patients with a severe level of co- morbidity and high serum c-reactive protein levels at the time of diagnosis should receive particular attention.

Role of plasma c-reactive protein and white blood cell count in predicting in-hospital clinical events of acute type A aortic dissection

Background A few recent studies have reported that inflammation is associated with the prognosis of acute aortic dissection （AD）. There is, however, no systemic investigation regarding the role of plasma c-reactive protein （cRP） and white blood cell （WBc） levels in predicting in-hospital clinical events of acute type AAD. Methods The levels of high-sensitivity cRP and WBc counts were systemically determined after admission in 36 patients with acute type A AD. The variations of plasma cRP and WBc levels in different time windows （admission, 1, 2, 3, 4, 6, 8 days） in patients with acute type AAD were analyzed between patients with events and without events. Results During hospitalization, five patients died, and increased levels of cRP and WBc were found in patients died with acute type A AD compared with patients survived （P 〈0.01, respectively）. Medical treatment may significantly decrease inflammatory response in survived patients with acute type A AD. Additionally, patients with complication of pleural effusion showed higher cRP and WBc levers （P=0.046, P=-0.018, respectively）. Lower WBc levels were found in survived patients treated medically （P=-0.001）. Moreover, mean cRP and WBc levels had positive correlations with aortic diameter （r=0.364, P--0.000; r=0.333, P=0.000, respectively） and age （r=0.270, P=0.000, respectively）, while negative correlations with the time from onset of symptoms to hospital admission （r= -0.229, P=0.000, r= -0.200, P=0.002, respectively）. Univariate analysis showed that age 〉65 years, cRP zl 2.05 rag/L, WBc 〉12.16×10^9/L, aortic diameter 〉48 mm, pleural effusion and diastolic blood pressure 〉105 mmHg were associated with hospital mortality. Elevated plasma apolipoprotein AV in acute coronary syndrome is positively correlated with triglyceride and c-reactive protein

Background Increased triglyceride （TG） occurs in patients with acute coronary syndrome （AcS）, and apolipoprotein AV （apoAV） has been shown to lower TG levels. In the present study, we investigated plasma apoAV level and its relationship with TG and c-reactive protein （cRP） in AcS patients. Methods A total of 459 subjects were recruited and categorized into control group （n=-116）, stable angina （SA） group （n=115）, unstable angina group （n=116） and acute myocardial infarction group （n=112）. Plasma apoAV level was measured by a sandwich ELISA assay. Results compared with controls （（100.27±22.44） ng/ml）, plasma apoAV was decreased in SA patients （（76.54±16.91） ng/ml） but increased in patients with unstable angina （（330.89±66.48） ng/ml, P 〈0.05） or acute myocardial infarction （（368.66±60.53） ng/ml, P 〈0.05）. Inverse correlations between apoAV and TG were observed in the control or stable angina groups （r= -0.573 or-0.603, respectively, P 〈0.001）, whereas positive correlations were observed in the patients with unstable angina or acute myocardial infarction （r=0.696 or 0.690, respectively, P 〈0.001）. Furthermore, a positive relationship between apoAV and cRP was observed in the AcS patients but not in the non-AcS subjects. conclusion The plasma apoAV concentration is increased and positively correlates with TG and cRP in AcS patients.

A role for inflammation has become well established over the past decade or more in theories describing the atherosclerotic disease process. From a pathological viewpoint, all stages, ie, initiation, growth, and complication of the atherosclerotic plaque, might be considered to be an inflammatory response to injury. Several prospective studies recently showed that plasma high sensitivity c-reactive protein （hscRP） levels, which are one of the markers of systemic inflammation, are a powerful predictor of future myocardial infarction and cardiac death among apparently healthy individuals. However, the association between the plasma hscRP levels and the extent of coronary stenosis in subjects remains controversial. Some studies previously demonstrated such associations, whereas other could not found. Gensini＇s score assigns a severity score for a stenosed vessel depending on the degree of luminal narrowing and the importance of its location.
