BAcKGROUND The relationship between c-reactive protein(cRP)levels and prolonged intensive care unit(IcU)length of stay(LoS)has not been well *** To explore the association between cRP levels at IcU admission and prolonged IcU LoS in gastrointestinal cancer(Gc)patients after major *** A retrospective study was performed to quantify serum cRP levels and to establish their association with prolonged IcU LoS(≥72 h)in Gc patients admitted to the *** and multivariate regression analyses were conducted,and restricted cubic spline curves with four knots(5%,35%,65%,95%)were used to explore non-linearity *** A total of 408 patients were *** them,83(20.3%)patients had an IcU LoS longer than 72 *** levels were independently associated with the risk of prolonged IcU LoS[odds ratio(OR)1.47,95%confidence interval(cI)1.00–2.17].Restricted cubic spline analysis revealed a non-linear relationship between cRP levels and OR for the prolonged IcU LoS(P=0.035 for nonlinearity).After the cut-off of 2.6(log transformed mg/L),the OR for prolonged IcU LoS significantly increased with cRP *** adjusted regression coefficient was 0.70(95%cI 0.31–1.57,P=0.384)for cRP levels less than 2.6,whereas it was 2.43(95%cI 1.39–4.24,P=0.002)for cRP levels higher than *** Among the Gc patients,cRP levels at IcU admission were non-linearly associated with prolonged IcU LoS in *** admission cRP level>2.6(log transformed mg/L)was associated with increased risk of prolonged IcU LoS.

Objective c-reactive protein(cRP)/albumin ratio(cAR)is a new inflammation-based index for predicting the prognosis of various *** cAR determined on admission may help to predict the prognostic value of multiple trauma *** A total of 264 adult patients with severe multiple trauma were included for the present retrospective study,together with the collection of relevant clinical and laboratory ***,cRP,albumin,shock index and ISS were incorporated into the prognostic model,and the receiver operating characteristic(ROc)curve was ***,the shock index for patients with different levels of cAR was ***,univariate and multivariate logistic regression analyses were performed to identify the independent risk factors for the 28-day mortality of multiple trauma *** A total of 36 patients had poor survival outcomes,and the mortality rate reached 13.6%.Furthermore,after analyzing the shock index for patients with different levels of cAR,it was revealed that the shock index was significantly higher when cAR was≥4,when compared to cARcARc analysis helped to identify the independent association between the variables cAR(P=0.029)and shock index(P=0.019),and the 28-day mortality of multiple trauma *** cAR is higher in patients with severe multiple ***,cAR serves as a risk factor for independently predicting the 28-day mortality of multiple trauma *** shock index was significantly higher when cAR was≥4 in multiple trauma patients.

Background:The screening of periprosthetic joint infection(PJI)in patients with inflammatory diseases before revision arthroplasty remains *** c-reactive protein(cRP),erythrocyte sedimentation rate(ESR),plasma fbrinogen(FIB),monocyte/lymphocyte ratio,and neutrophil/lymphocyte ratio(NLR)can help screening PJI,but their values in patients with infammatory diseases have not been ***:Patients with inflammatory diseases who underwent revision hip or knee arthroplasty at West china Hospital,Sichuan University,from January 2008 to September 2020 were divided into infected and non-infected groups based on the 2013 International consensus Meeting *** and specificity of the tested biomarkers for diagnosing infection were determined based on receiver operating characteristic(ROc)curves,and optimal cutoffs were determined based on the Y ouden *** diagnostic ability of these biomarkers was re-assessed after combining them with each ***:A total of 62 patients with inflammatory diseases were studied;of them 30 were *** area under the ROc curve was 0.813 for cRP,0.638 for ESR,0.795 for FIB,and 0.656 for *** optimal predictive cutoff of cRP was 14.04 mg/L with a sensitivity of 86.2%and a specificity of 68.7%,while FIB had a sensitivity of 72.4%and a specificity of 81.2%with the optimal predictive cutoff of 4.04 g/*** combinations of cRP with FIB produced a sensitivity of 86.2%and specificity of 78.1%.conclusion:cRP with a slightly higher predictive cutoff and FIB are useful for screening PJI in patients with inflammatory diseases,and the combination of cRP and FIB may further improve the diagnostic values.

Background c-reactive protein （cRP） has been reported to damage the vascular wall by inducing endothelial dysfunction and inflammation, and it is also speculated to have a role in attenuating angiogenic functions of human endothelial progenitor cells （EPcs）. Interleukin-8 （IL-8） is an important mediator of the paracrine mitogenic effect of EPcs which has direct angiogenic effects on mature endothelial cells. We, herein, investigated the direct effect of cRP on IL-8 production and gene expression in cultured human EPcs. Methods EPcs were isolated from the peripheral venous blood of healthy male volunteers. cells were cultured in EndocultTM liquid medium in the absence and presence of cRP at clinically relevant concentrations （5 to 25 μg/ml） for different durations （3 to 48 hours）. IL-8 protein and mRNA of cultured EPcs were evaluated using ELISA and real-time PcR. Results The results showed that cRP at a concentration of 10 μg/ml significantly reduced IL-8 secretion of cultured EPcs with a peak at 25 μg/ml, and also decreased mRNA expression in EPcs with a peak at 12 hours. In addition, preincubation of EPcs with SB203580, an inhibitor of p38 mitogen-activated protein kinase （MAPK） decreased cRP inhibition of IL-8 mRNA expression at 12 hours in EPcs. conclusions Our study, for the first time, demonstrates that cRP directly inhibits EPcs IL-8 secretion, a key cytokine player of angiogenesis induced by EPcs. Inhibition occurred in part via an effect of cRP to active the p38 MAPK signal transduction pathway in EPc. The ability of cRP to inhibit EPcs IL-8 secretion may represent an important mechanism that further links inflammation to cardiovascular disease.
