Glioblastoma Multiforme(GBM)is a rapidly progressing brain *** the relatively low percentage of cancer patients with glioma diagnoses,recent statistics indicate that the number of glioma patients may have increased over the past *** therapeutic options for glioma patients include tumor resection,chemotherapy,and concomitant radiation therapy with an average survival of approximately 16 *** rapid progression of gliomas has spurred the development of novel treatment options,such as cancer gene therapy and oncolytic *** testing of oncolytic adenoviruses using glioma models revealed both positive and negative sides of the virotherapy *** we present a detailed overview of the glioma virotherapy field and discuss auxiliary therapeutic strategies with the potential for augmenting clinical efficacy of GBM virotherapy treatment.

Objective To investigate th e value of proton magnetic resonance spectroscopy ( 1H-MRS) on diagnosis a nd differential diagnosis of the intracranial diseases by the MRS results of 52 patients. Methods 12 patients with benign glioma, 16 patients with malignant glioma, 10 patients with meningioma, 8 patients with virus encephalitis, and 6 patients with cerebral infarction underwent MRS in th e lesion region. We measured the area within the spectra of N-acetyl-aspartate (NAA), creatine/phosphocreatine (Cr), choline compounds (Cho), and lactate (Lac ). Results The spectra of meningiomas were characterized by abs ence of NAA. The spectra of gliomas were characterized by the decrease of NAA an d Cr, but the increase of Cho. The ratio of Cho to Cr was 2.25±1.21 in benign g liomas, while the ratio of Cho to Cr was 4.65±2.21 in malignant gliomas. The sp ectra of virus encephalitis appeared the decrease of NAA and the normality of Cr , with the 1.25±0.21 ratio of Cho/Cr. The apparent Lac wave could be seen in al l cerebral infarctions. Conclusion The value of 1H-MRS plays a significant role in the diagnosis and differential diagnosis of gliomas, mening iomas, virus encephalitis, and cerebral infarctions.

A new scoring system for predicting extent of resection in medial sphenoid wing meningiomas based on three-dimensional multimodality fusion imaging

Background: Three-dimensional (3D) fusion imaging has been proved to be a promising neurosurgical tool for presurgical evaluation of tumor removal. We aim to develop a scoring system based on this new tool to predict the resection grade of medial sphenoid wing meningiomas (mSWM) ***: We included 46 patients treated for mSWM from 2014 to 2019 to evaluate their tumors'' location, volume, cavernous sinus involvement, vascular encasement, and bone invasion by 3D multimodality fusion imaging. A scoring system based on the significant parameters detected by statistical analysis was created and ***: The tumor volumes ranged from 0.8 cm3 to 171.9 cm3. A total of 39 (84.8%) patients had arterial involvement. Cavernous sinus (CS) involvement was observed in 23 patients (50.0%) and bone invasion was noted in 10 patients (21.7%). Simpson I resection was achieved in 10 patients (21.7%) and Simpson II resection was achieved in 17 patients (37.0%). Fifteen patients (32.6%) underwent Simpson III resection and 4 patients (8.7%) underwent Simpson IV resections. A scoring system was created. The score ranged from 1 to 10 and the mean score of our patients was 5.3 ± 2.8. Strong positive monotonic correlation existed between the score and resection grade (Rs = 0.772,P < 0.001). The scoring system had good predictive capacity with an accuracy of 69.60%.Conclusions: We described a scoring system that enabled neurosurgeons to predict extent of resection and outcomes for mSWM preoperatively with 3D multimodality fusion *** registration: Retrospectively registered

The Evaluation of Efficacy and Side Effects of Radiotherapy in Cerebral Meningiomas

Aims: The current study aimed to evaluate the efficacy, side effects of radiotherapy and factors influencing treatment outcome in patients received radiotherapy for cerebral meningiomas. Methods and Material: In the last two decades, a total of 35 patients with cerebral meningioma who received radiotherapy in our clinic were evaluated statistically in terms of survival, toxicity and prognostic factors. The records of patients diagnosed with cerebral meningioma who underwent postoperative radiotherapy were retrospectively analyzed. Statistical analysis used: Statistical comparisons were made using IBM SPSS v24.0. Kaplan-Meier method was used to compare survival times. Results: Five of the patients had grade I meningioma showing recurrence, 18 were grade II meningioma and 12 were grade III meningioma. Patients were treated with 54 or 60 Gy adjuvant radiotherapy with 2 Gy daily fractions according to histopathological grade and operation type. Three and five-year overall survival rates were 56% and 40%, respectively and median overall survival was 36 months. Progression was observed in 15 of the 35 patients included in the study. Three and five-year progression-free survival (PFS) rates of the patients were 56% and 50%, respectively. There was a statistically significant correlation between histopathological grade and overall survival among patients. However;no statistical difference found in overall survival of patients in terms of tumor location, operation type and age. Conclusions: In the treatment of cerebral meningiomas, changes in radiotherapy dose and field designs can be predicted according to the tumor grade and operation type. In addition, it is thought that large scale studies are needed to determine prognostic factors more meticulously.

Glioma,the most frequent primary brain tumor in adults,is a highly infiltrative tumor exhibiting resistance to most treatments and associated with short survival of *** emission tomography(PET)imaging using various tracers takes advantage of the increased metabolic rate of neoplastic cells,in order to detect tumors and validate the treatment *** most frequently used PET tracer,the(18)F-fluorodeoxyglucose(FDG),is useful during the initial and follow-up assessment of patients with gliomas because it can assist in the selection of the initial biopsy site and to assess early response to a given therapeutic ***,when there is tumor re-growth after an initial remission,FDG-PET can differentiate between true tumor recurrence versus necrosis from radiation *** developed PET tracers may exhibit better sensitivity than FDG to diagnose primary brain tumors,but may occasionally produce false positive results in various *** any event,PET is a useful tool in patients with central nervous system cancer during both initial assessment and follow-up.

A logarithmic spiral is a self-similar spiral curve, which often appears in nature, e.g., mollusk shells. In the normal tissues of the human body, the cochlea is also an approximate logarithmic spiral. However, approximate logarithmic spirals are rarely,if ever, reported in tumors. Recently, we treated a 26-year-old woman with a one-year history of headache. Preoperative magnetic resonance imaging showed a possible brain tumor with radiological characteristics similar to a typical logarithmic spiral,which has seldom been observed before. Because of the obvious clinical symptoms and discomfort, as well as the possible presence of an intracranial tumor, we performed a craniotomy and tumor resection. Perioperatively, we found an intact tumor capsule,with contents that appeared like egg-drop soup mixed with hairs. A post-operative pathological diagnosis of a dermoid cyst was made. However, more interestingly, the logarithmic spiral could be described mathematically with a high goodness of fit. In this paper, we present this case with various clinical images.

Glioblastoma multiforme is the most common type of primary central nervous system tumor and is noted for its short survival and poor response to chemotherapeutic ***,the relapse rate is very high,and there is no reference drug for second-line *** this study,a patient was treated with the Soffi etti *** induction phase was fotemustine 75 mg/m^(2) at day 1 and day 8 and bevacizumab 10 mg/kg at day 1 and day *** maintenance phase was fotemustine 75 mg/m^(2) and bevacizumab 10 mg/kg every 3 weeks for two ***-up magnetic resonance imaging showed post-surgical changes at the left occipital level,without contrast enhancement,and toxic left leuko-encephalopathy post-treatment without mass effect and with no evidence of tumor *** patient then was maintained with bevacizumab monotherapy until it was withdrawn when pulmonary thromboembolism *** tumor regrowth,fotemustine was started again as maintenance *** patient achieved stabilization of his disease until his death due to thromboembolic and infectious complications.
