Gold nanorods and nanohydroxyapatite hybrid hydrogel for preventing Bone tumor recurrence via postoperative photothermal therapy and bone regeneration promotion

Osteosarcoma is a malignant Bone tumor,which often occurs in adolescents.However,surgical resection usually fails to completely remove the tumor clinically,which has been the main cause of postoperative recurrence and metastasis,resulting in the high death rate of patients.At the same time,osteosarcoma invades a large area of the bone defect,which cannot be self-repaired and seriously affects the life quality of the patients.Herein,a bifunctional methacrylated gelatin/methacrylated chondroitin sulfate hydrogel hybrid gold nanorods(GNRs)and nanohydroxyapatite(nHA),which possessed excellent photothermal effect,was constructed to eradicate residual tumor after surgery and bone regeneration.In vitro,K7M2wt cells(a mouse Bone tumor cell line)can be efficiently eradicated by photothermal therapy of the hybrid hydrogel.Meanwhile,the hydrogel mimics the extracellular matrix to promote proliferation and osteogenic differentiation of mesenchymal stem cells.The GNRs/nHA hybrid hydrogel was capable of photothermal treatment of postoperative tumors and bone defect repair in a mice model of tibia osteosarcoma.Therefore,the hybrid hydrogel possesses dual functions of tumor therapy and bone regeneration,which shows great potential in curing Bone tumors and provides a new hope for tumor-related bone complex disease.

The treatment of bone defect after Bone tumor resection is a great challenge for orthopedic surgeons.It should consider that not only to inhibit tumor growth and recurrence,but also to repair the defect and preserve the limb function.Hence,it is necessary to find an ideal functional biomaterial that can repair bone defects and inactivate tumor.Magnetic nanoparticles(MNPs)have its unique advantages to achieve targeted hyperthermia to avoid damage to surrounding normal tissues and promote osteoblastic activity and bone formation.Based on the previous stage,we successfully prepared hydroxyapatite(HAP)composite poly(lactic-co-glycolic acid)(PLGA)scaffolds and verified its good osteogenic properties,in this study,we produced an HAP composite PLGA scaffolds modified with MNPs.The composite scaffold showed appropriate porosity and mechanical characteristics,while MNPs possessed excellent magnetic and thermal properties.The cytological assay indicated that the MNPs have antitumor ability and the composite scaffold possessed good biocompatibility.In vivo bone defect repair experiment revealed that the composite scaffold had good osteogenic capacity.Hence,we could demonstrate that the composite scaffolds have a good effect in bone repair,which could provide a potential approach for repairing bone defect after Bone tumor excision.

A comparative study of calcium sulfate artificial bone graft versus allograft in the reconstruction of bone defect after tumor curettage

Background Cavity reconstruction after benign Bone tumor removal is varied and controversial.AIIograft is widely used but is associated with complications.New bone substitutes,such as calcium sulfate artificial bone,have been introduced for Bone tumor operation.However,the bone healing response of artificial bone has not been compared with allograft bone.We therefore compared calcium sulfate grafts （study group） with bone allografts （control group） for the treatment of benign Bone tumors.Methods We retrospectively reviewed 50 patients who underwent calcium sulfate reconstruction and 50 patients who underwent allograft cancellous bone reconstruction.The two groups were well matched.The mean follow-up time of the study group was 19.9 （12-55） months.We investigated bone healing response,complications,and factors affecting bone healing.Results At the last follow-up,84％（42/50） of cases in the study group and 62％（31/50） of cases in the control group had achieved clinical healing （P=0.013）.The initial healing rate showed no significant difference between the two groups （100％ vs.96％,P=0.153）.The mean healing times for calcium sulfate and allograft bone were 9.6 （3-42） months and 13.8 （3-36） months,respectively （P ＜0.01）.Complications in the study group were minor and resolved.Implant volume was a significant factor affecting bone healing.Conclusion The calcium sulfate bone substitute showed a satisfactory healing outcome and safety profile in reconstruction of bone defects after benign Bone tumor curettage,especially in smaller cavities.

Primary malignant Bone tumors can be life-threatening.Surgical resection of tumor plus chemotherapy is the standard clinical treatment.However,postoperative recovery is hindered due to tumor recurrence caused by residual tumor cells and bone defect caused by resection of tumor tissue.Herein,a multifunctional mussel-inspired film was fabricated on Mg alloy,that is,an inner hydrothermal-treated layer,a middle layer of polydopamine,and an outer layer of doxorubicin.The modified Mg alloy showed excellent photothermal effect and thermal/pH-controlled release of doxorubicin.The synergistic effect of chemotherapy and photothermal therapy enabled the modified Mg alloy to kill Bone tumor in vitro and inhibit tumor growth in nude mice.Moreover,because of the controlled release of Mg ions and biocompatibility of polydopamine,the modified Mg alloy supported extracellular matrix mineralization,alkaline phosphatase activity,and bone-related gene expression in C3H10T1/2.bone implantation model in rats verified that the modified Mg showed excellent osteointegration.These findings prove that the use of mussel-inspired multifunction film on Mg alloy offers a promising strategy for the therapy of primary malignant Bone tumor.

Ultrasonographic Diagnosis of Bone tumor of the Knee and Its Clinical Implication

In order to evaluate the value of the ultrasonography in the diagnosis of tumor of the knee and its clinical implication, 67 patients with clinically suspected Bone tumor of the knee were examined by ultrasound. The ultrasonographic characteristics of different Bone tumors were studied and compared with the results of pathologic characters after operation. Ultrasonography can readily visualize the bony destruction and the pathologic change of the periosteum and the soft tissue related to Bone tumor. Fifty-two cases of malignant Bone tumors and 15 cases of giant cell tumors were diagnosed by ultrasonography. Pathologically, there were 54 cases of malignant Bone tumor and 13 cases of giant cell tumor. It was concluded that ultrasonographic examination might be a useful method for the diagnoses of Bone tumor of the knee and play an important role in guiding needle biopsy and electing operative method and approach.

Bone tumors occur in bone or its accessory tissues.Benign Bone tumors are easy to cure and have good prognosis,while malignant Bone tumors develop rapidly and have poor and high mortality.So far,there is no satisfactory treatment method.Here,we designed a universal template vector for Bone tumor therapy that simultaneously meets the needs of bone targeting,tumor killing,osteoclast suppression,and tumor imaging.The template is composed of a polydopamine(PDA)core and a multifunctional surface.PDA has excellent biosafety and photothermal performance.In this study,alendronate sodium(ALN)is grafted to enable its general bone targeting function.PDA core can carry a variety of chemotherapy drugs,and the rich ALN group can carry a variety of metal ions with an imaging function.Therefore,more personalized treatment plans can be designed for different Bone tumor patients.In addition,the PDA core enables photothermal therapy and enhanced chemotherapy.Through template drug Doxorubicin(DOX)and template imaging ion Fe(II),we systematically verified the therapeutic effect,imaging effect,and inhibition of bone dissolution of the agent on Osteosarcoma(OS),a primary malignant Bone tumor,in vivo.In conclusion,our work provides a more general template carrier for the clinical treatment of Bone tumors,through which personalized treatment of Bone tumors can be achieved.

Three-dimensional digital technology-assisted precise tumor resection and reconstruction of the femoral trochanter and postoperative functional recovery:a retrospective study

Background The trochanter of the femur is a common site for Bone tumors.However,locating the specificboundary of Bone tumor infiltration and determining the surgical method can be challenging.The objective of thisstudy was to review the diagnosis,treatment,and surgical outcomes of patients with tumors or tumor-like changesin the femoral trochanter after computer-assisted precise tumor resection and hip-preserving reconstruction ofthe trochanter.Methods From January 2005 to September 2020,11 patients with trochanteric tumors(aged:18–53 years;sixmales and five females)were treated in Guangzhou First People’s Hospital.The cases included aneurysmal bonecyst(n=1),giant cell tumor of bone(n=2),fibrous histiocytoma of bone(n=1),endochondroma(n=1),andfibrous dysplasia of bone(n=6).For patients with trochanteric tumors,computed tomography and magnetic resonance imaging scanning were performed before operation to obtain two-dimensional image data of the lesion.Athree-dimensional digital model of bilateral lower limbs was reconstructed by computer technology,the boundary of tumor growth was determined by computer simulation,the process of tumor resection and reconstructionwas simulated,and the personalized guide template was designed.During the operation,the personalized guideplate guided the precise resection of the tumor,and the allogeneic bone was trimmed to match the shape of thebone defect.Results All 11 patients underwent accurate resection of the tumor or tumor-like lesion and reconstruction ofthe hip.In eight cases,the lesion was confined to the trochanter,which was fixed with large segment allogeneicbone,autologous iliac bone,and proximal femoral anatomic plate.In three cases,allogeneic bone,autologousiliac bone,and femoral reconstruction nail were used to fix the tumor under the trochanter.Postoperative Xray examination showed that the repair and reconstruction of the bone defect was effective,and callus bridgingbetween the allogenic bone and autogenous bone was observed 6 months after operation.All patients recov

MicroRNAs(miRNAs)are small single-stranded,non-coding RNA molecules involved in the pathogenesis and progression of cancer,including osteosarcoma.We aimed to clarify the pathways involving miRNAs using new bioinformatics tools.We applied WikiPathways and PathVisio,two open-source platforms,to analyze miRNAs in osteosarcoma using miRTar and ONCO.IO as integration tools.We found 1298 records of osteosarcoma papers associated with the word“miRNA”.In osteosarcoma patients with good response to chemotherapy,miR-92a,miR-99b,miR-193a-5p,and miR-422a expression is increased,while miR-132 is decreased.All identified miRNAs seem to be centered on the TP53 network.This is the first application of PathVisio to determine miRNA pathways in osteosarcoma.MiRNAs have the potential to become a useful diagnostic and prognostic tool in the management of osteosarcoma.PathVisio is a full pathway editor with the potentiality to illustrate the biological events,augment graphical elements,and elucidate all the physical structures and interactions with standard external database identifiers.

Magnetic Resonance Imaging for the Assessment of Long Bone tumors

Background： Wide resection margins of osseous tumors are associated with a low incidence of local recurrence, making accurate measurement of the intraosseous extent of primary malignant long Bone tumors is crucial. We compared the intraosseous tumor extent assessed by magnetic resonance imaging （MRI） with the gross specimen to evaluate the accuracy of MRI. Methods： A total of 255 patients with primary malignant tumors in the long bones were included. Using MRI, we defined the length of tumor as the distance from the articular surface to the boundary between abnormal and normal marrow signal. The extent of the abnormal intraosseous signal was measured on unenhanced T1-weighted （T1WI） magnetic resonance images after chemotherapy. All gross surgical specimens were sectioned, and tumor extent was measured. Wilcoxon signed-rank test was used to test the differences between MRI and gross specimen findings. Spearman＇s correlation analysis was used to test the correlation between groups. Results： Median tumor length by gross specimen （112 mm; range, 45–300 mm） was longer than that by MRI （108 mm; range, 45–304 mm;Z = -6.916, P 〈 0.001）. Of 255 images, tumor length was accurately represented on 27 T1WI magnetic resonance images, overestimated on 79 images, and underestimated on 149 images. The median difference between imaging and gross specimen measurements was 2.0 mm （range： 1.0–15.0 mm） for the 79 cases where tumor length was overestimated, and 5.0 mm （range： 1.0–18.0 mm） for the 149 cases where tumor length was underestimated. The Spearman correlation demonstrated a high correlation of tumor length on gross specimen with the tumor length on MRI （R = 0.99, P 〈 0.01）. Conclusions： We conclude that preoperative MRI could be a useful method in determining intramedullary malignant Bone tumor boundaries and may serve as an accepted assessment method of long Bone tumors before limb-sparing surgery.
