Is adjuvant chemotherapy necessary for early gastric cancer?

Objective:To quantify the potential benefit of adjuvant chemotherapy(ACT)with respect to survival,and to identify factors for predicting prognoses in early gastric cancer ***:Patients with pT1 gastric cancer(GC)who underwent radical resection with D2 lymphadenectomy were retrospectively *** on lymph node metastasis(LNM)status and treatment regimens,patients were classified into groups,and clinicopathological variables,overall survival(OS),and disease-specific survival(DSS)were ***:Of 1,050 enrolled patients,151 patients(14.4%)had a positive LNM *** invasion,undifferentiated state,tumor size>2 cm,ulceration,and lymphovascular invasion were independent risk factors for LNM using multivariate *** 5-year OS of all patients was 96.4%.HER2 positive,perineural invasion,and LNM were independent factors for worse *** with pT1N3 GC had a worse 5-year OS and DSS than pT1N0,pT1N1,and pT1N2 patients(P<0.001).The 5-year OS and DSS for pT1N1 patients showed no significant difference between ACT and surgery only *** pT1N2 patients,the 5-year OS and DSS showed no significant difference between S-1 and Xelox *** pT1N3 patients,7(36.8%)received S-1,while 12(63.2%)received Xelox *** receiving Xelox treatment showed a better 5-year OS(75.0%vs.14.3%)and DSS(81.8%vs.20.0%)than patients receiving S-1(P<0.05).Conclusions:Curative surgery only was adequate for patients with pT1N0 and *** showed no survival benefits for pT1N2 ***,S-1 is the optimal choice for pT1N2 patients,when considering adverse *** is recommended for pT1N3 patients.

The development of wearable and portable electronics calls for flexible piezoelectric materials to fabricate selfpowered ***,a big challenge in piezoelectric material design is to boost the output performance while ensuring its flexibility and ***,all-organic and core-shell structured silk fibroin(SF)/poly(vinylidene difluoride)(PVDF)piezoelectric nanofibers(NFs)with excellent flexibility are fabricated using a simple electrospinning *** strong intermolecular interaction between SF and PVDF promotes theβ-phase nucleation in the core-shell structure,which significantly enhances the output *** output of 16.5 V was achieved in SF/PVDF NFs,which is more than 6-fold enhancement compared with that of pure PVDF *** addition,the piezoelectric device can sensitively detect the mechanical stimulation from joint bending,demonstrating its great potential in self-powered ***,the piezoelectric device can be also applied to control the movement of a smart car,successfully,achieving its application in the human-machine interaction.

Tumor heterogeneity of gastric cancer: From the perspective of tumor-initiating cell

Gastric cancer(GC) remains one of the most common and malignant types of cancer due to its rapid progression, distant metastasis, and resistance to conventional chemotherapy, although efforts have been made to understand the underlying mechanism of this resistance and to improve clinical outcome. It is well recognized that tumor heterogeneity, a fundamental feature of malignancy, plays an essential role in the cancer development and chemoresistance. The model of tumor-initiating cell(TIC) has been proposed to explain the genetic, histological, and phenotypical heterogeneity of GC. TIC accounts for a minor subpopulation of tumor cells with key characteristics including high tumorigenicity, maintenance of self-renewal potential, giving rise to both tumorigenic and non-tumorigenic cancer cells, and resistance to chemotherapy. Regarding tumor-initiating cell of GC(GATIC), substantial studies have been performed to(1) identify the putative specific cell markers for purification and functional validation of GATICs;(2) trace the origin of GATICs; and(3) decode the regulatory mechanism of GATICs. Furthermore, recent studies demonstrate the plasticity of GATIC and the interaction between GATIC and its surrounding factors(TIC niche or tumor microenvironment). All these investigations pave the way for the development of GATIC-targeted therapy, which is in the phase of preclinical studies and clinical trials. Here, we interpret the heterogeneity of GC from the perspectives of TIC by reviewing the above-mentioned fundamental and clinical studies of GATICs. Problems encountered during the GATIC investigations and the potential solutions are also discussed.

Neural Firing Mechanism Underlying Two-Electrode Discrimination by 3D Transcutaneous Electrical Nerve Stimulation Computational Model

Transcutaneous electrical nerve stimulation(TENS) has been widely used for sensory feedback which is a key consideration of improving the performance of prosthetic hands. Two-electrode discriminability is the key to realize high-spatial-resolution TENS, but the neural firing mechanism is not clear yet. The goal of this research is to investigate the neural firing patterns under two-electrode stimulation and to reveal the potential mechanisms. A three-dimensional(3 D) model is established by incorporating Aβ fiber neuron clusters into a layered forearm structure. The diameters of the stimulating electrodes are selected as 5, 7, 9 and 12 mm, and the two-electrode discrimination distance(TEDD) is quantified. It is found that a distant TEDD is obtained for a relatively large electrode size, and 7 mm is suggested to be the optimal diameter of stimulating electrodes. The present study reveals the neural firing patterns under two-electrode stimulation by the 3 D TENS model. In order to discriminate individual electrodes under simultaneous stimulation, no crosstalk of activated Aβ fibers exists between two electrodes. This research can further guide the optimization of the electrode-array floorplan.

Ultra-high conductive 3D aluminum photonic crystal as sulfur immobilizer for high-performance lithium-sulfur batteries

The practical application of lithium-sulfur (Li-S) batteries is hampered by the insulative nature of sulfur, sluggish electrochemical kinetics, and large volume variation, which result in capacity-fading at a large current density and poor cycling stability. Herein, a three-dimensional (3D) aluminum photonic crystal encapsulating sulfur (APC@S) composite as a binder-free cathode for Li-S battery is reported. The 3D APC@S cathode can deliver a fantastic capacity of 1,517.8 mAh·g^(−1) at 0.5 C, and retains 712.7 mAh·g^(−1) after 1,500 cycles at 2 C with a decay rate of 0.02% per cycle. Even at a high rate of 5 C, the reversible capacity can still maintain at 680.7 mAh·g^(−1) after 1,000 cycles with a capacity retention of 74.8%. Furthermore, the assembled soft-packaged Li-S battery also exhibits high reversible capacity and stable cycling performance. The excellent electrochemical performance is attributed to the 3D hierarchical and continuously porous structure and high conductive aluminum-wall, which can effectively trap polysulfides, confine the volume expansion on cycling, and accelerate electron/ion transfer. It is expected that this high conductive metal cathode has a great future for the application of energy storage devices.
