Prognostic significance of cytokine receptor-like factor 2 alterations in acute lymphoblastic leukemia:a meta-analysis

Background:Cytokine receptor-like factor 2(CRLF2)has been shown to play a role in the pathogenesis of acute lymphoblastic leukemia(ALL).Studies have examined the relationship between CRLF2 alterations such as over-expression or deregulation and clinical outcome in childhood ALL,but the results are *** metaanalysis aimed to explore the association between CRLF2 alterations and survival of pediatric patients with ***:Electronic databases updated to March 2014 were searched for relevant studies.A meta-analysis was made of twelve studies including 5945 patients to evaluate the prognostic significance of CRLF2 alterations on survival in childhood *** ratios(HRs)with 95%confidence intervals(CIs)were pooled across the studies using a fixed-effects ***:CRLF2 over-expression in childhood ALL was associated with poor prognosis in terms of relapse-free survival(RFS;HR=1.70,95%CI=1.28-2.24,P=0.000),event-free survival(EFS;HR=1.78,95%CI=1.05-3.01,R=0.032),and overall survival(OS;HR=2.28,95%CI=1.42-3.65,R=0.001).The combined data also suggested that CRLF2 deregulation in childhood ALL was correlated with poor EFS(HR=1.95,95%CI=1.46-2.61,R=0.000),RFS(HR=2.20,95%CI=1.53-3.18,P=0.000),and OS(HR=1.89,95%CI=1.24-2.87,P=0.003).Subgroup analysis on multivariate HRs showed that CRLF2 deregulation independently predicted a poor prognosis for childhood ***:The present meta-analysis reveals that both CRLF2 over-expression and deregulation are associated with poor prognosis in pediatric patients with ALL.

An Epistaxis Emergency Associated with Multiple Pollutants in Elementary Students

Emergencies of epistaxis in students caused by environmental pollution have rarely been reported to date. This study aimed to explore the cause of an emergency of epistaxis in elementary students by using a field epidemiological investigation. Twenty-two epistaxis cases from a single school with differences in gender, age, and classroom,were diagnosed within a period of 7 days. The air concentration of chromic acid mist （Cr6~） in the electroplating factory area, new campus, and residential area exceeded the limit of uncontrolled emissions. The emission of HCL and HzSO4was also observed. Formaldehyde levels in the classrooms exceeded the limits of indoor air quality. Abnormal nasal mucosa was significantly more frequent in the case group （93.3%） and control group 1 （of the same school）（66.7%） than in control group 2 （from a mountainous area with no industrial zone）（34.8%; P 〈 0.05 and P 〈 0.01, respectively）. On the basis of the pre-existing local nasal mucosal lesions, excessive chromic acid mist in the school＇s surrounding areas and formaldehyde in the classrooms were considered to have acutely irritated the nasal mucosa, causing epistaxis. Several lessons regarding factory site selection, eradication of chemical emissions, and indoor air quality in newly decorated classrooms, should be learned from this emergency.

AIM:To report a novel splicing mutation in the RPGR gene(encoding retinitis pigmentosa GTPase regulator)in a three-generation Chinese family with X-linked retinitis pigmentosa(XLRP).METHODS:Comprehensive ophthalmic examinations including best corrected visual acuity,fundus photography,vision field,and pattern-visual evoked potential were performed to identify the disease phenotype of a six-yearold boy from the family(proband).Genomic DNA was extracted from peripheral blood of five available members of the ***-exome sequencing(WES),Sanger sequencing,and pSPL3-based exon trapping were used to investigate the aberrant splicing of *** Splice Finder v3.1 and NNSPLICE v0.9 were used for in silico prediction of splice site ***:The proband was diagnosed as having retinitis pigmentosa(RP).He had severe symptoms with early onset.A novel splicing mutation,c.619+1G>C in RPGR was identified in the proband by WES and in four family members by Sanger *** splicing assays verified that c.619+1G>C in RPGR would result in the formation of a damaging alternative transcript in which the last 91 bp of exon 6 were skipped,leading to the subsequent deletion of 623 correct amino acids(c.529_619del ***177Glnfs*16).CONCLUSION:We identify a novel splice donor site mutation causing aberrant splicing of *** findings add to the catalog of pathological mutations of RPGR and further emphasize the functional importance of RPGR in RP pathogenesis and its complex clinical phenotypes.

Clinical application of extracorporeal membrane oxygenation in children with refractory cardiopulmonary failure

Background:This study aimed to discribe the experience in supporting children with refractory cardiopulmonary failure with extracorporeal membrane oxygenation(ECMO).Methods:We retrospectively reviewed 12 children with refractory cardiopulmonary failure supported with ECMO from February 2009 to August 2015 in the Pediatric Intensive Care Unit(PICU),Children's Hospital,zhejiang University School of ***:Seven of the 12 patients were weaned successfully from ECMO and dischaged from the hospital,with a survival rate of 58.3%(7/12).Among them,fi ve patients had acute fulminant myocarditis(AFM).Complications during ECMO included hemorrhage,hemolysis,thrombosis,acute kidney injury,and secondary hematogenous *** 1-24 month follow-up,the seven surviving patients recovered with normal cardiopulmonary ***:ECMO is useful for supporting children with refractory cardiopulmonary failure,especially for treatment of AFM.

Relationship between corneal biomechanical parameters and corneal sublayer thickness measured by Corvis ST and UHR-OCT in keratoconus and normal eyes

Use of the ulcerative colitis endoscopic index of severity and Mayo endoscopic score for predicting the therapeutic effect of mesalazine in patients with ulcerative colitis

Objective:The ulcerative colitis endoscopic index of severity(UCEIS)and the Mayo endoscopic score(MES)are developed as objective methods of evaluating endoscopic severity in patients with ulcerative colitis(UC).The aim of this study is to investigate the diagnostic accuracy of the UCEIS and MES in predicting the patient's response to ***:Consecutive patients with UC who had undergone colonoscopy within 1 month before starting mesalazine between October 2011 and July 2016 were retrospectively collected at the Department of Gastroenterology,Sir Run Run Shaw Hospital,zhejiang University School of *** median follow-up was 81 months,and all the data were analyzed in January *** primary outcome was the need for step-up treatment,which included the use of corticosteroids,immunomodulatory,or surgery during admission and *** were analyzed using the c2 or Fisher exact test,Spearman test,t-test,and ManneWhitney U ***:Totally,65 patients were enrolled,of whom 12(18.5%)needed step-up treatment due to nonresponse to *** UCEIS score,MES,and the ulcerative colitis disease activity index(UCDAI)score were significantly higher in patients who had nonresponse to mesalazine(UCEIS score:6.92±0.69 vs.4.45±1.17,p<0.001;MES:2.67±0.49 vs.2.15±0.69,p=0.024;UCDAI score:9.33±1.87 vs.6.70±2.38,p=0.002).In the multivariate analysis,the UCEIS score(OR=25.65,95%CI:3.048 e45.985,p=0.003),UCDAI score(OR=1.605,95%CI:1.144e2.254,p=0.006),and C-reactive protein level(OR=1.056,95%CI:1.006e1.108,p=0.026)were independent risk factors of *** area under the ROC curve of UCEIS was 0.95,with a sensitivity of 100%and specificity of 84.6%,a cut-off value of 6,which outperformed the MES with an area under the ROC curve of *** the UCEIS score≥6,60%of patients eventually needed step-up ***:The UCEIS is a useful instrument for predicting the therapeutic effect in patients with UC treated with *** high probabili
