The Mechanism of Cortico-Striato-Thalamo-Cortical Neurocircuitry in Response Inhibition and Emotional Responding in Attention Deficit Hyperactivity Disorder with Comorbid Disruptive Behavior Disorder

The neurocircuitries that constitute the corticostriato-thalamo-cortical（CSTC） circuit provide a framework for bridging gaps between neuroscience and executive function in attention deficit hyperactivity disorder（ADHD）, but it has been difficult to identify the mechanisms for regulating emotional problems from the understanding of ADHD comorbidity with disruptive behavior disorders（DBD）. Research based on ＂cool＇＇ and ＂hot＇＇executive functional theory and the dual pathway models,which are thought of as applied response inhibition and delay aversion, respectively, within the neuropsychological view of ADHD, has shed light on emotional responding before and after decontextualized stimuli, while CSTC circuit-related domains have been suggested to explain the different emotional symptoms of ADHD with or without comorbid DBD. This review discusses the role of abnormal connections in each CSTC circuit, especially in the emotion circuit, which may be responsible for targeted executive dysfunction at the neuroscience level. Thus, the two major domains – abstract thinking（cool） and emotional trait（hot） – trigger the mechanism of onset of ADHD.

In this paper, the development of existing artificial tracheal stents was briefly introduced. Their structures, strengths and weaknesses were analyzed. A new woven artificial tracheal stent that used leno-based construction was designed and fabricated at last. Additionally, a preliminary mechanical study was carried out on the new artificial tracheal stent, such as the radial supporting force and the elastic restoring force, making foundation for application of the new artificial tracheal stent.

Brief ICF Core Set for Obstructive Pulmonary Diseases: Validation from the Perspective of Chinese Respiratory Physicians

Obstructive pulmonary diseases(OPDs)are the leading causes of mortality and disability *** International Classification of Functioning,Disability and health(ICF)provides a framework for systemat-ically assessing functioning and disability in patients with chronic *** date,the ICF core sets for OPDs have not been validated by respiratory physicians in China’s *** study was aimed at validating the brief ICF core set for OPDs from the perspective of Chinese respiratory physicians.A three-round,consensus-building survey was conducted with Chinese respiratory physicians from shanghai,Jiangsu,and Zhejiang from December 2 to December 16,2017,using the Delphi *** answers were analyzed for consensus *** this study,52 experts(about 67.3%men)with a mean working experience of(19.3±6.3)years completed the consensus *** 3 rounds,the following 13 ICF categories were considered crucial for the assessment of OPDs,with an agreement of over 70%:4 in body functions(b410-heart functions,b440-respiratory functions,b455-exercise tolerance functions,and b460-sensations associated with cardiovascular and respiratory functions),2 in body structures(s410-structure of cardiovascular system,and s430-structure of respiratory system),4 in activities and participation(d230-carrying out daily routine,d450-walking,d455-moving around,and d640-doing housework),and 3 in environmental factors(e110-products or substances for personal consumption,e225-climate,and e260-air quality).Twelve(70.6%)categories in the current brief ICF core set were validated,and one additional category,b410-heart functions,was *** conclusion,Chinese respiratory physicians largely supported the current brief ICF core set for ***,the newly added and unconfirmed categories need further investigation.

Research progress in pathogenic genes of hereditary non-syndromic mid-frequency deafness

Hearing impairment is considered as the most prevalent impairment worldwide. Almost 600 million people in the world suffer from mild or moderate hearing impairment, an estimated 10% of the human population. Genetic factors play an important role in the pathogenesis of this disorder. Hereditary hearing loss is divided into syndromic hearing loss （associated with other anomalies） and non-syndromic hearing loss （not associated with other anomalies）. Approximately 80% of genetic deafness is non-syndromic. On the basis of the frequency of hearing loss, hereditary non-syndromic hearing loss can be divided into high-, mid-, low-, and total-frequency hearing loss. An audiometric finding of mid-frequency sensorineural hearing loss, or a ＂bowl-shaped＂ audiogram, is uncommon. Up to now, merely 7 loci have been linked to mid-frequency hearing loss. Only four genetic mid- frequency deafness genes, namely, DFNA10 （EYA4）, DFNA8/12 （TECTA）, DFNA13 （COLIIA2）, DFNA44 （CCDC50）, have been reported to date. This review summarizes the research progress of the four genes to draw attention to mid-frequency deafness genes.

Feasibility and safety of single-incision laparoscopic cholecystectomy versus conventional laparoscopic cholecystectomy in an ambulatory setting

Background: Single-incision laparoscopic surgery has emerged as an alternative to conventional laparoscopic cholecystectomy(LC) in the clinical setting. Limited information is available on the possibility of performing single-incision laparoscopic surgery as an ambulatory procedure. This study aimed to determine the feasibility and safety of single-incision laparoscopic cholecystectomy(SILC) versus conventional LC in an ambulatory setting. Methods: Ninety-one patients were randomized to SILC( n = 49) or LC( n = 42). The success rate, operative duration, blood loss, hospital stay, gallbladder perforation, drainage, delayed discharge, readmission, total cost, complications, pain score, vomiting, and cosmetic satisfaction of the two groups were then compared. Results: There were significant differences in the operative time(46.89 ± 10.03 min in SILC vs. 37.24 ± 10.23 min in LC;P 0.05). Conclusions: Ambulatory SILC is safe and feasible for selected patients. The advantages of SILC as compared with LC are improved cosmetic satisfaction and lower total costs.

Study of the relationship between selfstigma and subjective quality of life for individuals with chronic schizophrenia in the community

Background Studies conducted outside of China have found that stigma can predict low quality of life, and research in China has shown that stigma has an impact on the quality of life of convalescent patients with schizophrenia. Nevertheless, there is no indepth research on the impact of stigma on the quality of life of *** To research the correlation of stigma and the subjective quality of life of persons with chronic schizophrenia in the *** We adopted a stratified sampling *** questionnaire, quality of life scale and stigma scale were given to 602 persons with chronic schizophrenia in the *** The mean(SD) value of the Subjective Quality of Life Scale total scores was 32.99(13.85). The mean(SD)value of the total stigma scores was 43.50(5.02). After correlation analysis and multivariate stepwise regression analysis, it was shown that the total scores for self-stigma and each factor were positively correlated with subjective quality of life(r=0.462, p<0.001), psychosocial factors(r=0.517, p<0.001), and symptoms and adverse reaction factor scores(r=0.363, p<0.001),and the correlation coefficients were statistically *** The higher the stigma of persons with schizophrenia in the community, the lower the subjective quality of life is. This suggests that reduction of self-stigma should be considered in the improvement of the quality of life of persons with schizophrenia in the community.

Changes in IncRNAs and related genes in β-thalassemia minor and β-thalassemia major

β-thalassemia is caused by β-globin gene mutations. However, heterogeneous phenotypes were found in individuals with same genotype, and still undescribed mechanism underlies such variation. We collected blood samples from 30 β-thalassemia major, 30 β-thalassemia minor patients, and 30 matched normal controls. Human lncRNA Array v2.0 （8 × 60 K, Arraystar） was used to detect changes in long non-coding RNAs （lncRNAs） and mRNAs in three samples each from β-thalassemia major, β-thalassemia minor, and control groups. Compared with normal controls, 1424 and 2045 lncRNAs were up- and downregulated, respectively, in β-thalassemia major patients, whereas 623 and 349 lncRNAs were up- and downregulated, respectively, in β-thalassemia minor patients. Compared with β-thalassemia minor group, 1367 and 2356 lncRNAs were up- and downregulated, respectively, in β-thalassemia major group. We selected five lncRNAs that displayed altered expressions （DQ583499, X-inactive specific transcript （Xist）, IincRNA-TPM1, MRFS16P, and lincRNA-RUNX2-2） and confirmed their expression levels in all samples using real-time polymerase chain reaction. Based on coding-non-coding gene co-expression network and gene ontology biological process analyses, several signaling pathways were associated with three common organ systems exhibiting β-thalassemia phenotypes： hematologic, skeletal, and hepatic systems. This study implicates that abnormal expression levels of lncRNAs and mRNA in β-thalassemia cases may be correlated with its various clinical phenotypes.

Clinical characteristics and risk factors of severe respiratory syncytial virus-associated acute lower respiratory tract infections in hospitalized infants

Background:To investigate the clinical characteristics and analyze risk factors for severe respiratory syncytial virus(RSV)infection in hospitalized infants with acute lower respiratory tract infections(ALRIs).Methods:A retrospective review of the medical records of infants with RSV-associated ALRIs between March 1st,2011 and February 29th,2012 was *** were followed up over the phone or by outpatient visit six and twelve months after ***:Among 913 RSV-associated ALRIs infants,288(31.5%)had severe infections,which accounted for 4.2%of hospitalized *** hospital RSV mortality rate was 1.0%.The proportions of cases with tachypnea,apnea,cyanosis,and fine rales were significantly higher in the severe ALRIs group(all P<0.001).Multivariate logistic regression showed that low-birth-weight[1.698(1.028-2.805)],age less than 3 months old[3.385(2.174-5.271)],congenital heart disease[1.667(1.149-2.418)],bronchopulmonary dysplasia[8.505(1.731-41.780)],and airway abnormalities[2.246(1.008-5.005)]were independent risk factors for severe *** incidence of bronchitis,pneumonia and readmission in the severe group was significantly higher than that of the non-severe group during the one-year follow-up(all P<0.001).Conclusions:Younger age,low birth weight and underlying disease are associated with severe RSVassociated ***,severe RSV infections may be associated with a higher frequency of subsequent bronchitis,pneumonia and re-hospitalization in the following year.
