Patients with Crohn’s disease(CD)are at a considerable risk for intestinal *** 25%of patients with CD will undergo an intestinal resection within 10 years of *** complications after CD surgery have been reported in 20%-47%of the *** general and CD-related risk factors are associated with postoperative complications,and comprise non-modifiable(e.g.,age)and potentially modifiable risk factors(e.g.,malnutrition).Prehabilitation focuses on the preoperative period with strategies designed to optimize modifiable risk factors concerning the physical and mental condition of the individual *** aim of prehabilitation is to enhance postoperative recovery and return to or even improve preoperative functional *** improvement of nutritional status,physical fitness,cessation of smoking,psychological support,and critical revision of preoperative use of CD medication are important *** of the effect on postoperative outcome in CD patients are scarce,and guidelines lack recommendations on tailored *** this opinion review,we review the current evidence on the impact of screening and management of nutritional status,physical fitness,CD medication and laboratory values on the postoperative course following an intestinal resection in CD *** addition,we aim to provide guidance for individualized multimodal prehabilitation in clinical practice concerning these modifiable factors.

Organizing resilient infrastructure initiatives:A study on conceptualization,motivation,and operation of ten initiatives in the Netherlands

Resilient infrastructure is critical to a sustainable and functioning *** management and(re)development are highly complex processes encompassing various stakeholders’interests while they are pres-sured by the uncertainty of climate change and social *** response to these challenges,various resilience initiatives emerged with different motivations and *** purpose of this research is to understand the interplay between motivations and organizational approaches as well as resilience *** can provide insights into which domains of resilience have been focused on and what needs to be improved in their or-ganizational approaches to realize *** research specifically investigates ten resilient infrastructure initiatives in the *** using scoping review and content analysis,our results highlight that resilience initiatives conceptualize resilience in different ways,mainly focusing on built and organizational resilience with a focus on long-term and wider geographic *** initiative had several motivations,including 1)creat-ing innovative solutions,2)sharing knowledge,3)promoting commitment and cooperation,and 4)promoting *** motivations are reflected in the organizational *** example,there was a strong link between the motivation‘creating shared knowledge’and the organizational approach‘research collaboration.’Generic motivation such as‘promoting resilience’does not have one mainstreaming approach,which shows promoting resilience in practice is still in the exploration *** research provides major motivations and organizational approaches and their link within the resilient infrastructure initiatives which can contribute to better organizing similar initiatives aiming for resilient infrastructure.

Changes in Timing and kinematics of goal directed eye-hand movements in early-stage Parkinson’s disease

Objective:Many daily activities involve intrinsic or extrinsic goal-directed eye and hand *** extensive visuomotor coordination network including nigro-striatal pathways is required for efficient timing and positioning of eyes and *** aim of this study was to investigate how Parkinsons disease(PD)affects eye-hand coordination in tasks with different cognitive ***:We used a touch screen,an eye-tracking device and a motion capturing system to quantify changes in eye-hand coordination in early-stage PD patients(H&Y<2.5)and age-matched *** and kinematics of eye and hand were quantified in four eye-hand coordination tasks(pro-tapping,dual planning,anti-tapping and spatial memory task).Results:In the pro-tapping task,saccade initiation towards extrinsic goals was not ***,in the dual planning and anti-tapping task initiation of saccades towards intrinsic goals was faster in PD *** movements were differently affected:initiation of the hand movement was only delayed in the pro-tapping and dual planning ***,hand movements in PD patients were slower executed compared to ***:Whereas initiation of saccades in an extrinsic goal-directed task(pro-tapping task)is not affected,early stage PD patients have difficulty in suppressing reflexive saccades towards extrinsic goals in tasks where the endpoint is an intrinsic goal(*** planning and anti-tapping task).This is specific for eye movements,as hand movements have delayed responses in the pro-tapping and dual planning *** suggests that reported impairment of the dorsolateral prefrontal cortex in early-stage PD patients affects only inhibition of eye *** conclude that timing and kinematics of eye and hand movements in visuomotor tasks are affected in PD *** result may have clinical significance by providing a behavioral marker for the early diagnosis of PD.

Contactins are a group of cell adhesion molecules that are mainly expressed in the brain and play pivotal roles in the organization of axonal domains, axonal guidance, neuritogenesis, neuronal development, synapse formation and plasticity, axo-glia interactions and neural regeneration. Contactins comprise a family of six members. Their absence leads to malformed axons and impaired nerve conduction. Contactin mediated protein complex formation is critical for the organization of the axon in early central nervous system development. Mutations and differential expression of contactins have been identified in neuro-developmental or neurological disorders. Taken together, contactins are extensively studied in the context of nervous system development. This review summarizes the physiological roles of all six members of the Contactin family in neurodevelopment as well as their involvement in neurological/neurodevelopmental disorders.

Management of extracranial carotid artery stenosis during endovascular treatment for acute ischaemic stroke:results from the MR CLEAN Registry

Background The optimal management of ipsilateral extracranial internal carotid artery(ICA)stenosis during endovascular treatment(EVT)is *** compared the outcomes of two different strategies:EVT with vs without carotid artery stenting(CAS).Methods In this observational study,we included patients who had an acute ischaemic stroke undergoing EVT and a concomitant ipsilateral extracranial ICA stenosis of≥50%or occlusion of presumed atherosclerotic origin,from the Dutch Multicenter Randomized Clinical Trial of Endovascular Treatment for Acute Ischemic Stroke in the Netherlands(MR CLEAN)Registry(2014-2017).The primary endpoint was a good functional outcome at 90 days,defined as a modified Rankin Scale score≤*** endpoints were successful intracranial reperfusion,new clot in a different vascular territory,symptomatic intracranial haemorrhage,recurrent ischaemic stroke and any serious adverse *** Of the 433 included patients,169(39%)underwent EVT with *** 123/168(73%)patients,CAS was performed before intracranial *** 42/224(19%)patients who underwent EVT without CAS,a deferred carotid endarterectomy or CAS was *** with and without CAS were associated with similar proportions of good functional outcome(47%vs 42%,respectively;adjusted OR(aOR),0.90;95%CI,0.50 to 1.62).There were no major differences between the groups in any of the secondary endpoints,except for the increased odds of a new clot in a different vascular territory in the EVT with CAS group(aOR,2.96;95%CI,1.07 to 8.21).Conclusions Functional outcomes were comparable after EVT with and without *** during EVT might be a feasible option to treat the extracranial ICA stenosis but randomised studies are warranted to prove non-inferiority or superiority.

m TOR signaling in liver regeneration: Rapamycin combined with growth factor treatment

AIM: To investigate the effects of mammalian target of rapamycin(mT OR) inhibition on liver regeneration and autophagy in a surgical resection ***: C57BL/6 mice were subjected to a 70% partial hepatectomy(PH) and treated intraperitoneally every 24 h with a combination of the m TOR inhibitor rapamycin(2.5 mg/kg per day) and the steroid dexamethasone(2.0 mg/kg per day) in phosphate bufferedsaline(PBS) or with PBS alone as vehicle control. In the immunosuppressant group, part of the group was treated subcutaneously 4 h prior to and 24 h after PH with a combination of human recombinant interleukin 6(IL-6; 500 μg/kg per day) and hepatocyte growth factor(HGF; 100 μg/kg per day) in PBS. Animals were sacrificed 2, 3 or 5 d after PH and liver tissue and blood were collected for further analysis. Immunohistochemical staining for 5-Bromo-2'-deoxyuridine(Brd U) was used to quantify hepatocyte proliferation. Western blotting was used to detect hepatic microtubule-associated protein 1 light chain 3(LC3)-Ⅱ protein expression as a marker for autophagy. Hepatic gene expression levels of proliferation-, inflammation- and angiogenesisrelated genes were examined by real-time reverse transcription-polymerase chain reaction and serum bilirubin and transaminase levels were analyzed at the clinical chemical core facility of the erasmus mc-University Medical ***: m TOR inhibition significantly suppressed regeneration, shown by decreased hepatocyte proliferation(2% vs 12% Brd U positive hepatocyte nuclei at day 2, P < 0.01; 0.8% vs 1.4% at day 5, P = 0.02) and liver weight reconstitution(63% vs 76% of initial total liver weight at day 3, P = 0.04), and furthermore increased serum transaminase levels(aspartate aminotransferase 641 U/L vs 185 U/L at day 2, P = 0.02).
