Objective To clarify the feasibility and ascendency of both the laparoscopic and open management of symptomatic and large adrenal cysts. Materials and Methods From November 1997 to December 2007, 24 patients were evaluated and treated for adrenal cysts at the department of Urology, Ruijin hospital Affiliated to The shanghai jiao tong university school of medicine, shanghai, ***. 12 patients underwent traditional open operation and 12 patients underwent laparoscopic management. The surgical indications for adrenal cyst management were symptomatic in 17 cases, cyst size 4 cm or greater in 6, one of which was combined with malignancy. Results In the open series, six patients underwent adrenal cystectomy, 1 underwent partial adrenalectomy and 4 underwent total adrenalectomy and one case was performed total nephrectomy and adrenalectomy. In the laparoscopic series, 11 patients underwent adrenal cystectomy and 1 underwent total adrenalectomy. Mean operative time of open series was longer than that of laparoscopic series (123.8±34.2 vs 95.0±38.0 min, Phospital stay of open series was also longer than that of laparoscopic series (9.17 ± 1.85 vs 6.50 ± 1.17 days, P < 0.05). Conclusions Laparoscopic adrenal surgery is a safe and effective treatment. The laparoscopic approach should be considered preferable for patients who require adrenal surgery if no contraindications preclude its use. When the cyst wall thickness is more than 0.5 cm, with calcification and bloody cystofluidus or complicated other urinary touble especially malignant tumor, open management should be considered first.

Acute promyelocytic leukemia (APL) is a distinct subtype of acute myeloid ***,it is identified as the M3 subtype of acute myeloid leukemia by the French-American- British classification and cytogenetically is characterized by a balanced reciprocal translocation between chromosomes 15 and 17,which results in the fusion between promyelocytic leukemia (PML)gene and retinoic acid receptor(RAR).It seems that the disease is the most malignant form of acute leukemia with a severe bleeding tendency and a fatal course of only weeks. Chemotherapy (CT;daunorubicin,idarubicin and cytosine arabinoside)was the front-line treatment of APL with a complete remission (CR) rate of 75% to 80% in newly diagnosed patients. Despite all these progresses,the median duration of remission ranged from 11 to 25 months and only 35% to 45% of the patients could be cured by *** the introduction of all-trans retinoic acid (ATRA) in the beatment and optimization of the ATRA-based regimens,the CR rate was raised up to 90% to 95% and 5-yeer disease free survival(DFS) to 74%.The use of arsenic trioxide(ATO) since early 1990s further improved the clinical outcome of refractory or relapsed as well as newly diagnosed *** this article,we review the history of introduction of ATRA and ATO into clinical use and the mechanistic studies in understanding this model of cancer targeted therapy.

Objective The effect of mix-cultured BMC on the survival of cardiac allograft was observed, after the recipients were pre-conditioned by a non-myeloablative regimen. Methods The heart from C57BL/6 mouse was transplanted into BALB/c abdominal cavity. The recipients were divided as follows. I: no treatment. II: TBI (treated by 3 Gy total body irradiation on d 0 before the operation) + rapamune (rapa, 1 mg/kg/d from d 1 to d 14 post-transplantation). III: TBI + rapa + the mix-cultured BMC (1×107 per mouse). IV: treated by the same regimen as Group III, while using C3H mouse as the donor. The survival of cardiac allograft in all four groups was observed. The percentage of CD4+CD25+ T and donor-derived cells in peripheral blood and spleen of recipient were measured by FCM, and the Th1/Th2 cytokine profiles in the allograft were measured by immunohistochemistry. Results Compared with others groups, in the Group III, the allograft survival was prolonged significantly (32.8 ± 5.9 d,p3+CD4+ T was 50.15 ± 15.01% and 18.47 ± 5.32% respectively, and the percentage of donor-derived cells was 2.39±1.27% and 9.22±2.83% respectively;When compared with other groups, the percentages of CD4+CD25+ T and donor-derived cells in peripheral blood and spleen of recipient were all increased significant (p<0.05);The decreased expression of IFN-γin the allograft were also observed. Conclusion Adoptive transfer of mix-cultured BMC prolonged the survival of cardiac allograft in mice in a donor antigen-specific manner.

We have established an in vivo model for visualizing the developmental and chemotactic movement of primitive macrophages,by generating a stable transgenic line expressing EGFP under the zebrafish lysozyme C promoter [TG(zlyz:EGFP)].We show that the earliest macrophages originate from the head mesenchyme nearby the bilateral plate mesoderm at～22 hpf,then migrate either within the head or toward the yolk sac close to the circulation *** macrophages subsequently colonize the posterior intermediate cell mass(ICM)through ***,the resident macrophages,but not the circulating macrophages,migrate rapidly to the site of inflammation induced by tail transection,with their number peaking at approximately 5-6 hours after *** hours post-injury,these recruited macrophages initiate retrograde migration from the inflamed tail back to the ICM mesenchyme,suggesting a resolution of the ***,an anti-inflammatory glucocorticoid hormone (dexamethasone)suppresses both the chemotactic and retrograde migration of macrophages during the onset and resolution of ***,the TG(zlyz:EGFP)zebrafish provides an invaluable tool not only for elucidating the pleiotropic roles of macrophages in innate immunity,tumorigenesis and organogenesis during embryogenesis,but for identifying the therapeutic small molecules against human disorders associated with deregulated migration and chemotaxis of macrophage in vivo.

Objective The administration of midazolam had no effect upon the electrophysiologic prope rties of cardiac tissue. Methods Radiofrequency ablation was performed in 201 consecutive persistent AF patients (150 males,mean age 58.1±10.5 years),who under sedation with a bolus of midazolam and anal gesia with a continuous infusion of *** intravenous bolus of midazolam was used for in ducing sedation for electrical cardioversion in patients whose atrial arrhythmias were not terminate d by ablation. Results After extensive ablation,atrial arrhythmia was failed to be terminated by ablation i n 106(52.7%) ***,termination of atrial arrhythmias and restoration of sinus rhy thm were observed in 5 patients(4.7%,all male,mean age 53.0±9.9 years old) 16.0±4.2 seconds after the administration of midazolam(l-2mg) without antiarrhythmics or electrical cardioversion. Two patients encountered the recurrence of atrial flutter(AFL),and one of them had a second *** 32.8±16.7 months post the last procedure,4(80%) patients were free of arrhyth mias without the use of antiarrhythmics and 1 patient remained in drug-refractory persistent AFL. Conclusions In patients who underwent extensive ablation for persistent AF,an intravenous bolus of midazolam may have the cardioversion effectiveness.

Palladin was originally found up-regulated with NB4 cell differentiation induced by *** of palladin results in neural tube closure defects,liver herniation and embryonic *** we further report thatpailadinembryos exhibit a significant defect in erythropoiesis,characterized by dramatic reduction in definitive erythrocytes derived from fetal liver but not primitive erythrocytes from yolk *** reduction of erythrocytes is accompanied by increased apoptosis of erythroblasts and partial blockage of erythroid ***,colony forming assay shows no differences between wt and mutant fetal liver or yolk sac in the number and size of colonies *** addition,palladinfetal liver cells can reconstitute hematopoiesis in lethally-irradiated *** data strongly suggest that deficient erythropoiesis in palladin fetal liver is mainly due to compromised erythropoietic *** expected,erythroblastic island in palladin fetal liver was found *** liver cells fail to form erythroblastic island in ***,wt macrophages can form such units with either wt or mutant erythroblasts,while mutant macrophages lose their ability to bind wt or mutant *** data demonstrate that palladin is crucial for definitive erythropoiesis and erythroblastic island formation,and especially,required for normal function ofmacrophages in fetal liver.

Objective:to establish a new type of measurement and analysis by using 3dMD photogrammetric system for diagnosis of facial ***:Under ideal circumstances,a phantom is used to examine the precision and accuracy of the 3dMD system.A clinical setting is simulated by varying different parameters distance and system ***:The handling of the system was unproblematic in matters of data acquisition and data *** was very reliable,with a mean global error of 0.132mm(0.02-0.437mm)for mannequin head ***: The precision and accuracy of the tested system are more than suffcient for clinical needs and greater than those of other methods,such as direct anthropometry and two-dimensional *** evaluated system can be recommended for evaluation and documentation of the facial surface and could offer new opportunities in reconstructive,orthognathic,and craniofacial surgery.

c-Myc promotes tubular cell apoptosis in ischemia-reperfusion-induced renal injury by negatively regulating c-FLIP and enhancing FasL/Fas-mediated apoptosis pathway

c-Myc plays an important role in cell proliferation,differentiation,and cell ***/Fas pathway is a key regulator of cell *** study was aimed to investigate the effects of c-Myc on the FasL/Fas pathway in ischemia-reperfusion(I/R)-induced renal *** were objected to bilateral renal ischemia for 60 min and reperfused for 24 or 48 ***-52 E cells were treated with hypoxia-reoxygenation(H/R)or *** was used to identify the distribution of *** apoptosis was assessed by TUNEL ***-c-Myc and recombinant pcDAN 3.0 were used to overexpress c-Myc and c-FLIP,*** assay and luciferase assay were used to detect the binding of c-Myc to c-FLIP *** I/R rats,c-Myc was increased significantly and mainly located in renal tubular epithelial cells;meanwhile,c-FLIP was decreased,cleaved caspase-8,cleaved caspase-3 and TUNEL-positive staining cells were *** of I/R rats with c-Myc inhibitor 10058-F4 significantly attenuated the decrease in c-FLIP,the increase in cleaved caspase-8,cleaved caspase-3,TUNEL-positive cells,Scr and BUN in I/R *** NRK-52 E cells,hypoxia and reoxygen induced the increase in c-Myc and decrease in *** and luciferase assay results indicated that cMyc binds to the promoter region of c-FLIP *** of c-Myc markedly decreased *** of c-FLIP inhibited the increase in cleaved caspase-8 and caspase-3 induced by *** indicated that c-Myc is increased in kidneys of I/R rats and negatively regulates the expression of c-FLIP,then enhanced FasL-induced cell apoptosis in I/R stress.
