Clinical evaluation of endoscopic resection for treatment of large gastric stromal tumors

BACKGROUND Gastric stromal tumor is a digestive tract mesenchymal tumor with malignant potential, and endoscopic techniques have been widely used in the treatment of gastric stromal tumors, but there is still controversy over their use for large gastric stromal tumors(≥ 3 cm).AIM To evaluate the clinical long-term efficacy and safety of endoscopic resection for large(≥ 3 cm) gastric stromal *** All patients who underwent endoscopic resection or surgery at our hospital from 2012 to 2017 for pathologically confirmed gastric stromal tumor with a maximum diameter of ≥ 3 cm were collected. The clinical data, histopathologic characteristics of the tumors, and long-term outcomes were *** A total of 261 patients were included, including 37 patients in the endoscopy group and 224 patients in the surgical group. In the endoscopy group, the maximum tumor diameter was 3-8 cm; the male: Female ratio was 21/16; 34 cases had low-risk tumors, 3 had intermediate-risk, and 0 had high-risk; the mean follow-up time was 30.29 ± 19.67 mo, no patient was lost to follow-up, and no patient received chemotherapy after operation; two patients with recurrence had low-risk stromal tumors, and neither had complete resection under endoscopy. In the surgical group, the maximum tumor diameter was 3-22 cm; the male: Female ratio was 121/103; 103 cases had low-risk tumors, 75 had intermediate-risk, and 46 had high-risk; the average follow-up time was 38.83 ± 21.50 mo, 53 patients were lost to follow-up, and 8 patients had recurrence after operation(6 cases had high-risk tumors, 1 had intermediate-risk, and 1 had low-risk). The average tumor volume of the endoscopy group was 26.67 ± 26.22 cm^3(3.75-120), all of which were less than 125 cm^3. The average volume of the surgical group was 273.03 ± 609.74 cm^3(7-4114). Among all patients with a tumor volume < 125 cm^3,7 with high-risk stromal tumors in the surgical group(37.625 cm^3 to 115.2 cm^3)accounted for 3.8%(7/183); of those

This article investigates two fairness criteria with regard to adaptive resource allocation for uplink orthogonal frequency division multiple access （OFDMA） systems. Nash bargaining solution （NBS） fairness and proportional fairness （PF） are two suitable candidates for fairness consideration, and both can provide attractive trade-offs between total throughput and each user＇s capacity. Utilizing Karush-kuhn- Tucker （KKT） condition and iterative method, two effective algorithms are designed, to achieve NBS fairness and proportional fairness, respectively. Simulation results show that the proposed resource allocation algorithms achieve good tradeoff between the overall rate and fairness, with little performance loss from the total capacity.

Objectives:To evaluate the effect of echinacoside(ECH)on cognitive dysfunction in post cerebral stroke model ***:The post stroke cognitive impairment rat model was created by occlusion of the transient middle cerebral artery(MCAO).The rats were randomly divided into 3 groups by a random number table:the sham group(sham operation),the MCAO group(received operation for focal cerebral ischemia),and the ECH group(received operation for focal cerebral ischemia and ECH50 mg/kg per day),with 6 rats in each group,The infarct volume and spatial learning were evaluated by triphenyl tetrazolium chloride staining and Morris water *** expression of α7 nAChR in the hippocampus was detected by *** contents of acetylcholine(ACh),malondialdehyde(MDA),glutathione(GSH),superoxide dismutase(SOD),activities of choline acetyltransferase(ChAT),acetylcholinesterase(AChE),and catalase(CAT)were evaluated by enzyme linked immunosorbent *** neural apoptosis and autophagy were determined by TUNEL staining and LC3 staining,***:ECH significantly lessened the brain infarct volume and ameliorated neurological deficit in infarct volume and water content(both P<0.01).Compared with MCAO rats,administration of ECH revealed shorter escape latency and long retention time at 7,14 and 28 days(all P<0.01),increased the α7 nAChR protein expression,ACh content,and ChAT activity,and decreased AChE activity in MCAO rats(all P<0.01).ECH significantly decreased MDA content and increased the GSH content,SOD,and CAT activities compared with MCAO rats(all P<0.05).ECH suppressed neuronal apoptosis by reducing TUNEL-positive cells and also enhanced autophagy in MCAO rats(all P<0.01).Conclusion:ECH treatment helped improve cognitive impairment by attenuating neurological damage and enhancing autophagy in MCAO rats.

n-Alkanes are widely used in paleoenvironmental ***,our understanding of changes in the distribution of n-alkanes with climatic and environmental factors remains unclear in arid/semi-arid *** sampled 26 surface sediments from three climatic zones across the southwestern Tibetan Plateau to evaluate the sensitivity of chain length distributions of n-alkanes to climatic and environmental *** observations demonstrate that average chain length(ACL),proportion of aquatic macrophyte(Paq),carbon preference index(CPI)and ratio of the contents of nC_(27)and nC_(31)(nC_(27)/nC_(31))are all sensitive to hydroclimatic *** contrast to commonly-adopted assumptions,the correlations between these indices and hydrological parameters are not always good,which indicates that the interpretation of n-alkane indices is special on the southwestern Tibetan *** might be related to the vegetation characteristics and seasonality of biological activity,and need to be considered in paleoclimatic *** impact of seasonal precipitation on n-alkanes indices was also evaluated.

Recovering iron and aluminum efficiently is the key route to utilize low-grade high-iron *** to optimize the iron separating process and elevate both Fe and Al recovery ratio,three different Fe-Al recovery processes with different magnetic roasting(R),Bayer leaching process(L)and magnetic separation(S)orders were *** studied processes include bauxite leaching→red mud roasting→magnetic separation(L-R-S),bauxite roasting→magnetic separation→leaching(R-S-L)and bauxite roasting→leaching→magnetic separation(R-L-S).The iron recovery ratio,Fe2O3 content in iron concentration and the bauxite dissolution ratio of each process were ***,the optimizations of the leaching,roasting and magnetic separation conditions were *** indicate that the R-S-L process should be an advisable order to recover both alumina and *** the three processes,the R-S-L route had the highest alumina dissolution ratio and iron recovery ratio,which was 86.20%and 69.58%,respectively,while the Fe2O3 content of the iron concentrate was 40.66%.

AIM To investigate the role of the complement 5a(C5a)/C5 a receptor(C5a R) pathway in the pathogenesis of acute liver failure(ALF) in a mouse *** BALB/c mice were randomly assigned to different groups, and intraperitoneal injections of lipopolysaccharide(LPS)/D-galactosamine(D-Gal N)(600 mg/kg and 10 μg/kg) were used to induce ALF. The KaplanMeier method was used for survival analysis. Serum alanine aminotransferase(ALT) levels, at different time points within a 1-wk period, were detected with a biochemistry analyzer. Pathological examination of liver tissue was performed 36 h after ALF induction. Serum complement 5(C5), C5 a, tumor necrosis factor-α(TNF-α), interleukin(IL)-1β, IL-6, high-mobility group protein B1(HMGB1) and sphingosine-1-phosphatelevels were detected by enzyme-linked immunosorbant assay. Hepatic morphological changes at 36 h after ALF induction were assessed by hematoxylin and eosin staining. Expression of C5 a R, sphingosine kinase 1(Sph K1), p38-MAPK and p-p38-MAPK in liver tissue, peripheral blood mononuclear cells(PBMCs) and peritoneal exudative macrophages(PEMs) of mice or RAW 264.7 cells was analyzed by western blotting. C5 a R m RNA levels were detected by quantitative real-time *** Activation of C5 and up-regulation of C5 a R were observed in liver tissue and PBMCs of mice with ALF. Blockade of C5 a R with a C5 a R antagonist(C5a Ra C5 a Ra) significantly reduced the levels of serum ALT, inflammatory cytokines(TNF-α, IL-1β and IL-6) and HMGB1, as well as the liver tissue damage, but increased the survival rates(P < 0.01 for all). Blockade of C5 a R decreased Sph K1 expression in both liver tissue and PBMCs significantly at 0.5 h after ALF induction. C5 a Ra pretreatment significantly downregulated the phosphorylation of p38-MAPK in liver tissues of ALF mice and C5 a stimulated PEMs or RAW 264.7 cells.
