AIM:To evaluate the efficacy and safety of sodium hyaluronate solution(SH) in endoscopic submucosal dissection(ESD) of gastric ***:A prospective multicenter randomized,double blind,controlled trial was designed and utilized in this study.A total of 76 patients with 5-20 mm sized gastric neoplasms were enrolled at three academic hospitals in South Korea from June 2011 to October *** were randomly assigned to the 0.4% sodium hyaluronate or control *** lesions underwent endoscopic *** was performed with 0.4%SH and normal saline(NS) solution for submucosal *** was assessed using en bloc resection and the number of additional *** evaluation variables were the volume of injection material,steepness of mucosal elevation,bleeding rate,procedural time and operator ***,the safety was assessed by analyzing adverse events during the ***:The usefulness rate in the 0.4%SH group and the controlled group had statistically significant difference under intention to treat(ITT) analysis(90.91% vs 61.11% P = 0.0041).Under per protocol(PP),the usefulness rate is statistically significant different(93.10% vs 61.76%,P = 0.0036).The difference in volume of the solution injected between 0.4%SH group and the controlled group and NS group was also statistically significant under intention to treat and per protocol analysis(ITT:0.03 ± 0.02 mL vs 0.06 ± 0.03 mL,P = 0.0003,PP:0.03 ± 0.02 mL vs 0.06 ± 0.03 mL,P = 0.0004).Satisfaction above the grade good was significantly higher in the SH group under intention to treat and per protocol analysis(ITT:90.91% vs 61.11%,P = 0.0041,PP = 93.11% vs 61.77%,P = 0.0022).Adverse events above grade 3 were not noticed in either *** adverse events were treated and were judged as not associated with the submucosal injection ***:0.4%SH solution is a safe and effective agent that doesn't cause any significant adverse events and is useful for submucos

响应面法优化金银花凉茶浸提工艺的研究

采用响应面分析法优化金银花凉茶的浸提工艺条件,以金银花、菊花、夏枯草和鱼腥草为原料,绿原酸得率为指标.在单因素试验基础上,应用Box-Benhnken中心组合设计试验,对浸提工艺影响较大的料液比、浸提时间、浸提温度3个工艺参数进行优化.结果表明:金银花等原料的绿原酸水提工艺的最佳条件为不粉碎原料、固定金银花1g,菊花、夏枯草和鱼腥草质量比为2∶2∶1共1.5g,料液比为1∶32,浸提时间为71min,浸提温度为78℃,浸提2次,在该条件下验证绿原酸得率达到3.01%,接近于预测值3.04%.研究表明,优化得到的回归模型具有良好的预测能力.

Low survival rate occurs in patients who initially experience a spontaneous return of circulation after cardiac arrest（CA）. In this study, we induced asphyxial CA in adult male Sprague-Daley rats, maintained their body temperature at 37 ± 0.5°C, and then observed the survival rate during the post-resuscitation phase. We examined neuronal damage in the hippocampus using cresyl violet（CV） and Fluore-Jade B（F-J B） staining, and pro-inflammatory response using ionized calcium-binding adapter molecule 1（Iba-1）, glial fibrillary acidic protein（GFAP）, and tumor necrosis factor-alpha（TNF-α） immunohistochemistry in the hippocampus after asphyxial CA in rats under normothermia. Our results show that the survival rate decreased gradually post-CA（about 63% at 6 hours, 37% at 1 day, and 8% at 2 days post-CA）. Rats were sacrificed at these points in time post-CA, and no neuronal damage was found in the hippocampus until 1 day post-CA. However, some neurons in the stratum pyramidale of the CA region in the hippocampus were dead 2 days post-CA. Iba-1 immunoreactive microglia in the CA1 region did not change until 1 day postCA, and they were activated（enlarged cell bodies with short and thicken processes） in all layers 2 days postCA. Meanwhile, GFAP-immunoreactive astrocytes did not change significantly until 2 days post-CA. TNF-α immunoreactivity decreased significantly in neurons of the stratum pyramidale in the CA1 region 6 hours post-CA, decreased gradually until 1 day post-CA, and increased significantly again 2 days post-CA. These findings suggest that low survival rate of normothermic rats in the early period of asphyxia-induced CA is related to increased TNF-α immunoreactivity, but not to neuronal damage in the hippocampal CA1 region.

AIM: To determine the genomic changes in hepatitis B virus(HBV) and evaluate their role in the development of hepatocellular carcinoma(HCC) in patients chronically infected with genotype C ***: Two hundred and forty chronic hepatitis B(CHB) patients were subjected and followed for a median of 105 mo. HCC was diagnosed in accordance with AASLD guidelines. The whole X, S, basal core promoter(BCP), and precore regions of HBV were sequenced using the direct sequencing ***: All of the subjects were infected with genotype C HBV. Out of 240 CHB patients, 25(10%) had C1653 T and 33(14%) had T1753 V mutation in X region; 157(65%) had A1762T/G1764 A mutations in BCP region, 50(21%) had G1896 A mutation in precore region and 67(28%) had pre-S deletions. HCC occurred in 6 patients(3%). The prevalence of T1753 V mutation was significantly higher in patients who developed HCC than in those without HCC. The cumulative occurrence rates of HCC were 5% and 19% at 10 and 15 years, respectively, in patients with T1753 V mutant, which were significantly higher than 1% and 1% in those with wild type HBV(P < 0.001).CONCLUSION: The presence of T1753 V mutation in HBV X-gene significantly increases the risk of HCC development in patients chronically infected with genotype C HBV.

AIM: To evaluate clinical outcomes of patients that underwent surgery, transarterial embolization (TAE), or supportive care for spontaneously ruptured hepatocellular carcinoma (HCC). METHODS: A consecutive 54 patients who diagnosed as spontaneously ruptured HCC at our institution between 2003 and 2012 were retrospectively enrolled. HCC was diagnosed based on the diagnostic guidelines issued by the 2005 American Association for the Study of Liver Diseases. HCC rupture was defined as disruption of the peritumoral liver capsule with enhanced fluid collection in the perihepatic area adjacent to the HCC by dynamic liver computed tomography, and when abdominal paracentesis showed an ascitic red blood cell count of > 50000 mm 3 /mL in bloody fluid. RESULTS: Of the 54 patients, 6 (11.1%) underwent surgery, 25 (46.3%) TAE, and 23 (42.6%) supportive care. The 2-, 4and 6-mo cumulative survival rates at 2, 4 and 6 mo were significantly higher in the surgery (60%, 60% and 60%) or TAE (36%, 20% and 20%) groups than in the supportive care group (8.7%, 0% and 0%), respectively (each, P < 0.01), and tended to be higher in the surgical group than in the TAE group. Multivariate analysis showed that serum bilirubin (HR = 1.09, P < 0.01), creatinine (HR = 1.46, P = 0.04), and vasopressor requirement (HR = 2.37, P = 0.02) were significantly associated with post-treatment mortality, whereas surgery (HR = 0.41, P < 0.01), and TAE (HR = 0.13, P = 0.01) were inversely associated with posttreatment mortality. CONCLUSION: Post-treatment survival after surgery or TAE was found to be better than after supportive care, and surgery tended to provide better survival benefit than TAE.

瞄准：用一台新奇设备介绍并且评估功效和内视镜的粘膜下层解剖(ESD ) 的技术方面，叉刀。方法：从 2004 年 3 月到 2008 年 4 月， ESD 在一个单个第三级的工作分派中心用 Flexknife (组 B ) 在使用一把叉刀(Endo FSregister )( 组 A ) 的 265 胃的损害上并且在 72 胃的损害上被执行。我们回顾地比较了 resected 标本的肿瘤，病理检查所见，和尺寸的内视镜的特征。我们也比较了在二个组之间的整块切除术率，完全的切除术率，复杂并发症，和过程时间。结果：resected 标本的吝啬的尺寸是 4.27 +/- 在组 A 和 4.29 +/- 的 1.26 厘米在组 B 的 1.48 厘米。整块切除术率是 95.8%(254/265 损害) 在组 A 并且 93.1%(67/72 ) 在组 B。没有 resected 边缘的肿瘤房间侵略的完全的 ESD 在 81.1% 被获得(215/265 ) 组 A 并且在 73.6%(53/72 ) 组 B。穿孔率是 0.8%(2/265 ) 在组 A 并且 1.4%(1/72 ) 在组 B。吝啬的过程时间是 59.63 +/- 在组 A 和 76.65 +/- 的 56.12 min 在组 B 的 70.75 min (P < 0.05 ) 。结论：叉刀(Endo FSregister ) 为临床的实践是有用的并且有的优点减少过程时间。

采用响应面分析法优化山楂中总黄酮超声辅助提取的工艺条件.以总黄酮(以金丝桃苷计)得率为指标,在单因素试验的基础上,应用了响应面中的Box-Benhnken模型设计试验,对提取工艺影响较大的山楂粉碎粒度、超声时间、料液比、超声功率4个工艺参数进行优化.结合实际操作,最佳的工艺条件为粉碎粒度40目,超声时间33min,料液比1∶33(g/mL),超声功率160W,在此条件下,总黄酮得率为5.21%,与预测值5.26%基本相符.

AIM: To investigate prevalence of Clonorchis sinensis in patients with gastrointestinal symptoms, and the relation of the infection to hepatobiliary diseases in 26 hospitals in ***: Consecutive patients who had been admitted to the Division of Gastroenterology with gastrointestinal symptoms were enrolled from March to April 2005. Of those who had been diagnosed with clonorchiasis, epidemiology and correlation between infection and hepatobiliary diseases were surveyed by ***: Of 3080 patients with gastrointestinal diseases, 396 (12.9%) had clonorchiasis and 1140 patients (37.2%) had a history of eating raw freshwater fish. Of those with a history of raw freshwater fish ingestion, 238 (20.9%) patients had clonorchiasis. Cholangiocarcinoma was more prevalent in C. sinensis-infected patients than non-infected patients [34/396 (8.6%) vs 145/2684 (5.4%), P = 0.015]. Cholangiocarcinoma and clonorchiasis showed statistically significant positive cross-relation (P = 0.008). Choledocholithiasis, cholecystolithiasis, cholangitis, hepatocellular carcinoma, and biliary pancreatitis did not correlate with ***: Infection rate of clonorchiasis was still high in patients with gastrointestinal diseases in Korea, and has not decreased very much during the last two decades. Cholangiocarcinoma was related to clonorchiasis, which suggested an etiological role for the parasite.
