Myocardial metastasis from ZEB1-and TWIST-positive spindle cell carcinoma of the esophagus:A case report

BACKGROUND Metastatic cardiac tumors are known to occur more frequently than primary cardiac tumors,however,they often remain asymptomatic and are commonly dis-covered on *** tumors with a relatively high frequency of cardiac metastasis include mesothelioma,melanoma,lung cancer,and breast cancer,whereas reports of esophageal cancer with cardiac metastasis are *** SUMMARY The case of a 60-year-old man who complained of dysphagia is *** gastrointestinal endoscopy showed a submucosal tumor-like elevated lesion in the esophagus causing ***-enhanced computed tomography showed left atrial compression due to the esophageal tumor,multiple liver and lung metastases,and a left pleural *** examination of a biopsy speci-men from the esophageal tumor showed spindle-shaped cells,raising suspicion of esophageal *** disease progressed rapidly,and systemic chemotherapy was deemed necessary,however,due to his poor general condition,adminis-tration of cytotoxic agents was considered *** his high Combined Positive Score,nivolumab was administered,however,the patient soon died from the *** autopsy confirmed spindle cell carcinoma(SCC)of the esophagus and cardiac metastasis with similar histological *** stem cell markers,ZEB1 and TWIST,were positive in both the primary tumor and the cardiac *** To the best of our knowledge,there have been no prior reports of cardiac metastasis of esophageal *** case highlights our experience with a patient with esophageal SCC who progressed rapidly and died from the disease,with the autopsy examination showing cardiac metastasis.

This paper presents a compact XYZ micro-stage driven by an impact driving mechanism.A moving body is translationally actuated along the X-,Y-,and Z-axes in millimeter-scale range and with nanometer-scale ***-N thin-film strain sensors are integrated into the micro-stage for closed-loop ***-loop control is also carried *** motion errors in six degrees of freedom are also investigated for this *** is clarified by analysis of finite element method that rotational motion errors around the driving axis are caused by torques due to tensions from the elastic hinges of *** mechanical structure of XYZ micro-stage to cancel out the torque generated is proposed and the second prototype is *** the rotational motion error is successfully suppressed in the second prototype,a rotational motion error of more than 0.1°remains due to remaining torque and assembly error of the elastic ***,in order to reduce the rotational motion error,the prototype is designed in which the location of elastic hinges is *** designing a mechanical structure in which torque is suppressed,all rotational motion errors are successfully reduced to less than 0.05°in the prototype with single-layer hinges.

Complete response with sorafenib and transcatheter arterial chemoembolization in unresectable hepatocellular carcinoma

Patients with advanced hepatocellular carcinoma(HCC) showing portal vein tumor thrombosis(PVTT) have an extremely poor prognosis. According to treatment guidelines, the only option for HCC patients with PVTT is sorafenib chemotherapy. However, in Asia, various treatments have been attempted and possible prolongation of overall survival has been repeatedly reported. We herein report the first case of a patient with an initially unresectable advanced HCC with PVTT who underwent curative hepatectomy after sorafenib and transcatheter arterial chemoembolization(TACE) showing complete histological response. Two months after induction with sorafenib, a significant decrease in serum alpha-fetoprotein level was observed and computed tomography imaging showed a significant decrease in tumor size. Because of remaining PVTT, TACE and curative resection were performed. The combination of sorafenib and TACE may be an effective treatment for HCC patients with PVTT.

Fistula between digestive tract and airway is one of the complications after esophagectomy with lymph node dissection. A case of esophagotracheal fistula secondary to esophagitis 9 years after esophagectomy and gastric pull-up for treatment of esophageal carcinoma is described. It was successfully treated with transposition of a pedicled pectoralis major muscle flap.

Stent migration, which causes issues in stent therapy for esophageal perforations, can counteract the therapeutic effects and lead to complications. Therefore, techniques to regulate stent migration are important and lead to effective stent therapy. Here, in these cases, we placed a removable fully covered self-expandable metallic stent(FSEMS) in a 52-year-old man with suture failure after surgery to treat Boerhaave syndrome, and in a 53-yearold man with a perforation in the lower esophagus due to acute esophageal necrosis. At the same time, we nasally inserted a Sengstaken-Blakemore tube(SBT), passing it through the stent lumen. By inflating a gastric balloon, the lower end of the stent was supported. When the stent migration was confirmed, the gastric balloon was lifted slightly toward the oral side to correct the stent migration. In this manner, the therapy was completed for these two patients. Using a FSEMS and SBT is a therapeutic method for correcting stent migration and regulating the complete migration of the stent into the stomach without the patient undergoing endoscopic rearrangement of the stent. It was effective for positioning a stent crossing the esophagogastric junction.

Using the Simplex Method for a Type of Allocation Problems

In this study we discuss the use of the simplex method to solve allocation problems whose flow matrices are doubly stochastic. Although these problems can be solved via a 0 - 1 integer programming method, H. W. Kuhn [1] suggested the use of linear programming in addition to the Hungarian method. Specifically, we use the existence theorem of the solution along with partially total unimodularity and nonnegativeness of the incidence matrix to prove that the simplex method facilitates solving these problems. We also provide insights as to how a partition including a particular unit may be obtained.

The treatment of historical buildings represents challenges for those who are concerned with conservation. The difficulty lies in how to apply the treatment without destroying the old components and the original design of the building. The treatment processes take place by the classification of the type and the value of the building, then the determination of the deterioration level decides the level of intervention. The aim of the research is to provide a method for assessing the ways of preserving the architectural and urban heritage, which includes all the factors that affect the preservation of the heritage so as to access that it contributes to access to the deficiencies in the ways of preserving the heritage and to build a plan for action to avoid them in the future. The research followed a causal comparative research approach that began with an analytical study to determine the factors affecting preservation and its considerations, by collecting and analyzing the data of the most important international and national charters and recommendations for the preservation. This study defined the preservation works into 4 levels of interventions passed on the intervention objectives, building＇s physical state, causes for the deterioration and maintenance works. By comparing the deterioration level with the intervention level for the conservation procedures, the result indicates the suitability of the work performed. This result could be number between 4 and -4.

Existence and Stability Property of Almost Periodic Solutions in Discrete Almost Periodic Systems

In this paper, we consider an almost periodic system which includes a system of the type , where k is a positive integer, aij are almost periodic in n and satisfy aij(n)≥0 for i≠j,? for 1≤j≤m. In the special case where aij(n) are constant functions, above system is a mathematical model of gas dynamics and was treated by T. Carleman and R. D. Jenks for differential systems. In the main theorem, we show that if the m X m matrix (aij(n)) is irreducible, then there exists a positive almost periodic solution which is unique and has some stability. Moreover, we can see that this result gives R. D. Jenks’ result for differential model in the case where aij(n) are constant functions. In Section 3, we consider the linear system with variable cofficients . Even in nonlinear problems, this linear system plays an important role, as their variational equations, and it is requested to determine the uniform asymptotically stability of the zero solution from the information about A(n). In order to obtain the existence of almost periodic solutions of both linear and nonlinear almost periodic discrete systems: above linear system and? for 1≤i≤m, respectively, we shall consider between certain stability properties, which are referred to as uniformly asymptotically stable, and the diagonal dominance matrix condition.
