The prognostic significance of percentage of tumour involvement according to disease risk group in men treated with radical prostatectomy

We investigated the prognostic significance of percentage of tumour involvement （PTI） according to the clinicopathological features of prostate cancer among patients who underwent radical prostatectomy （RP）. A retrospective study of 534 patients who underwent RP between September 2003 and March 2008 without any neoadjuvant or adjuvant therapy was performed. The associations of PTI with various clinicopathological features and biochemical recurrence-free survival were examined via uni- and multivariate analyses. The predictive accuracy of the multivariate model was assessed with a receiver operating characteristics-derived area under the curve. PTI was demonstrated to be significantly associated with preoperative prostate-specific antigen （PSA） level （P=0.001）, pathological Gleason score （P〈0.001）, extraprostatic tumour extension （P〈0.001）, seminal vesicle invasion （P〈0.001） and positive surgical margin （P〈0.001） in univariate analyses. When patients were stratified into disease risk groups, PTI was an independent predictor of biochemical recurrence-free survival in multivariate analysis only among the low-risk group （P=0.033） but not the intermediate- （P=0.287） or the high-risk groups （P=0.828）. The addition of the PTI did not significantly increase the accuracy of the multivariate model devised for the prediction of biochemical recurrence-free survival among both total patients （P=-0.459） and the low-risk group （P=0.268）, respectively. In conclusion, although PTI appeared to be a more significant prognostic factor among patients with low-risk disease than among those with higher risk diseases, overall, the PTI may not provide additional prognostic information beyond what can already be obtained via established prognostic factors.

The effect of diet-induced insulin resistance on DNA methylation of the androgen receptor promoter in the penile cavernosal smooth muscle of mice

Population studies have suggested an association between diabetes and the symptoms of testosterone deficiency. Recently, the expression of the androgen receptor （AR） has been shown to be decreased in diabetic patients. Furthermore, diabetes has been shown to induce global methylation. In this study, we used an animal model to investigate whether diabetes results in increased methylation of the AR promoter and whether these changes are associated with the decreased expression of AR in penile cavernosal smooth muscle tissue. Twenty C57BL/6J mice were divided into two groups, receiving either high- （mature diabetic） or low- （mature control） caloric meals for 14 weeks. Another 10 mice were killed at 1 week （young control）. Animals in the mature diabetic group showed decreased testosterone levels, although this was not statistically significant. In both control groups, no significant methylation was observed in the AR promoter region CpG island （-85 to ＋339）. In the mature diabetic group, significant methylation was observed at ＋ 185 and ＋200 of the AR promoter. These changes were associated with increased homeostatic model assessment for insulin resistance （HOMA-IR） and decreased corpus cavernosal tissue mass and expression of AR mRNA and protein. We conclude that in these animals, insulin resistance increased the methylation of the GC-rich regions of the AR promoter, leading to decreased AR expression.

Is two-stage reimplantation effective for virulent pathogenic infection in a periprosthetic hip? A retrospective analysis

AIM: To investigate the effectiveness of two-stage reimplantation using antibiotic-loaded bone cement(ALBC) and the risk factors associated with failure to control periprosthetic joint infection(PJI).METHODS: We retrospectively reviewed 38 consecutive hips managed using two-stage reimplantation with ALBC. The mean follow-up period was 5.4 years(range: 2.5-9 years). RESULTS: The causative pathogens were isolated from 29 patients(76%), 26 of whom were infected with highly virulent organisms. Sixteen patients(42%) underwent at least two first-stage debridements. An increased debridement frequency correlated significantly with high comorbidity(P < 0.001), a lower preoperative Harris hip score(HHS; P < 0.001), antimicrobial resistance, and gram-negative and polymicrobial infection(P = 0.002). Of the 35 patients who underwent two-stage reimplantation, 34 showed no signs of recurrence of infection. The mean HHS improved from 46 ± 12.64 to 78 ± 10.55 points, with 7(20%), 12(34%), 11(32%)and 5(14%) patients receiving excellent, good, fair and poor ratings, respectively. CONCLUSION: The current study demonstrated that two-stage reimplantation could successfully treat PJI after hip arthroplasty. However, the ability of ALBC to eradicate infection was limited because frequent debridement was required in high-risk patients(i.e., patients who are either in poor general health due to associated comorbidities or harbor infections due to highly virulent, difficult-to-treat organisms). Younger patients have poorer biochemical outcome after radical prostatectomy in high-risk prostate cancer

The aim of this study was to investigate the prognostic significance of patient age with respect to tumour aggressiveness in men who underwent radical prostatectomy （RP） for prostate cancer. In this study, we reviewed the records of 743 patients who received RP without neoadjuvant or adjuvant therapy at our institution and were followed up for 〉2 years postoperatively. For our analyses, the patients were divided into two groups according to age： younger （〈60 years） and older （≥ 60 years）. Through uni- and multivariate analyses, associations of various clinicopathological parameters, including biochemical recurrence-free survival, with patient age, were evaluated among all patients, and the patients were stratified according to their D＇Amico risk classification. Among all subjects, younger （n= 126） and older （n=617） patients showed no significant differences regarding pathological parameters and biochemical recurrence-free survival （P=0.288）. For the high-risk group （n= 206）, younger patients had a lower rate of biochemical recurrence-free survival following surgery than older patients （P=0.017）, despite the fact that no significant differences were observed regarding various known prognostic parameters between the two age groups. In addition, multivariate analysis revealed that age was an independent predictor of biochemical recurrence-free survival among the high-risk group （P=O.O03）. Our results showed that relatively younger patients have a comparable biochemical outcome compared with their older counterparts following RP performed for prostate cancer. However, among patients with high-risk disease, younger patients have a worse biochemical outcome following RP compared with older patients.

The 5-year functional outcomes after radical prostatectomy： a real-life experience in Korea

We investigated the functional outcomes regarding erectile function and urinary continence up to 5 years following radical prostatectomy （RP） in a cohort of Korean men. We retrospectively analyzed the clinicopathologic data of 85 Korean men who received open uni- or bilateral nerve-sparing RP for clinically localized prostate cancer and were followed up for at least 5 years postoperatively. From medical records, patients＇ status regarding urinary and erectile function at baseline and postoperative followups after RP was assessed. At 24 and 60 months after RP, proportions of subjects continent （no pads used） were 89.4% and 97.6%, respectively （P = 0.007）. Excluding subjects （n = 24） who preoperatively reported having severe erectile dysfunction or lacked relevant informations, proportions of subjects capable of having vaginal intercourse regardless of erectile aid usage were 47.5% and 37.7% at 24 and 60 months from RP, respectively （P = 0.022）. Patient＇s age at surgery （P = 0.047） and salvage radiation therapy （P = 0.026） were observed to be significant predictors of having erections sufficient for intercourse at 60 months from RP in multivariate analysis. Our results showed that while patients＇ postoperative status regarding urinary continence at 2 years from RP is generally maintained or improved at 5 year point, erectile function was observed to significantly declined from 2 years to 5 years following RP. Such decline in erectile function following RP may be more significant among men who were relatively older at surgery or those who received salvage therapy during postoperative follow-ups.

Curvature range measurements of the arcuate fasciculus using diffusion tensor tractography

Because Broca＇s area and Wernicke＇s area in the brain are connected by the arcuate fasciculus, understanding the anatomical location and morphometry of the arcuate fasciculus can help in the treatment of patients with aphasia. We measured the horizontal and vertical curvature ranges of the arcuate fasciculus in both hemispheres in 12 healthy subjects using diffusion tensor tractography. In the right hemisphere, the direct curvature range and indirect curvature range values of the arcuate fasciculus horizontal part were 121.13 ± 5.89 and 25.99 ± 3.01 degrees, respectively, and in the left hemisphere, the values were 121.83 ± 5.33 and 27.40 ± 2.96 degrees, respectively. In the right hemisphere, the direct curvature range and indirect curvature range values of the arcuate fasciculus vertical part were 43.97 ± 7.98 and 30.15 ± 3.82 degrees, respectively, and in the left hemisphere, the values were 39.39 ± 4.42 and 24.08 ± 4.34 degrees, respectively. We believe that the measured curvature ranges are important data for localization and quantitative assessment of specific neuronal pathways in patients presenting with arcuate fasciculus abnormalities.

Flow Visualization and Performance Measurements of a Flagellar Propeller

A new type of propeller that is optimized for low Reynolds numbers is required to propel a small object in a medium where the flow is dominated by viscous rather than inertial forces. A propeller in the shape of a bacterial flagellum seems an appro- priate choice for driving a small object. Accordingly, in this study, we visualized the velocity field induced by a spring-like propeller inspired by the Escherichia coli flagellum, using a macroscopic model and applying stereoscopic particle image velocimetry. We also experimentally evaluated the effect of pitch and rotational speed on the performance of this flagellar propeller. Silicone oil, which has a kinematic viscosity 100,000 times that of water, was used as the working fluid to generate a low Reynolds number for the macroscopic model. Thrust, torque, and velocity were measured as functions of pitch and rota- tional speed, and the efficiency of the propeller was calculated from the measured results. We found that the flagellar propeller reached a max!mum efficiency when the pitch angle was approximately 53°. Compared to pitch, rotational speed had a relatively small effect on the efficiency, and the pitch altered the flow pattern behind the rotating propeller.

Effects of edible insect extracts on the antioxidant, physiochemical, and microbial properties of Tteokgalbi during refrigerated storage

In this study,Tteokgalbi,a meat product,was prepared with edible insect larvae extracts and its antioxidant,physiochemical,and microbial properties were evaluated during refrigerated *** prepared with 2%*** and 2%*** showed the highest radical-scavenging activity and metal-chelating activity,but the lowest thiobarbituric acid reactive substance(TBARS)value,among the treatments(P<0.05).In addition,those antioxidant activities were the lowest and the TBARS value was the highest in control Tteokgalbi(P<0.05)during all storage *** prepared with 2%*** showed the highest brownness index,pH,and cooking yield,but the lowest shear force value(P<0.05).During all storage days,the total aerobic bacteria level did not differ significantly among the *** conclusion,*** extracts(2%)were effective as antioxidants,inhibiting lipid oxidation,and improving the quality properties of Tteokgalbi,without negatively affecting microbial *** results confirmed that the edible insect extracts have industrial value as a food additive.
