Background We used abdominal ultrasound scan （USS）, computed tomography （CT） and magnetic resonance imaging （MRI） findings in venous spread of renal cell carcinoma （RCC） to determine the superior extent of inferior vena cava （IVC） thrombus and IVC wall invasion and compared them with surgical and pathological reports. Methods From January 1999 to August 2007, 25 patients were diagnosed with RCC with IVC tumour thrombus. Before their operation, all patients had USS, contrast enhanced CT and MRI to find the superior extent of tumour thrombus and IVC wall invasion. All postprocessing techniques were performed by experienced radiologists. Two pathologists reported on all pathology specimens. The superior extent of tumour thrombus was confirmed by the senior surgeon at each operation, using the levels of thrombus defined according to 2004 Mayo Clinic classification. The radiographic results were compared with surgical and pathological findings. Results All patients had radical nephrectomy and tumour thrombus excision. Eight patients had RCC on the left side and 17 on the right side. According to the clinical and pathological findings, 6 patients had level I tumour thrombus, 9 level II, 5 level III and 5 level IV. Six patients had IVC wall invasion. No patient had evidence of lymph node or distant metastases. Of the 25 patients, USS correctly diagnosed the superior extent of tumour thrombus in 18/25, CT 23/25 and MRI 23/25. USS found 1 case of IVC wall invasion preoperatively. Conclusions Multidectector computed tomography and magnetic resonance imaging are comparable and more effective than abdominal ultrasound in diagnosing inferior vena cava tumour thrombus in renal cell carcinoma. None of the three methods can detect inferior vena cava wall invasion.

INTRODUCTION Sorafenib is a multi-kinase inhibitor that inhibits angiogenesis by targeting the vascular endothelial growth factor receptor 2 and platelet-derived growth factor receptor pathways while blocking cell proliferation by targeting the Ras/mitogen-activated protein kinase signaling *** global phase III trials[Sorafenib Hepatocellular Carcinoma Assessment Randomized Protocol(SHARP)[1]and Asia-Pacific trial[2]]showed that sorafenib prolonged the survival of patients with advanced hepatocellular carcinoma(HCC).The results of these studies were rapidly disseminated worldwide and were enthusiastically accepted by physicians specializing in liver cancer *** on the positive results of the SHARP trial,[1]the EU and USA approved sorafenib for advanced HCC in October and November 2007,*** was also approved for patients with unresectable and metastatic HCC in July 2008 in China,but patients have to pay the cost by themselves.

AIM： To find the precautions against the safety in caudate lobe resection. METHODS： The clinical data obtained from 11 cases of primary liver cancer in caudate lobe who received hepatectomy successfully were retrospectively analyzed. Four safe procedures were used in resection of primary liver cancer in caudate lobe：（1） selection of appropriate skin incision to obtain excellent exposure of operative field; （2） adequate mobilization of the liver to allow the liver to be displaced upwards to the left or to the right; （3） preparatory placement of tapes for total hepatic vascular exclusion, so that this procedure could be used to control the fatal bleeding of the liver when necessary; （4） selection of the ideal route for hepatectomy based on the condition of the tumor and the combined removal of multiple lobes if necessary. Among the 11 cases, simple occlusion of vessels of porta hepatis was used in caudate Iobectomy for 6 cases, while in the other cases, the vessels were intermittently occluded several times or total hepatic vascular isolation was used in the caudate Iobectomy. Combined partial right hepatectomy was done for 3 cases, combined left lateral Iobectomy for 2 cases and caudate Iobectomy alone for 6 cases. RESULTS： Operation was successful for all the 11 cases. Intermittent inflow occlusion was performed for all patients for 15 min at 5-min intervals. Blockade was performed twice in 3 patients and total hepatic vascular exclusion was performed in one of the three patients. Blockade was performed three times in one patient, including a total hepatic vascular exclusion. Total hepatic vascular exclusion was performed only in one patient. The mean blood loss was 300 mL. Ascites and pleural effusion occurred in 4 patients, jaundice in 1 patient. Six patients died of tumor recurrence in 6, 11, 12, 13, 15, 19 mo after operation, respectively. The other 5 patients have survived more than 16 mo since the operation. CONCLUSION： Caudate Iobectomy for liver cance

Transition metal-based nanomaterials have emerged as promising electrocatalysts for oxygen evolution reaction(OER).Considerable research efforts have shown that self-reconstruction occurs on these nanomaterials under operating conditions of OER ***,most of them undergo incomplete reconstruction with limited thickness of reconstruction layer,leading to low component utilization and arduous exploration of real catalytic ***,we identify the dynamic behaviors in complete reconstruction of Co-based complexes during *** hollow phytic acid(PA)cross-linked CoFe-based complex nanoboxes with porous nanowalls are designed because of their good electrolyte penetration and mass transport ability,in favor of the fast and complete reconstruction.A series of experiment characterizations demonstrate that the reconstruction process includes the fast substitution of PA by OH-to form Co(Fe)(OH)xand subsequent potential-driven oxidation to Co(Fe)*** obtained CoFeOOH delivers a low overpotential of 290 mV at a current density of 10 mA cm^(-2)and a long-term *** experiment results together with theory calculations reveal that the Fe incorporation can result in the electron rearrangement of reconstructed CoFeOOH and optimization of their electronic structure,accounting for the enhanced OER *** work provides new insights into complete reconstruction of metal-based complexes during OER and offers guidelines for rational design of high-performance electrocatalysts.

Postoperative pleural effusion occurs frequently after hepatectomy. The risk factors, prevention and management of postoperative pleural effusion in patients with primary liver cancer (PLC) who have undergone hepatectomy and the value of the argon beam coagulator (ABC) for the prevention of pleural effusion are studied. METHODS:A total of 523 patients with PLC at our institution who had had right hepatectomy from July 2000 to June 2004 were studied retrospectively. Comparative analysis was made to identify the factors contributing to postoperative pleural effusion and the efficacy of various managements. RESULTS:Of the 523 patients whose livers were dissociated using argon beam cutting and/or coagulation, 20(3.8%) developed pleural effusions;whereas in the other 467 patients underwent hepatectomy with suture ligation of the diaphragmatic secondary wound surface during the same period, 49(10.5%) had pleural effusion (Pying cirrhosis. CONCLUSIONS: Dissociation of the liver by argon beam cutting and/or coagulation can save suture ligation of the diaphragmatic secondary wound surface and may also prevent postoperative pleural effusion. Pleural drainage using an indwelling central-venous-catheter (CVC) in the pleural cavity is safe and efficacious.

β-ray-induced X-ray spectroscopy(BIXS)is a promising method for tritium detection in solid materials because of its unique advantages,such as large detection depth,nondestructive testing capabilities,and low requirements for sample ***,high-accuracy reconstruction of the tritium depth profile remains a significant challenge for this *** this study,a novel reconstruction method based on a backpropagation(BP)neural network algorithm that demonstrates high accuracy,broad applicability,and robust noise resistance is *** average reconstruction error calculated using the BP network(8.0%)was much lower than that obtained using traditional numerical methods(26.5%).In addition,the BP method can accurately reconstruct BIX spectra of samples with an unknown range of tritium and exhibits wide applicability to spectra with various tritium ***,the BP network demonstrates superior accuracy and stability compared to numerical methods when reconstructing the spectra,with a relative uncertainty ranging from 0 to 10%.This study highlights the advantages of BP networks in accurately reconstructing the tritium depth profile from BIXS and promotes their further application in tritium detection.

The oxygen reduction reaction(ORR)is a crucial process in Zn-air systems,and the catalyst plays a significant role in this ***,reported catalysts often suffer from poor durability and stability during the ORR ***,we synthesized La-Fe bimetallic nanoparticles encapsulated in a N-doped porous carbon dodecahedron(La-Fe/NC)originated from ZIF-8 by a simple direct *** La-Fe/NC catalyst had a numerous mesopores and dendritic outer layer generated by carbon nanotubes(CNTs),forming a high conductivity network that helped to optimize electron transfer and mass transport in the ORR *** effect of different doping transition metals and metal ratios on the ORR activity of Zn-air batteries was *** alkaline media,the La-Fe/NC showed the highest ORR catalytic activity,with a half-wave potential(E_(1/2))of 0.879 V(***,Pt/C 0.845 V).After 5000 cycles,the E_(1/2)of the La-Fe/NC catalyst only decreased by 7 m V,and its performance in stability tests and methanol tolerance tests was superior to Pt/*** used as the air electrode in a Zn-air battery,the La-Fe/NC catalyst demonstrated an excellent specific capacity of 755 m Ah/g and a peak power density of179.8 m W/cm~*** results provide important insights for the development of high-performance Zn-air batteries and new directions for the design of ORR catalysts.
