Case Report of Combined Sixth and Twelfth Cranial Nerve Palsy: A Rare Case of Clival Syndrome Arising from Thymoma

Godtfredsen syndrome or clival syndrome is a rare syndrome of abducens and hypoglossal nerve palsies (cranial nerve 6th and 12th respectively) that localizes to a clival mass. There are few reported cases of this clinical presentation. The aim of this case report is to describe this rare manifestation observed in a woman with clival metastases arising from a thymoma. A previously well 34-year-old native lady presented to a district hospital in Sabah, Malaysia, with history of blurring of vision and headache for 1 month. Cranial nerve examination reveals right abducens nerve palsy (right 6th CN) and right hypoglossal nerve palsy (right 12th CN). Initial imaging with CT brain reveals a subtle extra-axial hyperdense mass adjacent to the clivus and a routine chest x-ray reveals a mediastinal mass. Further imaging of thorax showed right anterior mediastinal mass, which then proceeded with Video-Assisted Thoracoscopic Surgery (VATS) guided biopsy. The biopsy result was consistent with the finding of thymoma, type B2. She was then diagnosed with aggressive form of thymoma, which unfortunately has metastasized to the bone, lung, liver and brain. Despite prognosis at the time of diagnosis is guarded, she still opted to undergo chemotherapy. Despite the completion of 6 cycles of chemotherapy, her disease progressed, and she eventually succumbed to the illness. In short, the presence of combined 6th and 12th palsy should alert clinician to the possibility of clival mass or metastases and hence could earlier workup with appropriate imaging can lead to earlier diagnosis and better treatment outcome.

ASLOP:A field-access affinity-based structure data layout optimizer

By rearranging the data,data layout optimizations improve the utilization of a cache line between two of its successive refills,thus reducing the total number of cache line refills and improving the performance of a *** this paper,we show that to enable structure data layout optimizations to be effective,two parameters,namely intra-instance affinity and inter-instance affinity,need to be considered at the same time in order to model the cache line utilization more *** also propose a lightweight approach to measure intra-instance affinity and inter-instance affinity to avoid complex memory trace analyses.A prototype,called ASLOP,has been implemented in the Open64 compiler and evaluated using benchmarks from SPEC CPU 2000,SPEC CPU 2006 and Olden benchmark suites that have extensive structure *** approach can achieve up to 48.1% performance improvement over the original programs,and 11.9% over the optimized programs using maximal reshaping,an existing approach that is known to produce close to the best results,on the two platforms we tested.

Recently, two-phase schemes for removing salt-and-pepper and random-valued impulse noise are proposed in [6, 7]. The first phase uses decision-based median filters to locate those pixels which are likely to be corrupted by noise (noise candidates). In the second phase, these noise candidates are restored using a detail-preserving regularization method which allows edges and noise-free pixels to be preserved. As shown in [18], this phase is equivalent to solving a one-dimensional nonlinear equation for each noise *** can solve these equations by using Newton's method. However, because of the edgepreserving term, the domain of convergence of Newton's method will be very narrow. In this paper, we determine the initial guesses for these equations such that Newton's method will always converge.

Numerical characteristics of the full operator hybrid simulation method

The full operator method （FOM） has been proposed to overcome some of the shortcomings of the commonly used operator splitting method （OSM）. In particular, the FOM is improved by increasing the accuracy of both the predictor and corrector using the estimated tangent stiffness of the tested structure. The numerical characteristics of the FOM, including stability and accuracy, are investigated in this study. It is shown that FOM is conditionally stable. The stability and accuracy characteristics are dependent on the accuracy of the estimated tangent stiffness and the parameters associated with the acceleration variation in the time-stepping integration method. Mass-spring systems with different types of nonlinearity, including hardening, stiffening, and softening behavior, are used to evaluate the performance of the FOM. It is found that the FOM can capture these types of nonlinearity with satisfactory accuracy. Using a prototype 12-story composite coupled wall system, the influences of the strong nonlinearity of the system as well as the displacement control errors from hydraulic actuators on the performance of the FOM are explored. The results show that the FOM is capable of generating reasonably accurate results despite the presence of strong structural nonlinearity and displacement control errors.

Improved Dual-image Quality with Reversible Data Hiding Using Translocation and Switching Strategy

Dual-image reversible data hiding(RDH)is a technique for hiding important *** technology can be used to safely deliver secret mes-sages to the recipient through dual images in an open network without being easily *** recipient of the image must receive the two stego-images before the secret message can be completely *** is one of the main advantages of data hiding technology;to increase the imperceptibility,the quality requirements of the stego-images are relatively important.A dual ste-ganographic image RDH method,called a DS-CF scheme that can achieve a bet-ter steganographic image quality using the center folding(CF)*** this paper,we developed a translocation and switching strategy(TaS)to shorten the distances between the stego-pixel coordinates and the cover pixel coordinates after information being *** with the DS-CF scheme,our proposed DS-TaS scheme can effectively improve the quality of the steganographic images at the same level of embedding *** experimental results show that the PSNR of our DS-TaS scheme at k=1 was 55.66 dB,which is an increase of 1.5 dB,and is 51.43 dB for k=2,46.66 dB for k=3,and 40.91 dB for k=*** addition,the PSNR values of the stego images was increased by 1.5,0.29,0.29,and 0.19 dB,*** shows that our proposed dual-image RDH method can optimize the visual quality of the stego-images and is better than many other dual-image RDH techniques.

Interplay of laser power and pore characteristics in selective laser melting of ZK60 magnesium alloys:A study based on in-situ monitoring and image analysis

This study offers significant insights into the multi-physics phenomena of the SLM process and the subsequent porosity characteristics of ZK60 Magnesium(Mg)***-speed in-situ monitoring was employed to visualise process signals in real-time,elucidating the dynamics of melt pools and vapour plumes under varying laser power conditions specifically between 40 W and 60 *** morphological analysis was performed using Scanning-Electron Microscopy(SEM),demonstrating a critical correlation between laser power and pore *** laser power led to increased pore coverage,whereas a denser structure was observed at higher laser *** laser power influence on porosity was further confirmed via Optical Microscopy(OM)conducted on both top and cross-sectional surfaces of the *** increase in laser power resulted in a decrease in pore coverage and pore size,potentially leading to a denser printed part of Mg alloy.X-ray Computed Tomography(XCT)augmented these findings by providing a 3D volumetric representation of the sample internal structure,revealing an inverse relationship between laser power and overall pore *** laser power appeared to favour the formation of interconnected pores,while a reduction in interconnected pores and an increase in isolated pores were observed at higher *** interplay between melt pool size,vapour plume effects,and laser power was found to significantly influence the resulting porosity,indicating a need for effective management of these factors to optimise the SLM process of Mg alloys.

Fertilization is a process involving multiple steps that lead to the final fusion of one sperm and the oocyte to form the *** of the steps,acrosome reaction(AR),is an exocytosis process,during which the outer acrosome membrane fuses with the inner sperm membrane,leading to the release of acrosome enzymes that facilitate sperm penetration of the egg *** AR has been investigated for decades,the initial steps of AR in vivo,however,remain largely unknown.A well elucidated model holds the view that AR occurs on the surface of the zona pellucida(ZP),which is triggered by binding of sperm with one of the ZP glycosylated protein,***,this model fails to explain the large number of‘falsely’acrosome-reacted sperms found within the cumulus layer in many species *** the emerging evidence of cross-talk between sperm and cumulus cells,the potential significance of AR in the cumulus oophorus,the outer layer of the egg,has been gradually *** we review the acrosome status within the cumulus layer,the cross-talk between sperm and cumulus cells with the involvement of a novel spermreleased factor,NYD-SP8,and re-evaluate the importance and physiological significance of the AR in the cumulus in fertilization.
