Multiple Cranial Nerve Palsies in Otolaryngology Consultation: An Atypical Clinical Presentation Revealing Myasthenia Gravis

Introduction: Myasthenia gravis is a chronic autoimmune neuromuscular disease, presents with weakness and fatigability of striated skeletal muscles. It is a rare disease in Cameroon. We report an uncommon case of myasthenia gravis in a patient with feeding difficulties, notion of oronasal reflux and swallowing disorders as first complaints. Observation: We report the case of a 29-year-old woman consulted at our department of Otolaryngology and Cervico-Facial Surgery for dysphagia and swallowing disorders. She also presented with facial diplegia, oculomotor paralysis, nasal voice, and dysarthria which has been evolving for several years now. The clinical examination revealed multiple cranial nerve palsies. The complementary workup showed a decrement of more than 50% in the electroneuromyography and the presence of anti-acetylcholine receptor autoantibodies in the blood workup. A diagnosis of myasthenia confirms clinical presumption. We initially observed a worsening of neuromuscular disorders despite the pyridostigmine treatment and subsequently a clear improvement of the clinical features concerning swallowing and speech disorders, oculomotricity and facial diplegia under a treatment combining prednisone and azathioprine. Conclusion: Myasthenia gravis is a rare and potentially fatal autoimmune neuromuscular disease. We thus highlight the atypical clinical presentation and therapeutic itinerary of our patient and the importance to think about this clinical diagnosis in front of any multiple cranial nerve paralysis in otolaryngology consultation.

Immuno-Histochemical Profile of Breast Cancers at the General Hospital of Douala-Cameroon

Introduction: The aim of the study was to describe the immunohistochemical aspects of breast cancers at Douala General Hospital, Cameroon. Methodology: This was a descriptive study with retrospective data collection, conducted from January 1st 2010 and December 31st 2019. It was focused on histologically proven breast cancers followed up at Douala General Hospital. Results: We collected 285 cases of breast cancer, all female, representing an annual frequency of 28.5 cases. The mean age of the patients was 48 ± 13 years. The most frequent histological form was infiltrating ductal carcinoma (83.9%), with grade II predominating (38.6%). Immunohistochemical analyses were performed on 89 patients (31.23%). The triple-negative subtype was the most common class (35.95%), followed by Luminal A (32.60);Human Epidermal Receptor 2-class and Luminal B tumors were observed at a frequency of 13.48% each. Treatment consisted mainly of surgery (78.60%) combined with chemotherapy (65.97%) and/or radiotherapy (63.16%). Hormonal treatment was used in only 10.18% of patients. Targeted therapy was exceptional (4%). Conclusion: Triple-negative phenotype is the most common. However, potentially hormone-sensitive tumors account for almost half of all patients, who are relatively young. Individualized treatments are rare. It is important to systematize these analyses for all breast cancers, with a view to appropriate management in our environment.

The decarbonization of the chemical industry and a shift toward circular economies because of high global CO_(2) emissions make CO_(2) an attractive feedstock for manufacturing ***,H_(2) is a low-cost and carbon-free reductant because technologies such as solar-driven electrolysis and supercritical water(scH_(2)O) gasification enable sustainable production of molecular hydrogen(H_(2)).We review the recent advances in engineering Ralsto-nia eutropha,the representative species of“Knallgas”bacteria,for utilizing CO_(2) and H_(2) to autotrophically produce 2,3-butanediol(2,3-BDO).This assessment is focused on state-of-the-art approaches for splitting H_(2) to supply en-ergy in the form of ATP and NADH to power cellular reactions and employing the Calvin-Benson-Bassham cycle for CO_(2) *** challenges and opportunities for application and future perspectives are discussed in the context of developing other promising CO_(2) and H_(2)-utilizing microorganisms,exemplified by Zymomonas mobilis.

The Epidemiological Profil of Digestive Cancers in Secondary and Tertiary Health Care Facilities in Cameroon

Background: Cancers have been a major public health problem in developing countries in recent years. The aim of this study was to determine the epidemiological profile of digestive cancers in secondary and tertiary health care facilities in Cameroon. Methodology: This was a cross sectional and descriptive study conducted over a period of 12 months in 14 health structures over the national territory. Included in our study were patients aged 15 and above with a histologically proven digestive cancer, patients with a clinical, biological and morphologic evidence of a digestive cancer. Socio-demographic (age, gender, region of origin, profession), clinical (symptoms on diagnosis, personal and family past history, consumption habits, tumour location) and paraclinical data were recorded on a pretested questionnaire. Data was analysed using SPSS version 20.0. Quantitative data was expressed as means with their corresponding standard deviations. Chi square was used for correlation between variables. A P value 0.05 was considered statistically significant. Results: Five hundred and eighty-two cases of digestive cancers were recorded out of 37,780 consultations/admissions during the study period giving a prevalence of 1.5% with a male predominance (58.1%). The mean age was 53.11 ± 17.26 years (15 - 99) with 33.8% of them below 45 years of age. Tumours were predominantly localized in the liver (43.5%) and colon (24.9%). Adenocarcinoma was the most common histological type in 44.5% of all cases. Alcohol consumption was found to be associated with colorectal cancer (p = 0.028) while tobacco consumption was found to be significantly associated with oesophageal cancer (p ≤ 0.001) and gastric cancer (p = 0.0047). Conclusion: A third of patients with digestive cancers were aged less than 45 years suggesting an early onset of these cancers in our setting. Relatively low prevalence with the liver being the most common site of localisation in our setting.

A General Model for Hepatitis B Disease with Age-Dependent Susceptibility and Transmission Probabilities

A SEI model for hepatitis B is constructed where the susceptibility and other crucial transmission probabilities depend on the chronological age and the basic reproduction rate R0 is derived. Under suitable (biological and mathematical) assumptions in a closed population, results of Houpa D. D. E. et al. [1] are extended from constant case of p and q to age-dependent case: the disease-free equilibrium is globally asymptotically stable (GAS) if R0 . On the other hand, R0 > 1 induces that endemic equilibrium is GAS and the system is uniformly persistent.

Structure of nanocrystalline WC-10%Co powder

Nanostructured WC Co powders obtained by mechanical milling were investigated by combination of X ray diffraction (XRD) and high resolution transmission electron microscopy (HRTEM) techniques. Rietveld analysis indicates that the experimental XRD patterns cannot be satisfactorily explained with the hexagonal WC structure. HRTEM image analysis shows that in the as milled nanostructured powder, many WC grains contain stacking faults lying on the plane {10·0}. Analysis of phase images show that these defects are nearly periodically ordered along the direction. Based on these observations, a structural model was proposed for the WC grains with ordered stacking faults, which is in fact equivalent to a superstructure of WC with space group Amm2. When this model describing the faulted fraction of WC is introduced together with the normal WC structure (space group P6-m2) into the Rietveld refinement, a much better agreement between the calculated and experimental XRD profiles was obtained. This study allows to obtain the lattice parameters, grain size, microstrain and other structural information on the as milled powders.

The Epidemio-Clinic Profile of Clandestine Abortion at Saint Joseph Hospital (Kinshasa)

Objective: To identify the epidemiologic profile of the woman having practiced the clandestine abortion, to determine the frequency and the complications of the clandestine abortion and to evaluate the prognosis of the clandestine abortion in our area. Method: The present study is descriptive and retrospective covering the period from January 2017 to December 2017, the one year period. On 118 patients received in the service for clandestine abortion, 60 were retained for the study. The 58 other files were excluded because of not containing sufficient information for this study. The descriptive statistical analyses were applied for the data analysis. Results: The frequency of the allowed patients for complication of clandestine abortion caused at the Saint Joseph hospital of Kinshasa was 6.03%. The most concerned population with this practice was less than 15 years and more than 45 years;they were unmarried (41.6%) and nulliparous (41.6%) in most cases. Ancillary medical staff (especially nurses) was accused the most in this practice of the illegal abortion caused by the dilatation and curettage with 50%. The genital haemorrhage constitutes the principal complication and reason for consultation and accounted for 78.3%. The assumption of responsibility is primarily medical and surgical. The vital prognosis is satisfactory for the immediate one, because no death is noted. Conclusion: The clandestine abortion remains public health problem in our communities. The results found in our study call for certain reflections so that to reduce its frequency and to avoid its complications.
