STRICTA清单是针对针刺临床试验中干预措施报告的指南。作为CONSORT清单的扩展版,STRICTA清单促进了针刺临床研究干预措施报告质量的提升。随着时代的发展,临床研究范式不断更新,现有的STRICTA清单能否继续指导当下及未来的临床研究的报告,是亟需考虑的问题。本文系统梳理了STRICTA清单的发展与更新过程,对STRICTA清单的效用特点及有待改进之处进行了分析,并就当前遇到的困境与挑战提出了几点改进建议,以促进STRICTA清单的进一步更新与完善。

Microbiome changes in the gastric mucosa and gastric juice in different histological stages of Helicobacter pylori-negative gastric cancers

BACKGROUND The gastric microbiota in patients with gastric cancer(GC)has received increasing attention,but the profiling of the gastric microbiome through the histological stages of gastric tumorigenesis remains poorly understood,especially for patients with Helicobacter pylori-negative GC(HPNGC).AIM To characterize microbial profiles of gastric mucosa and juice for HPNGC carcinogenesis and identify distinct taxa in precancerous *** The 16S rRNA gene analysis was performed on gastric mucosa from 134 Helicobacter pylori-negative cases,including 56 superficial gastritis(SG),9 atrophic gastritis(AG),27 intestinal metaplasia(IM),29 dysplasia(Dys),and 13 GC cases,to investigate differences in gastric microbial diversity and composition across the disease *** addition,paired gastric mucosa and juice samples from 18 SG,18 IM,and 18 Dys samples were analyzed.α-Diversity was measured by Shannon and Chao1 indexes,andβ-diversity was calculated using partial least squares discrimination analysis(PLS-DA).Differences in the microbial composition across disease stages in different sample types were assessed using the linear discriminant analysis effect *** The diversity and composition of the bacterial microbiota in the gastric mucosa changed progressively across stages of gastric *** diversity of the gastric mucosa microbiota was found to be significantly lower in the IM and Dys groups than in the SG group,and the patients with GC had the lowest bacterial community richness(P<0.05).Patients with IM and those with Dys had similar gastric mucosa microbiota profiles with Ralstonia and Rhodococcus as the predominant *** network analysis showed that there was increasing correlation strength between IM and Dys(|correlation threshold|≥0.5,P<0.05).GC and its precancerous lesions have distinguishable bacterial taxa;our results identified HPNGC-associated bacteria Streptococcaceae and Lactobacillaceae(P<0.05).Additionally,across precan

Application of immune checkpoint inhibitors and microsatellite instability in gastric cancer

In this editorial we comment on the article by Li published in the recent issue of the World Journal of *** focus specifically on the application of immune checkpoint inhibitors(ICIs)and microsatellite instability(MSI)in gastric cancer(GC).The four pillars of GC management have long been considered,including surgery,chemotherapy,radiotherapy and targeted ***,immunotherapy has recently emerged as a“fifth pillar”,and its use is rapidly *** are four principal strategies for tumor immunotherapy:ICIs,tumor vaccines,adoptive immunotherapy and nonspecific *** them,ICIs are the most advanced and widespread type of cancer immunotherapy for *** breakthrough results for ICIs have paved the way to a new era of cancer *** particular,inhibition of the PD-1/PD-L1 axis with ICIs,including nivolumab and pembrolizumab,has emerged as a novel treatment strategy for advanced ***,these therapies are sometimes associated with often subtle,potentially fatal immune-related adverse events(irAEs),including dermatitis,diarrhea,colitis,endocrinopathy,hepatotoxicity,neuropathy and *** must be aware of these irAEs and improve the detection of these processes to prevent inappropriate discharges,emergency department revisits,and downstream *** studies have revealed that MSI-high or mismatch-repair-deficient tumors,regardless of their primary site,have a promising response to ***,it is important to detect MSI before applying ICIs for treatment of GC.

Placebo Response among Different Types of Sham Acupuncture for Low Back Pain:A Systematic Review and Meta-Analysis of Randomized Controlled Trials

Background: Low back pain(LBP) is a prevalent and debilitating condition that poses a significant burden on healthcare systems. Acupuncture has been proposed as a promising intervention for LBP, but the evidence supporting its specific effect is insufficient, and the use of sham acupuncture as a control in clinical trials presents challenges due to variations in sham acupuncture techniques and the magnitude of the placebo ***: To investigate the magnitude of the placebo response of sham acupuncture in trials of acupuncture for nonspecific LBP, and to assess whether different types of sham acupuncture are associated with different responses. Methods: Four databases including PubMed, EMBASE, MEDLINE, and the Cochrane Library were searched through April 15, 2023, and randomized controlled trials(RCTs) were included if they randomized patients with LBP to receive acupuncture or sham acupuncture intervention. The main outcomes included the placebo response in pain intensity, back-specific function and quality of life. Placebo response was defined as the change in these outcome measures from baseline to the end of treatment. Random-effects models were used to synthesize the results, standardized mean differences(SMDs, Hedges’ g) were applied to estimate the effect size. Results: A total of 18 RCTs with 3,321 patients were included. Sham acupuncture showed a noteworthy pooled placebo response in pain intensity in patients with LBP [SMD –1.43, 95% confidence interval(CI) –1.95 to –0.91, I2=89%]. A significant placebo response was also shown in back-specific functional status(SMD –0.49, 95% CI –0.70 to –0.29, I2=73%),but not in quality of life(SMD 0.34, 95% CI –0.20 to 0.88, I2=84%). Trials in which the sham acupuncture penetrated the skin or performed with regular needles had a significantly higher placebo response in pain intensity reduction, but other factors such as the location of sham acupuncture did not have a significant impact on the placebo ***

Resilient performance of self-centering hybrid rocking walls with curved interface under pseudo-static loading

Frame and rocking wall(FRW)structures have excellent resilient performance during ***,the concrete at interfacial corners of rocking walls(RWs)is easily crushed due to local extreme compression during the rocking *** innovative RW with a curved interface is proposed to prevent interfacial corners from producing local damage,enhancing its earthquake resilient performance(ERP).The precast wall panel with a curved interface is assembled into an integral self-centering hybrid rocking wall(SCRW)by two post-tensioned unbonded prestressed ***,two ordinary energy dissipation steel rebars and two shear reinforcements are arranged to increase the energy dissipation capacity and lateral *** SCRW specimens and one monolithic reinforced concrete(RC)shear wall(SW)were tested under pseudo-static loading to compare the ERPs of the proposed SCRW and the SW,focusing on studying the effect of the curved interface on the *** key resilient performance of rocking effects,failure modes,and hysteretic properties of the SCRW were *** results show that nonlinear deformations of the SCRW are concentrated along the interface between the SCRW and the foundation,avoiding damage within the *** restoring force provided by the prestressed tendons can effectively realize self-centering capacity with small residual deformation,and the resilient performance of the SCRW is better than that of monolithic *** addition,the curved interface of the SCRW makes the rocking center change and move inward,partially relieving the stress concentration and crush of *** rocking range of the rocking center is about 41.4%of the width of the SCRW.

In this paper,we consider the clustering of bivariate functional data where each random surface consists of a set of curves recorded repeatedly for each *** k-centres surface clustering method based on marginal functional principal component analysis is proposed for the bivariate functional data,and a novel clustering criterion is presented where both the random surface and its partial derivative function in two directions are *** addition,we also consider two other clustering methods,k-centres surface clustering methods based on product functional principal component analysis or double functional principal component *** results indicate that the proposed methods have a nice performance in terms of both the correct classification rate and the adjusted rand *** approaches are further illustrated through empirical analysis of human mortality data.

Experimental and numerical analyses of the effect of fibre content on the close-in blast performance of a UHPFRC beam

Limited research has been conducted on the influences of fiber content on close-in blasting characteristics for ultrahigh-performance fiber-reinforced concrete(UHPFRC)*** paper aims to address this knowledge gap through experimental and mesoscale numerical *** were conducted on ten UHPFRC beams built with varying steel fiber volumetric fractions subjected to close-in explosive ***,this study considered other parameters,such as the longitudinal reinforcement type and *** the case of UHPFRC beams featuring normal-strength longitudinal reinforcement of diametersΦ12,Φ16,andΦ20,a reduction in maximum displacement by magnitudes of19.6%,19.5%,and 17.4%was observed,respectively,as the volumetric fractions of fiber increased from1.0%to 2.5%.In addition,increasing the longitudinal reinforcement ratio and using high-strength steel longitudinal reinforcement both significantly reduced the deformation characteristics and increase the blasting resistances of UHPFRC ***,the effects on the local crushing and spalling damage were not significant.A mesoscale finite element model,which considers the impacts of fiber parameters on UHPFRC beam behaviors,was also established and well correlated with the test ***,parametric analyses were further conducted to examine the impacts of the steel fiber content and length and the hybrid effects of various types of microfibers and steel fibers on the blasting performance of UHPFRC beams.
