AIM:To determine the role of heparanase-1(HPSE-1)in orbital rhabdomyosarcoma(RMS),and to investigate the feasibility of HPSE-1 targeted therapy for RMS.METHODS:Immunohistochemistry was performed to analyze HPSE-1 expression in 51 cases of orbital RMS patients(including 28 cases of embryonal RMS and 23 cases of alveolar RMS),among whom there were 27 treated and 24 untreated with preoperative chemoradiotherapy.In vitro,studies were conducted to examine the effect of HPSE-1 silencing on RMS cell proliferation and tube formation of human umbilical vein endothelial cells(HUVECs).RD cells(an RMS cell line)and HUVECs were infected with HPSE-1 sh RNA lentivirus at a multiplicity of infection(MOI)of 10 and 30 separately.Real-time PCR and Western blot were applied to detect the m RNA and protein expression levels of HPSE-1.Cell viability of treated or control RD cells was evaluated by cell counting kit-8(CCK-8)assay.Matrigel tube formation assay was used to evaluate the effect of HPSE-1 RNAi on the tube formation of HUVECs.RESULTS:Immunohistochemistry showed that the expression rate of HPSE-1 protein was 92.9%in orbital embr yonal RMS and 91.3%in orbital alveolar RMS.Tissue from alveolar orbital RMS did not show relatively stronger staining than that from the embryonal orbital RMS.However,despite the types of RMS,comparing the cases treated chemoradiotherapy with those untreated,we have observed that chemoradiotherapy resulted in weaker staining in patients’tissues.The expression levels of HPSE-1 declined significantly in both the m RNA and protein levels in HPSE-1 sh RNA transfected RD cells.The CCK-8 assay showed that lentivirus-mediated HPSE-1 silencing resulted in significantly reduced RD cells viability in vitro.Silencing HPSE-1 expression also inhibited VEGF-induced tube formation of HUVECs in Matrigel.CONCLUSION:HPSE-1 silencing may be a promising therapy for the inhibition of orbital RMS progression.

Recurrent orbital space-occupying lesions:a clinicopathologic study of 253 cases

Objective：To analyze the clinical features,histopathologic classification and frequencies of various types of recurrent orbital space-occupying lesions.Methods：A retrospective study was carried out in 253 consecutive patients with recurrent orbital spaceoccupying lesions treated by surgical excision in the Institute of Orbital Diseases,the General Hospital of the Armed Police Force from January 2009 to December 2010.Results：The patients included 123 males and 130 females aged 2 to 78 years（mean,36.2 years）,and the last recurrence interval after operation ranged from 1 month to 40 years（median,4.75 years）.Of all the cases,159（62.8%）,65（25.7%）,20（7.9%）,8（3.2%） and 1（0.4%） had previously experienced once,twice,three,four and six times of surgeries,respectively.Among them,29（11.5%） cases had recurred 3 times or over,and 37（14.6%） cases got recurrence in 10 or more years postoperatively.Most of the patients with local recurrence presented with various clinical manifestations,while 31（12.3%） cases were symptom-free.Two hundred and thirty-one（91.3%） cases underwent surgical removal of the recurrent orbital lesions,and another 22（8.7%） cases had to receive the exenteration of orbit.Categories of these recurrent orbital lesions after operation were as follows：lacrimal gland tumors,65（25.7%） cases;vasogenic diseases,54（21.3%） cases;neurogenic tumors,42（16.6%） cases;secondary tumors,24（9.5%） cases;orbital inflammation,21（8.3%） cases;myogenic tumors,14（5.5%） cases;fibrous and adipose tumors,12（4.7%） cases;lympho-hematopoietic tumors,7（2.8%） cases;bone or cartilage tumors,7（2.8%） cases;orbital cysts,6（2.4%） cases;and indefinitely differentiated tumor,1（0.4%） case.The 10 top histopathologic diagnoses were lacrimal gland pleomorphic adenoma,hemangiolymphangioma,lacrimal gland adenoid cystic carcinoma,meningioma,inflammatory pseudotumor,neurofibroma,sebaceous gland carcinoma,vascular malformation,rhabdomyosarcoma and hemangioma.Conclusions：The variety of recurrent orbital lesions after operation includes mainly of tumors except

Dear Editor,We are writing to present a rare case of disseminated hydatid disease(HD)in the orbit and central nervous system(CNS)leading to visual dysfunction,and an improved surgical technique.This rare case demonstrates intracranial multiple hydatid cysts encroaching and penetrating the orbital wall leading to orbital hydatid cyst.Surgically reducing the cyst volume may be an excellent option to avoid rupture of orbital hydatid cyst.

头颈部肿瘤是异质性最高的恶性肿瘤之一,多学科综合诊疗(MDT)是个体化精准诊疗和疾病全程管理的核心。MDT诊疗也是头颈部肿瘤重要的教学模式,但在实际教学中受到时空限制。通过构建基于互联网、医院HIS/PACS/LIS/EMR系统、医疗可视化大屏、口腔内窥镜、远程会诊平台及其他可接入的音视频终端等组成的互联网可视化平台,将其应用于头颈部肿瘤MDT临床教学中,让医学生和临床规培学员通过联网可视化平台参与头颈部肿瘤MDT,深度学习这一异质性最大的恶性肿瘤,为临床医学教学模式提供新的途径。

新生儿乳糜胸临床特点及预后分析

目的探讨新生儿乳糜胸的临床特点及预后。方法回顾性分析北京儿童医院新生儿中心2016年6月至2023年6月明确诊断为乳糜胸新生儿的临床资料,根据乳糜胸发病原因分为先天性组和获得性组,比较两组临床特点、治疗方法及预后情况。结果共纳入23例患儿,先天性组17例(73.9%),获得性组6例(26.1%)。两组患儿性别、胎龄、出生体重比较差异无统计学意义(P>0.05)。与获得性组相比,先天性组淋巴细胞百分比(97.0%比85.0%)、使用红霉素(7/17比1/6)及奥曲肽(9/17比1/6)、特殊配方奶喂养(13/17比2/6)比例更高,右侧胸水发生率(1/17比3/6)、有创机械通气比例(6/17比6/6)、母乳喂养率(0/17比3/6)更低,差异均有统计学意义(P0.05)。18例经保守治疗胸腔积液吸收无复发;5例患儿住院期间死亡,其中4例因家长放弃治疗后死亡,1例胸导管结扎术后并发新生儿坏死性小肠结肠炎死亡。结论先天性乳糜胸与获得性乳糜胸严重程度、病程及总体预后相似,先天性乳糜胸使用红霉素及奥曲肽治疗率较获得性乳糜胸高。新生儿乳糜胸总体预后良好。

To the Editor:Although reports of neonatal upper airway obstruction in patients in neonatal intensive care units(NICUs)are common,neonatal tracheotomy is rare,possibly due to the small number of patients and the even smaller number of well-equipped hospitals to do a successful operation in neonates.However,despite the extensive efforts of qualified,skilled neonatologists and pediatric otorhinolaryngologists in performing a neonatal tracheotomy,some neonates did not undergo this surgery.When a neonate meets the indications of tracheotomy but his/her parent refuses to do the operation,he/she is facing a higher risk to be discharged against medical advice(DAMA)as per their parents’wish.To explore the preliminary evidence of neonatal tracheotomy in China,as well as to find out the outcome difference between neonates who underwent tracheotomy and those who did not undergo tracheotomy when they met the surgical indications,we conducted a retrospective study.Patients were treated in Beijing Children’s Hospital.Patients were categorized into the tracheotomy group or non-tracheotomy group according to whether their parents signed the tracheotomy consent and finally completed the tracheotomy.This study was approved by the Ethics Committee of Beijing Children’s Hospital,Capital Medical University(No.[2021]-E-139-R)and was registered in the Chinese Clinical Trial Registry(No.ChiCTR2100050720).All parents/guardians of participants provided written informed consent.

Environmental factors for the development of fetal urinary malformations

Background:The development of the kidneys and other organs of the urinary tract also follow the natural rule of gene-environment-lifestyle interaction.Both intrinsic and extrinsic factors may be associated with the etiology of various kinds of urinary malformations.The environmental factors belong to extrinsic factors,which have attracted increasing attention from researchers.Methods:Publications about urinary malformations were searched from databases such as PubMed,Elsevier,Chemical Abstract,Excerpta Medica,Chinese Hospital Knowledge Database and Wanfang Database.Results:Urinary malformation is associated with low birth weight,maternal diseases,placental insufficiency,maternal drug exposure,and maternal exposure to environmental pesticides.Living environment and socioeconomic factors may also influence the incidence of urinary malformation.Conclusion:It is important to understand the association of environmental factors with the development of the renal system and urinary malformation in order to decrease the incidence of urinary malformations.

目的比较华西心晴指数(hei)和护士用自杀风险评估量表(NGASR)在高自杀风险评估中的一致性,探讨hei在抑郁症患者高自杀风险中的预测价值。方法本研究为横断面研究设计,采用便利抽样,纳入2023年6-12月四川大学华西医院心理卫生中心386名抑郁症住院患者。纳入标准:根据国际疾病分类第十次修订版(ICD-10)确诊为抑郁症,年龄在18岁以上,完成了NGASR和hei问卷评估。排除标准:合并其他精神疾病的患者;存在严重认知障碍,无法进行有效沟通的患者。自编问卷收集年龄、性别、民族、婚姻状况和教育水平等人口学资料,同时采用hei问卷和护士用自杀风险评估表对患者进行评估。采用Spearman进行相关分析,对两个工具进行Kappa一致性检验,采用受试者工作特征曲线评估hei评分预测自杀高风险的性能,根据Youden指数确定hei分数的最佳截断值。结果本次研究共纳入386名抑郁症住院患者,平均年龄32岁,平均住院14 d。其中,女性252人(65.3%),男性134人(34.7%);89.4%为汉族,7.3%为藏族,其他少数民族占3.3%;51.3%的患者为已婚,41.2%未婚,其余为离异(6.5%)或丧偶(1.0%);26.2%的研究对象具有本科及以上文化程度,20.7%为大专文化程度,24.8%为高中或者中专文化程度,28.2%为初中及以下文化程度。386例抑郁症患者中,采用NGASR评估,高度自杀风险检出率为57.3%,采用hei评估,重度情绪不良检出率为53.6%。hei和NGASR评分一致性Kappa值为0.518(Phei评分筛查自杀高风险的ROC曲线下面积为0.829(95%可信区间:0.787~0.871),在hei评分达17分时,Youden指数最大为0.52,预测自杀高风险(NGASR≥9分)的特异性为76.36%,敏感性为76.02%。结论 hei和NGASR在抑郁症患者高自杀风险评估中具有中度的一致性,hei能有效预测抑郁症患者自杀高风险,17分为hei评估高自杀风险的最佳截断值。
