本文拟从整体上观察先秦至魏晋南北朝时期人们如何处理罪与罚的问题,从而探究中国人关于罪与罚的意识。先秦时期有个观念:上天根据人的品德而给予长短的寿命;也有个观念:自己的命运是早已因祖先行为的善恶而决定。到两汉时期就明确存在"罪"的观念,反省自身、感到自己有罪的情况,在汉代是很普遍的。祖先的罪过影响到自己,这样的看法比以前明显得多。关于"罪"的内容在文学作品当中虽不太多见,但在班固的《幽通赋》中能够看到,这很值得注意。可知当时人们认为,由于祖先的行为,自己出生时就背负了不幸和罪恶。在南北朝时期,所谓六朝志怪小说中,也包含了与罪过有关的、关于祖先与自己关系的观念。为什么大多汉代文学作品中几乎找不到"罪"的内容?尤其是诗中基本上见不到"罪",大概是因为诗这一文体有它自己的规则禁忌,即诗中不应表现这样关于"罪"的问题。《为焦仲卿妻作》等乐府民歌中时常出现"罪",这个事实也证明上面推测还是有合理性的。

Purpose: Intravitreal Triamcinolone Acetonide (IVTA) is a useful treatment option for various intraocular diseases such as Macular Edema (ME). The treatment can cause several complications, including transient elevation of intraocular pressure and cataract formation. Here, we describe an atypical complication of IVTA. Case: A 60-year-old Japanese man presented with ME associated with central retinal vein occlusion. We performed intravitreal injection of 4 mg of TA. However, the drug spread behind the posterior lens capsule and also flowed into the anterior chamber. Although the amount of TA particles behind the lens capsule decreased over time, these particles persisted for more than 2 months. Conclusion: Although IVTA is an easy and effective treatment for ME, TA is harmful when injected into inappropriate regions of the eye. Accurate IVTA injection is important for effective treatment.

Computer-Assisted Surgery for Mandibular Reconstruction Using a Patient-Specific Titanium Mesh Tray and Particulate Cancellous Bone and Marrow

Craniomaxillofacial surgery is difficult due to the complexity of the regional anatomy. Computer-assisted surgery is a promising tool aiming to improve the safety and precision of such surgery. A computer-assisted surgical navigation approach for reconstruction of mandibular defects using a patient-specific titanium mesh tray and particulate cancellous bone and marrow (PCBM) harvested from bilateral anterior ilia is proposed. This case report involves a large multicystic ameloblastoma affecting the right mandible of a 31-year-old male patient. Following detailed clinical examination, radiological interpretation, and histopathological diagnosis, computer-assisted surgical simulation with a virtual 3-dimensional (3-D) model was designed using surgical planning software based on the pre-operative computed tomography data. Long-span segmental resection of the mandible was planned, and the defect was analyzed for reconstruction using a patient-specific reconstruction titanium mesh tray mediated with computer-aided design and manufacturing (CAD/CAM) techniques. During the actual surgery, the ultrasonic bone cutting instrument in the surgeon’s hand was connected to the navigation system to touch an anatomical position on the patient. Therefore, osteotomies were performed finely and smoothly according to the navigation images of the cutting bone line by sequentially moving the instrument. Finally, a CAD/CAM-mediated titanium mesh tray condensed by PCBM was adapted to the remaining mandibular fragments. Six months postoperatively, the patient had a good mandibular configuration and facial contour. Integration of different technologies, such as software planning and 3-D surgical simulation, combined with intraoperative navigation and CAD/CAM techniques, provides safe and precise mandibular reconstruction surgery.

Background: Posttraumatic Stress Disorder (PTSD) is a psychiatric disorder found in individuals afflicted by a traumatic event including the natural disaster. “Tsunami” occurred in Andaman coast of Thailand on December 26, 2004, in which 33.6% of survivors were diagnosed as PTSD. This study aimed to explore the single nucleotide polymorphism (SNP). rs267943 genotype is located on chromosome 5 in the intron of the death-associated protein 1 (DAP1) gene and psychosocial factors for PTSD. Methods: Participants (N = 1970) were recruited from volunteers who have complete data both of DAP1 gene and psychosocial factor. Results: Using a binary logistic regression model, significant gene-environment interactions were found for the single nucleotide polymorphism (SNP) rs267943 and psychosocial factors including depression (adj. OR = 6.0, 95% CI = 4.29 - 8.39), neurotic personality (adj. OR = 2.73, 95% CI = 2.18 - 3.42), planning (adj. OR = 1.52, 95% CI = 1.20 - 1.93), use of emotional support (adj. OR = 1.32, 95% CI = 1.21 - 1.94) with statistical significant p Conclusion: This study demonstrated that GxE studies can be utilized to shed light on the origins of PTSD.

Posttraumatic stress disorder (PTSD) is a psychiatric disorder found in individuals afflicted by a traumatic event. Multiple environmental and genetic factors can contribute to PTSD susceptibility. Since it is rare to find members of the same family afflicted by the same catastrophic event, it is not practical to determine PTSD susceptibility genes by a gene linkage analysis. A natural disaster, such as the 2004 Tsunami, provided us with a rare chance for a genetic analysis of PTSD. To identify SNPs associated with PTSD susceptibility, we conducted a genome-association study (GWAS) in Thai-Tsunami survivors. Initial phase of the study with 396 chronic PTSD patients and 457 controls, we identified top ninety SNPs (P -4), which were further assessed in the second phase with 395 chronic PTSD patients and 798 controls. Two SNPs (rs267950 and rs954406), were identified in the second phase, and subjected to fine mapping using a data set from both phases. SNP rs267943 showed the strongest association with PTSD susceptibility and was in complete linkage disequilibrium with SNP rs267950 with P = 6.15 × 10-8, OR = 1.46 and 95% CI = 1.19 - 1.79, reaching genome-wide significance. SNP rs267943 is located on chromosome 5 in the intron of the death-associated protein 1 (DAP1) gene and, when linked to a synthetic promoter, could regulate transcription. To our knowledge, this is the first GWAS for PTSD susceptibility in an Asian population which could provide an important insight into the genetic contribution of PTSD and may lead to new treatment strategies for PTSD.
