Technical note on ALPPS for a patient with advanced hepatocellular carcinoma associated with invasion of the inferior vena cava

Patients with hepatocellular carcinoma have a very short life expectancy if they receive no surgical interven- tion. A relatively new surgical technique termed ＂Associating Liver Partition and Portal Vein Ligation for Staged Hepa- tectomy＂（ALPPS） has been employed for inducing rapid hy.ertrophy of the future liver remnant for patients waiting for hepatectomy. As portal vein embolization may not result in satisfactory hy.ertrophy before tumor progression occurs, ALPPS can be an alternative for patients with advanced hepa- tocellular carcinoma. Herein we describe an ALPPS procedure with tumor thrombectomy for a patient who had a small left liver lobe and a large hepatocellular carcinoma involving the whole right liver lobe and the middle hepatic vein and extend- ing into the inferior vena cava. In the first-stage operation, the right portal vein was controlled and divided with a Hemolock. The right hepatic artery was well protected. Hepatic transec- tion was performed with a 1-cm margin from the tumor. The middle hepatic vein trunk was preserved. Ten days afterwards, there was significant hy.ertrophy of the left lateral section of the liver, and the second-stage operation was conducted. ex- tended right hepatectomy and tumor thrombectomy were per- formed under sternotomy and total vascular exclusion. The patient had good recovery and was free of disease 10 months after the operation. ALPPS may be a good treatment option even for patients with advanced disease if carried out at high- volume centers.

As a joint effort between the Chinese Academy of Sciences and the Hong KongUniversity of Science and Technology, the BLOSSOMS sensor network project aims to identify researchissues at all levels from practical applications down to the design of sensor nodes. In thisproject, a heterogeneous sensor array including different ty.es of application-dependent sensors aswell as monitoring sensors and intruding sensors are being developed. Application-dependentpower-aware communication protocols are also being studied for communications among sensor nodes. Anontology-based middleware is built to relieve the burden of application developers from collecting,classifying and processing messy sensing contexts. This project is also developing a set of toolsallowing researchers to model, simulate/emulate, analy.e, and monitor various functions of sensornetworks.

There have been various theoretical attempts by researchers worldwide to link up different scales of plasticity studies from the nano-, micro- and macro-scale of observation, based on molecular dynamics, crystal plasticity and continuum mechanics. Very few attempts, however, have been reported in ultra-precision machining studies. A mesoplasticity approach advocated by Lee and yang is adopted by the authors and is successfully applied to studies of the micro-cutting mechanisms in ultra-precision machining. Traditionally, the shear angle in metal cutting, as well as the cutting force variation, can only be determined from cutting tests. In the pioneering work of the authors, the use of mesoplasticity theory.enables prediction of the fluctuation of the shear angle and micro-cutting force, shear band formation, chip morphology in diamond turning and size effect in nano-indentation. These findings are verified by.experiments. The mesoplasticity formulation opens up a new direction of studies to enable how the plastic behaviour of materials and their constitutive representations in deformation processing, such as machining can be predicted, assessed and deduced from the basic properties of the materials measurable at the microscale.

The CST bounce universe model——A parametric study

A bounce universe model with a scale-invariant and stable spectrum of primordial density perturbations was constructed using a consistent truncation of the D-brane dynamics from Ty.e IIB string theory. A coupling was introduced between the tachyon field and the adjoint Higgs field on the D3-branes to lock the tachyon at the top of its potential hill and to model the bounce process,which is known as the Coupled Scalar and Tachyon Bounce(CSTB) Universe. The CSTB model has been shown to be ghost free,and it fulfils the null energy condition; in addition, it can also solve the Big Bang cosmic singularity problem. In this paper we conduct an extensive follow-up study of the parameter space of the CSTB model. In particular we are interested in the parameter values that can produce a single bounce to arrive at a radiation-dominated universe. We further establish that the CSTB universe is a viable alternative to inflation, as it can naturally produce a sufficient number of e-foldings in the locked inflation epoch and in the post-bounce expansion to overcome the four fundamental limitations of the Big Bang cosmology, which are flatness, horizon,homogeneity and singularity, resulting in a universe of the current size.

The present paper evaluates the international status of the Chinese currency, the renminbi （RMB）, by.examining its use in the global market. Specifically, the discussion focuses on the recent developments of RMB trading in the global foreign exchange market, cross-border trade settlement in RMB, the Hong Kong offshore market and China＇s policies relating to the RMB. The evidence suggests that the use of the RMB overseas, especially in trade financing and in the off-shore market, has increased rapidly in recent years. However, compared with the size of the Chinese economy, the current scale of the use of the RMB is quite small. Although the RMB has great potential to become an international currency, its acceptance in the global economy is affected by both economic andpolitical factors. Attaining a fully fledged international RMB is still a distant goal.

AIM: α-Lipoic acid (ALA) has been used as an *** aim of this study was to investigate the effect of α-lipoic acid on cholecystokinin (CCK)-octapeptide induced acute pancreatitis in ***: ALA at 1 mg/kg was intra-peritoneally injected, followed by 75 μg/kg CCK-octapeptide injected thrice subcutaneously after 1, 3, and 5 h. This whole procedure was repeated for 5 d. We checked the pancreatic weight/body weight ratio, the secretion of pro-inflammatory cytokines and the levels of lipase,amylase of serum. Repeated CCK octapeptide treatment resulted in typical laboratory and morphological changes of experimentally induced ***: ALA significantly decreased the pancreatic weight/body weight ratio and serum amylase and lipase in CCK octapeptide-induced acute pancreatitis. However,the secretion of IL-1β, IL-6, and TNF-α were comparable in CCK octapeptide-induced acute ***: ALA may have a protective effect against CCK octapeptide-induced acute pancreatitis.

Reperfusion therapy is the preferred treatment for ischemic stroke,but is hindered by its short treatment window,especially in patients with diabetes whose reperfusion after prolonged ischemia is often accompanied by.exacerbated *** mechanisms underlying exacerbated hemorrhage are not fully *** study aimed to identify this mechanism by inducing prolonged 2-hour transient intraluminal middle cerebral artery occlusion in diabetic Ins2Akita/+mice to mimic patients with diabetes undergoing delayed mechanical *** results showed that at as early as 2 hours after reperfusion,Ins2Akita/+mice exhibited rapid development of neurological deficits,increased infarct and hemorrhagic transformation,together with exacerbated down-regulation of tight-junction protein ZO-1 and upregulation of blood-brain barrier-disrupting matrix metallopeptidase 2 and matrix metallopeptidase 9 when compared with normogly.emic Ins2+/+*** indicated that diabetes led to the rapid compromise of vessel integrity immediately after reperfusion,and consequently.earlier death and further aggravation of hemorrhagic transformation 22 hours after *** observation was associated with earlier and stronger up-regulation of pro-angiogenic vascular endothelial growth factor(VeGF)and its downstream phospho-erk1/2 at 2 hours after reperfusion,which was suggestive of premature angiogenesis induced by.early VeGF up-regulation,resulting in rapid vessel disintegration in diabetic *** reticulum stress-related pro-apoptotic C/eBP homologous protein was overexpressed in challenged Ins2Akita/+mice,which suggests that the exacerbated VeGF up-regulation may be caused by overwhelming endoplasmic reticulum stress under diabetic *** conclusion,the results mimicked complications in patients with diabetes undergoing delayed mechanical thrombectomy,and diabetes-induced accelerated VeGF up-regulation is likely to underlie exacerbated hemorrhagic transformat
