BACKGROUND Castleman's disease(CD)is a lymphoproliferative *** syndrome is classified as a variant of CD based on its key clinical manifestations of thrombocytopenia,anasarca(generalized edema and pleural effusion),fever(pyrexia),reticulin fibrosis in the bone marrow and the proliferation of megakaryocytes,and organomegaly(such as hepatosplenomegaly and multiple lymphadenopathies);TAFRO syndrome is mainly reported in Japanese *** our knowledge,this is the first pediatric case report detailing a CD-associated disorder progressing to *** SUMMARy A 10-year old male patient presented with fever and *** months before hospitalization,he had remarkable abdominal ***,he visited a clinic for a fever that lasted 5 *** physical findings were marked hepatosplenomegaly and cervical lymphadenopathy.A blood test revealed leukocytosis,microcytic anemia,aspartate aminotransferase-dominant transaminase elevation,high levels of C-reactive protein,polyclonal hypergammaglobulinemia,and high levels of interleukin-6 and vascular endothelial growth *** contrast computed tomography and magnetic resonance imaging suggested cirrhosis,which was confirmed by liver *** findings in the enlarged hepatic lymph nodes revealed both hyperplasia and atrophy of lymphoid follicles with some vascular hyperplasia and moderate plasmacytosis between the lymphoid follicles,which is compatible with lymph node histology in TAFRO *** was not effective in reducing the patient’s symptoms;therefore,the patient was prescribed *** date,the patient remains free of fever and continues to receive *** We described the clinicopathological features of TAFRO syndrome to highlight the clinical presentation of this rare disease in a pediatric case.

AIM To screen primary immunodeficiency,Wiskott-Aldrich syndrome(WAS),and chronic granulomatous disease(CGD) among children with inflammatory bowel disease(IBD).METHODS This was a single-center retrospective study. Eighteen children with IBD were investigated. We analyzed their expression of Wiskott-Aldrich syndrome protein(WASP) in lymphocytes and superoxide generation in phagocytes using flow cytometry. When the expression of WASP or superoxide generation was low or absent,we performed genetic analysis to determine the cause of this. RESULTS Eighteen patients were classified as having ulcerative colitis(n = 10),Crohn's disease(n = 5),or IBDunclassified(n = 3). In total,three patients revealed low expression of WASP associated with a WAS gene c.1378 C>T ***460 Ser mutation,which has previously been reported as a pathogenic mutation in WAS and X-linked thrombocytopenia. However,with respect to the major symptoms of WAS,none of these three patients showed either thrombocytopenia or increased susceptibility to infection,but one patient showed generalized eczema. No CGD patients were discovered in this *** Despite the lack of typical clinical manifestations of WAS,low expression of WASP could be associated with the pathogenesis of a subtype of IBD patients.

A novel technique for reducing reverberation artifact in acoustic shadow imaging using nonlinear ultrasound interaction, called nonlinear acoustic shadow method, has been developed and experimentally studied. In this technique, the conventional acoustic shadow method is modified by using the secondary wave generated by nonlinear interaction of two primary sound waves emitted from parametric array. Either conventional or nonlinear acoustic shadow imaging is carried out for aluminum square cylinder and the size of the shadow is compared. The result shows that the nonlinear acoustic shadow method reduces reverberation artifact inside the square cylinder and has better accuracy in the size measurement than conventional acoustic shadow method.

This paper presents the numerical study on the nonlinear sound propagation for the parametric array using the compressible form of Navier-Stokes equations combined with the mass and energy conservation equations and the state equation. These governing equations are solved by finite difference time domain (FDTD) based method. The numerical result is shown for the parametric sound propagation in the near field of the sound source in cylindrical coordinate. The result indicates the generation of low-frequency unsteady beat by the interaction of two frequency sound waves in the near field, which grows to a difference frequency sound in the far field.

Angiosarcoma of the scalp diagnosed by the presence of neck inflammation:a case report

Angiosarcoma is an uncommon malignancy, which spread out from the endothelial cells of vessels. Scalp angiosarcoma with cervical lymph node metastasis is particularly rare. This article describes a rare case of angiosarcoma of the scalp, presenting as neck inflammation. Imaging procedures such as computed tomography （CT）, magnetic resonance image （MRI） and ultrasonography （US） were not sufficient to diagnose this case. A needle biopsy provided an effective and accurate diagnosis of cervical lymph node metastasis. Additional observation and physical examination was required to diagnose the origin of the primary cancerous lesion. Once the angiosarcoma diagnosis was confirmed histologically, sequential weekly and monthly docetaxel （DTX） treatment was effective in preventing reoccurrence. Nonetheless, the optimization of angiosarcoma treatment remains a future goal. Although patients generally describe pain and swelling at the primary lesion site, this patient complained only of painful neck inflammation, without any indication of pain or swelling of the scalp. A revised diagnostic protocol should note that cervical lymph node metastasis of unknown primary origin may result from angiosarcoma of the scalp.

Sound Source Measurement of a Semi-Circular Cylinder in a Uniform Flow by Particle Image Velocimetry

In this paper, the measurement of an aerodynamic sound source for a semi-circular cylinder in a uniform flow is described using Particle Image Velocimetry (PIV). This experimental technique is based on vortex sound theory, where the time derivative of vorticity is evaluated with the aid of two sets of standard PIV systems. The experimental results indicate that the sound source for the semi-circular cylinder is located around the shear layer near the edge of the semi-circular cylinder. The sound source intensity and the area are reduced in the semi-circular cylinder compared with those of a circular cylinder. This result indicates that the aerodynamic sound of the semi- circular cylinder is smaller than that of the circular cylinder, which supports the microphone measurement result.
