老年听力水平与认知功能的关联研究

目的探究老年人听力水平与总体认知和各分认知域功能的关联。方法 2020至2021年从记忆障碍门诊和社区招募≥50岁的受试者进行听力测试和认知功能评估。采用纯音听力测试评估受试者听力水平；采用华山版成套神经心理测验评估受试者的认知功能；神经心理测试的粗分经标准转化为各认知领域z分。单因素和多因素(校正年龄、性别、受教育年限和ApoEε4等位基因)线性回归模型用于分析纯音听阈均值(PTA)和各认知域评分之间的关系。结果 279例受试者中，认知正常者82例(29.4%)，轻度认知损害者124例(44.4%)，痴呆者73例(26.2%)。纯音听力测试结果：正常听力60例(21.5%)，听力轻度下降139例(49.8%)，听力中重度下降80例(28.7%)。经校正年龄、性别、受教育年限和ApoEε4后发现，PTA和总体认知功能(β=-0.021,P<0.001)、执行功能(β=-0.019,P=0.013)、视空间功能(β=-0.018,P=0.039)、语言(β=-0.018,P=0.033)和注意力(β=-0.020,P=0.003)呈显著负相关，但与记忆(β=-0.010,P=0.301)无相关性。结论老年听力下降与总体认知功能、执行功能、视空间功能、语言和注意功能受损显著相关。

The accurate prediction of fertility outcomes is an extremely interesting and challenging task in reproductive medicine. Efforts in this area focus on classic statistical models and newer technologies, including machine learning. The modeling process has three steps, namely, data preparation, model selection and data fitting, and model validation. Here, we present a review of studies on these methods of fertility prediction. Various databases were searched using relevant keywords. Original studies with full-text available on this topic were included for review. Earlier studies explored prediction models for spontaneous pregnancy prognosis, reproductive outcomes after intrauterine insemination and in vitro fertilization, and implantation potential based on embryo morphology and morphokinetic data. Future directions for predictive modeling in reproductive medicine include solving problems presented by big data, identifying novel informative features, balancing predictive power and result interpretability, and validating models with gold-standard study designs.

We derive the analytic expression of the concurrence in the quantum Heisenberg XYZ model and discuss the influence of parameters J, Δ and Γ on the concurrence. By choosing different values of Γ and Δ, we obtain the XX, XY, XXX and XXZ chains. The concurrence decreases with increasing temperature. When T→0, the concurrence reaches its maximum value 1, *** entangled state, |Ψ = (((2)1/2)/2)(|01-|10), is maximum entanglement. For the XXZ chain, when Γ→∞, the concurrence will meet its maximum value max = (sinh (1/T))/(cosh (1/T)).

In anomaly detection, a challenge is how to model a user＇s dynamic behavior. Many previous works represent the user behavior based on fixed-length models. To overcome their shortcoming, we propose a novel method based on discrete-time Markov chains （DTMC） with states of variable-length sequences. The method firstly generates multiple shell command streams of different lengths and combines them into the library of general sequences. Then the states are defined according to variable-length behavioral patterns of a valid user, which improves the precision and adaptability of user profiling. Subsequently the transition probability matrix is created. In order to reduce computational complexity, the classification values are determined only by the transition probabilities, then smoothed with sliding windows, and finally used to discriminate between normal and abnormal behavior. Two empirical evaluations on datasets from Purdue University and AT＆T Shannon Lab show that the proposed method can achieve higher detection accuracy and require less memory than the other traditional methods.

Biomechanical effects of pedicle screw adjustments on the thoracolumbar burst fractures

Background Posterior pedicle screw device is widely used in treatment of thoracolumbar burst fractures. As the clinical operation is not based upon quantitative data of adjustments, the results are not optimal. At present, no study has assessed the associations between the device adjustments and the restoration of stiffness. We investigated the biomechanical effects that adjustments of a pedicle screw device had on the burst fracture, and explored an optimal adjustment. Methods Burst fractures were produced at L1 vertebra in 24 fresh calf spines （T12-L3）. The specimens were divided into four groups at random. Pedicle screw devices were attached to T13 and L2. Four device adjustments, consisting of distraction and extension, were applied. Adjustment 1 was pure 6~ extension, adjustment 2 was pure 5 mm distraction, adjustment 3 was 6~ extension followed by 5 mm distraction, and adjustment 4 was 5 mm distraction followed by 6~ extension. The effect of each adjustment on the stiffness restoration, anatomical reduction, and neural decompression for the burst fractures was analyzed and evaluated. Results Pure extension （Group 1） produced the closest segment height and the least restoration of the canal to the intact. Pure distraction （Group 2） restored stiffness most, but with only 60% stiffness of the intact value, and lost the segmental angle most to the intact. The combination of extension-distraction （Group 3 and Group 4） produced the maximum reduction of the anatomy and restoration of the canal in the burst fracture, and the least stiffness restoration. The sequence of extension and distraction did not affect stiffness restoration, anatomical reduction, and neural decompression. Conclusions The device adjustments affected stiffness restoration, anatomical reduction, and neural decompression.
