Impact of next-generation sequencing on antimicrobial treatment in immunocompromised adults with suspected infections

BACKGROUND:Prompt pathogen identification can have a substantial impact on the optimization of antimicrobial *** objective of the study was to assess the diagnostic value of next-generation sequencing(NGS)for identifying pathogen and its clinical impact on antimicrobial intervention in immunocompromised patients with suspected ***:This was a retrospective *** January and August 2020,47 adult immunocompromised patients underwent NGS testing under the following clinical conditions:1)prolonged fever and negative conventional cultures;2)new-onset fever despite empiric antimicrobial treatment;and 3)afebrile with suspected infections on *** data,including conventional microbial test results and antimicrobial treatment before and after NGS,were *** were analyzed according to documented changes in antimicrobial treatment(escalated,no change,or deescalated)after the NGS ***:The median time from hospitalization to NGS sampling was 19 *** relevant pathogens were detected via NGS in 61.7% of patients(29/47),more than half of whom suffered from fungemia(n=17),resulting in an antimicrobial escalation in 53.2% of patients(25/47)and antimicrobial de-escalation in 0.2% of patients(1/47).Antimicrobial changes were mostly due to the identification of fastidious organisms such as Legionella,Pneumocystis jirovecii,and *** the remaining three cases,NGS detected clinically relevant pathogens also detected by conventional cultures a few days *** antimicrobial treatment was subsequently adjusted according to the susceptibility test ***,NGS changed antimicrobial management in 55.3%(26/47)of patients,and conventional culture detected clinically relevant pathogens in 14.9% of the patients(7/47).CONCLUSION:With its rapid identification and high sensitivity,NGS could be a promising tool for identifying relevant pathogens and enabling rapid appropriate treatment in immunocompromised patients w

Orbital angular momentum-mediated machine learning for high-accuracy mode-feature encoding

Machine learning with optical neural networks has featured unique advantages of the information processing including high speed,ultrawide bandwidths and low energy consumption because the optical dimensions(time,space,wavelength,and polarization)could be utilized to increase the degree of ***,due to the lack of the capability to extract the information features in the orbital angular momentum(OAM)domain,the theoretically unlimited OAM states have never been exploited to represent the signal of the input/output nodes in the neural network ***,we demonstrate OAM-mediated machine learning with an all-optical convolutional neural network(CNN)based on Laguerre-Gaussian(LG)beam modes with diverse diffraction *** proposed CNN architecture is composed of a trainable OAM mode-dispersion impulse as a convolutional kernel for feature extraction,and deep-learning diffractive layers as a *** resultant OAM mode-dispersion selectivity can be applied in information mode-feature encoding,leading to an accuracy as high as 97.2%for MNIST database through detecting the energy weighting coefficients of the encoded OAM modes,as well as a resistance to eavesdropping in point-to-point free-space ***,through extending the target encoded modes into multiplexed OAM states,we realize all-optical dimension reduction for anomaly detection with an accuracy of 85%.Our work provides a deep insight to the mechanism of machine learning with spatial modes basis,which can be further utilized to improve the performances of various machine-vision tasks by constructing the unsupervised learning-based auto-encoder.

Graphene as adsorbent for highly efficient extraction of modified nucleosides in urine prior to liquid chromatography-tandem mass spectrometry analysis

RNA modifications have been involved in numerous biological processes, and aberrations of these modifications are tightly associated with various diseases including cancer. Herein, we developed graphenebased solid-phase extraction and robust ultra performance liquid chromatography-tandem mass spectrometry(UPLC-MS/MS) combined with stable isotope-dilution for simultaneous enrichment and accurate determination of 17 modified nucleosides in human urine. We found graphene could effectively adsorb various modified nucleosides in human urine samples. With this method, we identified and quantified these modified nucleosides in urine samples collected from lung cancer patients and healthy *** revealed that the levels of 12 modified nucleosides were all diminished in urine from lung cancer patients, compared with healthy controls. It is worth noting that we demonstrated, for the first time, the presence of 5,2-O-dimethyluridine(m~5U_m) in human urine. Together, we established a robust analytical method for simultaneous determinations of 17 modified nucleosides in human urine, and our results revealed a close correlation between the concentrations of urinary modified nucleosides and the occurrence of lung cancer, implying the potential applications of these modified nucleosides as noninvasive biomarkers for the early detection of lung cancer. Moreover, this study will stimulate future investigations on the regulatory roles of RNA modifications in the initiation and progression of lung cancer.

Simultaneous multiplex genome loci editing of Halomonas bluephagenesis using an engineered CRISPR-guided base editor

Halomonas bluephagenesis TD serves as an exceptional chassis for next generation industrial biotechnology to produce various ***,the simultaneous editing of multiple loci in *** TD remains a significant ***,we report the development of a multiple loci genome editing system,named CRISPR-deaminase-assisted base editor(CRISPR-BE)in *** *** system comprises two components:a cytidine(CRISPR-cBE)and an adenosine(CRISPR-aBE)deaminase-based base ***-cBE can introduce a cytidine to thymidine mutation with an efficiency of up to 100%within a 7-nt editing window in *** ***,CRISPR-aBE demonstrates an efficiency of up to 100%in converting adenosine to guanosine mutation within a 7-nt editing ***-cBE has been further validated and successfully employed for simultaneous multiplexed editing in *** *** findings reveal that CRISPR-cBE efficiently inactivated all six copies of the IS1086 gene simultaneously by introducing stop *** system achieved an editing efficiency of 100%and 41.67%in inactivating two genes and three genes,*** substituting the Pcas promoter with the inducible promoter PMmp1,we optimized CRISPR-cBE system and ultimately achieved 100%editing efficiency in inactivating three *** conclusion,our research offers a robust and efficient method for concurrently modifying multiple loci in *** TD,opening up vast possibilities for industrial applications in the future.

Single-source-precursor synthesis and air-plasma ablation behavior of(Ti,Zr,Hf)C/SiC ceramic nanocomposites at 2200℃

Dense monolithic(Ti,Zr,Hf)C/SiC ceramic nanocomposites with four different molar ratios of metallic elements in the(Ti,Zr,Hf)C phase(i.e.,Ti:Zr:Hf=1:1:1,2:3:5,2:3:3,and 1:2:1)were prepared upon pyrolysis of novel(Ti,Zr,Hf)-containing single-source precursors(SSPs),followed by spark plasma sintering(SPS).A thorough characterization was conducted to elucidate the synthesis of the SSPs,polymer-to-ceramic transformation,chemical/phase compositions,and microstructure of the SiTiZrHfC-based *** results revealed the feasibility of synthesizing nanocomposites with high(Ti,Zr,Hf)C contents using the SSP *** nanocomposites were characterized by a unique microstructure with in situ generated(Ti,Zr,Hf)C@C core-shell nanoparticles homogeneously mixed withβ-*** ablation behavior of the nanocomposites was evaluated on an air-plasma device for 60 ***,the nanocomposites exhibited excellent ablation resistance,and the lowest linear ablation rate reached−0.58μm/s at 2200°***,the ablation resistance can be dramatically improved by precisely tailoring the atomic ratios of metal elements within the(Ti,Zr,Hf)C phase via the molecular design of the *** formation of a multiple-oxide layer with both a high-meltingpoint phase((Ti,Zr,Hf)O_(2))and low-melting-point phases((Zr,Hf)TiO_(4))and glassy SiO_(2),as well as their structure,played a critical role in the enhanced ablation *** uniform distribution of the high-melting-point(Ti,Zr,Hf)O_(2)nano/microparticles throughout the glassy SiO_(2)matrix significantly enhanced the viscosity and stability of the oxide layer by the pinning effect,offering superior protection against the ingress of oxygen atoms and excellent resistance to mechanical erosion.

Sequential pyrolysis for understanding specific influence of cellulose- and lignin-derived volatiles on properties of counterpart char

Interactions of cellulose-and lignin-derived intermediates have been well documented during pyrolysis of lignocellulosic *** reaction network for the interactions is rather complex,as cellulose-derived volatiles could interact/react with not only lignin-derived volatiles but also lignin-derived char and vice *** probe specifically the impacts of cellulose-derived volatiles on the lignin-derived char or the opposite,herein the sequential pyrolysis was performed by arranging cellulose in the upper bed with lignin in the lower bed or lignin above with cellulose below at 350 and 650℃,*** results indicated that the sequential pyrolysis of cellulose→lignin or lignin→cellulose at 350℃induced increased char yield from formation of carbonaceous deposits via volatiles-char *** with the lignin-derived volatiles,the cellulose-derived volatiles,especially aldehyde fractions,were more reactive towards the lignin-derived char at 350℃,forming oxygen-rich lignin-derived char with a higher yield,an abundance of aliphatic structures and consequently lower thermal *** sequential pyrolysis of lignin→cellulose,more aromatics-rich species were deposited on cellulose-derived char,but the lignin-derived volatiles were less reactive for enhancing the char *** 650℃,instead of polymerisation of the volatiles on the char,either the cellulose-or lignin-derived char catalyzed the cracking of the counterpart volatiles to remove the aliphatic functionalities,which made the char more aromatic and thermally more stable.

Non-perturbative dynamics of flat-band systems with correlated disorder

We develop a numerical method for the time evolution of Gaussian wave packets on flat-band lattices in the presence of correlated *** achieve this,we introduce a method to generate random on-site energies with prescribed *** verify this method with a one-dimensional(1D)cross-stitch model,and find good agreement with analytical results obtained from the disorder-dressed evolution *** allows us to reproduce previous findings,that disorder can mobilize 1D flat-band states which would otherwise remain *** explained by the corresponding disorder-dressed evolution equations,such mobilization requires an asymmetric disorder-induced coupling to dispersive bands,a condition that is generically not fulfilled when the flat-band is resonant with the dispersive bands at a Dirac point-like *** exemplify this with the 1D Lieb *** analytical expressions are not available for the two-dimensional(2D)system due to its complexity,we extend the numerical method to the 2D a–T3 model,and find that the initial flat-band wave packet preserves its localization when a=0,regardless of disorder and ***,when a̸=0,the wave packet shifts in real *** interpret this as a Berry phase controlled,disorder-induced wave-packet *** addition,we present density functional theory calculations of candidate materials,specifically Hg1−*** flat-band emerges near the G point(α=0)in the Brillouin zone.

Effects of Ta content on microstructure and oxidation behavior of AlCoCrFeNiYTa_(x)high entropy alloy

The oxidation behavior of the Al CoCrFeNiYTa_(x)high entropy alloy(HEA)doped with different Ta contents at 1100℃is *** this study,different reasons for the spallation of the oxide scales of HEAs without/with excessive Ta addition after 1000 h of oxidation are discussed,and the most suitable Ta doping content for this HEA is explored,in which the HEA exhibits both an exceptionally low oxidation rate,and the formation of oxide scales composed of intact a-Al_(2)O_(3).Meanwhile,it is found that the addition of Ta can effectively inhibit the phase transition of β phase,which makes the surface of the oxide scale *** combining our experimental results with Wagner–Hauffe theory,the influence of Ta drag effect on oxide scale growth is explained,which provides some theoretical guidance for the subsequent design of AlCoCrFeNi HEAs,and can lead to the development of thermal barrier coatings at higher service temperature.
