BACKGROUND Budd-Chiari syndrome(BCS)is a rare heterogeneous liver disease characterized by obstruction of the hepatic venous outflow *** incidence of BCS is so low that it is difficult to detect in general practice and difficult to include within the scope of routine *** clinical manifestations of BCS are not specific;hence,BCS tends to be *** SUMMARY We report the case of a 33-year-old Chinese woman who presented with progressive distension in the upper *** was initially misdiagnosed with liver cirrhosis(LC)due to abnormalities on an upper abdominal computed tomography *** she was taking standard anti-cirrhosis therapy,her symptoms did not *** resonance imaging showed caudate lobe hypertrophy;and dilated lumbar and hemiazygos *** revealed membranous obstruction of the inferior vena cava owing to congenital vascular malformation.A definitive diagnosis of BCS was *** angioplasty was performed to recanalize the obstructed inferior vena cava and the patient’s symptoms were completely *** lacks specific clinical features and can eventually lead to LC. Clinicians andradiologists must carefully differentiate BCS from LC. Correct diagnosis andtimely treatment are vital to the patient's health.

Combined analysis of CRMP4 methylation levels and CAPRA-S score predicts metastasis and outcomes in prostate cancer patients

The present study analyzed the predictive value of combined analysis of collapsin response mediator protein 4 （CRMP4） methylation levels and the Cancer of the Prostate Risk Assessment （CAPRA-S） Postsurgical score of patients who required adjuvant hormone therapy （AHT） after radical prostatectomy （RP）, We retrospectively analyzed 305 patients with prostate cancer （PCa） who received RP and subsequent androgen deprivation therapy （ADT）. Two hundred and thirty patients with clinically high-risk PCa underwent immediate ADT, and 75 patients with intermediate risk PCa underwent deferred ADT. CRMP4 methylation levels in biopsies were determined, and CAPRA-S scores were calculated. In the deferred ADT group, the values of the hazard ratios for tumor progression and cancer-specific mortality （CSM） in patients with 〉15% CRMP4 methylation were 6.81 （95% CI, 2.34-19.80） and 12.83 （95% CI. 2.16-26.10）, respectively. Receiver-operating characteristic curve analysis indicated that CRMP4 methylation levels 〉15% served as a significant prognostic marker of tumor progression and CSM. In the immediate ADT group, CAPRA-S scores ：〉6 and CRMP4 methylation levels ：〉15% were independent predictors of these outcomes （uni- and multi-variable Cox regression analyses）. The differences in the 5-year progression-free survival between each combination were statistically significant. Combining CAPRA-S score and CRMP4 methylation levels improved the area under the curve compared with the CRMP4 or CAPRA-S model. Therefore, CRMP4 methylation levels ：〉15% were significantly associated with a poor prognosis and their combination with CAPRA-S score accurately predicted tumor progression and metastasis for patients requiring AHT after RP.

Clinical significance of telomerase activity in peritoneal lavage fluid from patients with gastric cancer and its relationship with cellular proliferation

AIM： To evaluate the efficacy of telomerase activity assay and peritoneal lavage cytology （PLC） examination in peritoneal lavage fluid for the prediction of peritoneal metastasis in gastric cancer patients, and to explore the relationship between telomerase activity and proliferating cell nuclear antigen expression.METHODS： Telomeric repeated amplification protocol （TRAP）-enzyme-linked immunosorbent assay （ELISA） was performed to measure the telomerase activity in 60 patients with gastric cancer and 50 with peptic ulcer. PLC analysis of the 60 patients with gastric cancer was used for comparison. The proliferating cell nuclear antigen （PCNA） in gastric carcinoma was immunohistochemically examined.RESULTS： The telomerase activity and PLC positive rate in peritoneal lavage fluid from patients with gastric cancer was 41.7% （25/60）, and 25.0% （15/60）, respectively. The positive rate of telomerase activity was significantly higher than that Qf PLC in the group of pT, （15/16 vs 9/16, P 〈 0.05）, P1-3 （13/13 vs 9/13, P 〈 0.05） and diffuse type （22/42 vs 13/42, P 〈 0.05）. The patients with positive telomerase activity, peritoneal metastasis, and serosal invasion had significantly higher levels of average PCNA proliferation index （PI）, （55.00 ± 6.59 vs 27.43 ± 7.72, 57.26 ±10.18 vs 29.15 ±8.31, and 49.82 ± 6.74 vs 24.65 ±7.33, respectively, P 〈 0.05）.CONCLUSION： The TRAP assay for telomerase activity is a useful adjunct for cytologic method in the diagnosis of peritoneal micrometastasis and well related to higher proliferating activity of gastric cancer. The results of this study also suggest a promising future therapeutic strategy for treating peritoneal dissemination based on telomerase inhibition.

Although a variety of nanoparticles with better-than-bulk material performances can be synthesized, it remains a challenge to scale the extraordinary properties of individual nanoscale units to the macroscopic level for bulk nanostructured materials. Here, we report a general and scalable biosynthesis strategy that involves simultaneous growth of cellulose nanofibrils through microbial fermentation and co-deposition of various kinds of nanoscale building blocks(NBBs) through aerosol feeding on solid culture substrates. We employ this biosynthesis strategy to assemble a wide range of NBBs into cellulose nanofibril-based bulk nanocomposites. In particular, the biosynthesized carbon nanotubes/bacterial cellulose nanocomposites that consist of integrated 3D cellulose nanofibril networks simultaneously achieve an extremely high mechanical strength and electrical conductivity, and thus exhibit outstanding performance as high-strength lightweight electromagnetic interference shielding materials. The biosynthesis approach represents a general and efficient strategy for large-scale production of functional bulk nanocomposites with enhanced performances for practical applications. Industrial-scale production of these bulk nanocomposite materials for practical applications can be expected in the near future.

Structure and magnetic properties of melt-spun Sm-Fe-Nb ribbons and their nitrides

SmFe（10-x）Nbx（x = 0, 0.1, 0.2, 0.3） ribbons and their nitrides were prepared by melt-spinning, followed by annealing and subsequent nitriding. The structure and magnetic properties were investigated by means of x-ray diffraction（xRD） using the Rietveld method, vibrating sample magnetometer（VSM）, transmission electron microscope（TEM） and Mossbauer spectroscopy. xRD analysis shows that the addition of Nb can impede the precipitation of Sm2 Fe（17） and a-Fe phases and the Nb atoms occupy 2 e site in the alloys. At 300 K, the mean hyperfine fields of 2 e site are 29.58 T with Nb doping at x = 0.1 and the corresponding Curie temperature is 552 K. The optimal properties of remanence of Br = 0.9 T, intrinsic coercivity of H（cj）= 741.5 kA·m^-1, and maximum magnetic energy product of（BH）（max）= 124.2 kJ·m-（-3） are gained at x = 0.10 in the nitrides.

Assessment of cerebrovascular reserve impairment using the breath-holding index in patients with leukoaraiosis

Many studies have demonstrated that leukoaraiosis is associated with impaired cerebrovascular reserve function. However, the definitive hemodynamic changes that occur in leukoaraiosis are not clear, and there are many controversies. This study aimed to investigate hemodynamic changes in symptomatic leukoaraiosis using transcranial Doppler ultrasonography and the breath-holding test in a Chinese Han population, from northern China. A total of 203 patients who were diagnosed with ischemic stroke or clinical chronic progressive ischemic symptoms were enrolled in this study, including 97 males and 106 females, with an age range of 43-93 years. The severity of leukoaraiosis was evaluated according to the Fazekas grading scale, and patients were divided into four groups accordingly. Grade 0 was no leukoaraiosis, and grades I, II, and III were mild, moderate, and severe leukoaraiosis, respectively, with 44, 79, 44, and 36 cases in each group. Transcranial Doppler ultrasonography and the breath-holding test were performed. The mean blood flow velocity of the bilateral middle cerebral artery was measured and the breath-holding index was calculated. The breath holding index was correlated with leukoaraiosis severity and cognitive impairment. Patients with a low breath holding index presented poor performance in the Montreal Cognitive Assessment (MoCA) and executive function tests. That is, the lower the breath holding index, the lower the scores for the MoCA and the higher for the trail-making test Parts A and B. These results indicate that the breath-holding index is a useful parameter for the evaluation of cerebrovascular reserve impairment in patients with leukoaraiosis. In addition, the breath-holding index can reflect cognitive dysfunction, providing a new insight into the pathophysiology of leukoaraiosis.
