We report a case of intraductal papillary mucinous neoplasm(IPMN) originating from the ileal heterotopic pancreas in a patient with hereditary non-polyposis colorectal cancer(HNPCC). A 49-year-old woman had a past history of total colectomy and total hysterectomy with bilateral salpingo-oophorectomy due to colonic adenocarcinoma and endometrial adenocarcinoma 11 years ago. Her parents died from colonic adenocarcinoma and her sister died from colonic adenocarcinoma and endometrial adenocarcinoma. The clinician found an ileal mass with necrotic change and the mass increased in size from 1.7 cm to 2.2 cm during the past 2 years on computed tomography. It was surgically resected. Microscopically, the ileal mass showed heterotopic pancreas with IPMN high grade dysplasia. Immunohistochemical staining revealed positive reactivity for MLH1/PMS2 and negative reactivity for MSH2/MSH6. This is the first report of IPMN originating from the ileal heterotopic pancreas in a patient with HNPCC in the English literature.

With the rise of remote collaboration,the demand for advanced storage and collaboration tools has rapidly ***,traditional collaboration tools primarily rely on access control,leaving data stored on cloud servers vulnerable due to insufficient *** paper introduces a novel mechanism that encrypts data in‘bundle’units,designed to meet the dual requirements of efficiency and security for frequently updated collaborative *** bundle includes updated information,allowing only the updated portions to be reencrypted when changes *** encryption method proposed in this paper addresses the inefficiencies of traditional encryption modes,such as Cipher Block Chaining(CBC)and Counter(CTR),which require decrypting and re-encrypting the entire dataset whenever updates *** proposed method leverages update-specific information embedded within data bundles and metadata that maps the relationship between these bundles and the plaintext *** utilizing this information,the method accurately identifies the modified portions and applies algorithms to selectively re-encrypt only those *** approach significantly enhances the efficiency of data updates while maintaining high performance,particularly in large-scale data *** validate this approach,we conducted experiments measuring execution time as both the size of the modified data and the total dataset size *** show that the proposed method significantly outperforms CBC and CTR modes in execution speed,with greater performance gains as data size ***,our security evaluation confirms that this method provides robust protection against both passive and active attacks.

LED lighting has problems such as short life span, power reduction due to the LED chip's heat, and decreased luminance efficiency etc. Many studies on the optimization of the thermal performance of LEDs have been conducted actively to solve these problems. The present study changes the fin shape and intervals of various heat sink designs so that LED can effectively produce a thermal effect. The water-cooling heat release system and the heat pipe methods show higher thermal performance results than the existing systems; however, the system is of high cost and requires a large-scale apparatus. This study carried out a numerical analysis of the thermal control system by applying heat flux and heat sink properties as an active heat release system using the commercial code ANSYS ver. 13.0 and verified the validity of the new heat sink design for temperature distribution analysis.

Mg-Al-Zn-M M （misch metal） alloy powders were manufactured by inert gas atomization and the characteristics of alloy powders were *** spite of the low fluidity and easy oxidation of the magnesium melt,the spherical powder was made successfully with the improved three piece nozzle systems of gas atomization unit. It was found that most of the solidified powders with particles size of less than 50μm in diameter were single crystal and the solidification structure of rapidly solidified powders showed a typical dendritic morphology because of supercooling prior to *** spacing of secondary denrite arms was deceasing as the size of powders was *** rapidly solidified powders were consolidated by vacuum hot extrusion and the effects of misch metal addition to AZ91 on mechanical properties of extruded bars were also *** extrusion of the rapidly solidified powders,their dendritic structure was broken into fragments and remained as grains of about 3μm in *** Mg-Al-Ce intermetallic compounds formed in the interdendritic regions of powders were finely broken,*** tensile strength and ductility obtained in as-extruded Mg-9 wt pct Al-1 wt pct Zn-3 wt pct MM alloy wereσ-（T.S.） =383 MPa andε=10.6%,*** of these improvements on mechanical properties were resulted from the refined microstructure and second-phase dispersions.

In this study, numerical analysis is performed to adopt the equivalence ratio on the high velocity oxygen fuel （HVOF） thermal spray coating systems equipped with a minimum length nozzle. The analysis is applied to investigate the axisymmetric, steady-state, turbulent, and chemically combusting flow both within the torch and in a free jet region between the torch and the substrate to be coated. The combustion is modeled using a single-step and eddy-dissipation model which assumes that the reaction rate is limited by the turbulent mixing rate of the fuel and oxidant. As the diameter of the nozzle throat is increased, the location of the Mach shock disc moves backward from the nozzle exit. As the throat diameter and the divergent portion are 6 mm and 8 mm, respectively, the pressure in the HVOF system is the lowest at the chamber and the expanding gas is steadily maintained with both high velocity and high temperature for different equivalence ratios. Thus, relatively minor amendments of the equivalence ratio and the geometry of HVOF can lead to improved control over coating characteristics.

Gastric neuroendocrine tumors are rare; however,the incidence has recently increased due to the increasing use of upper endoscopy. Neuroendocrine tumors arise from the excess proliferation of enterochromaffinlike(ECL) cells. The proliferative changes of enterochromaffin cells evolve through a hyperplasiadysplasia-neoplasia sequence that is believed to underlie the pathogenesis of gastric neuroendocrine tumors. Endoscopic resection is recommended as the initial treatment if the tumor is not in an advanced stage. However,there is no definite guideline for the treatment of recurrent gastric neuroendocrine tumors following endoscopic resection. Here,we report a rare case of gastric neuroendocrine tumors in a 56-yearold male who experienced two recurrences within 11 years after endoscopic resection. The patient finally underwent a total gastrectomy. The pathological features of the resected stomach exhibited the full hyperplasia-dysplasia-neoplasia sequence of the ECL cells in a single specimen.

AIM: To identify the associated risk factors for hyperechogenic pancreas (HP) which may be observed on endoscopic ultrasound (EUS) and to assess the relationship between HP and obesity. METHODS: From January 2007 to December 2007, we prospectively enrolled 524 consecutive adults who were scheduled to undergo EUS. Patients with a history of pancreatic disease or with hepatobiliary or advanced gastrointestinal cancer were excluded. Finally,284 patients were included in the analyses. We further analyzed the risk of HP according to the categories of visceral adipose tissue (VAT) and subcutaneous adipose tissue in 132 patients who underwent abdominal computed tomography scans. RESULTS: On univariate analysis, age older than 60 years, obesity (body mass index > 25 kg/m 2 ), fatty liver, diabetes mellitus, hypertension and hypercholesterolemia were identified as risk factors associated with HP (P < 0.05). On multivariate analysis, fatty liver [P = 0.008, odds ratio (OR) = 2.219], male gender (P = 0.013, OR = 2.636), age older than 60 years (P = 0.001, OR = 2.874) and hypertension (P = 0.044, OR = 2.037) were significantly associated with HP. In the subgroup analysis, VAT was a statistically significant risk factor for HP (P = 0.010, OR = 5.665, lowest quartile vs highest quartile). CONCLUSION: HP observed on EUS was associated with fatty liver, male gender, age older than 60 years, hypertension and VAT.

Pipeline processing is applied to mutiple flow tables(MFT)in the switch of software-defined network(SDN)to increase the throughput of the ***,the processing time of each flow increases as the size or number of flow tables gets *** this paper we propose a novel approach called PopFlow where a table keeping popular flow entries is located up front in the pipeline,and an express path is provided for the flow matching the table.A Markov model is employed for the selection of popular entries considering the match latency and match frequency,and Queuing theory is used to model the flow processing time of the existing MFT-based schemes and the proposed *** simulation reveals that the proposed scheme substantially reduces the flow processing time compared to the existing schemes,and the diference gets more significant as the flow arrival rate increases.
