Clinical outcomes of self-expandable metal stents in palliation of malignant anastomotic strictures caused by recurrent gastric cancer

AIM： To examine the technical feasibility and clinical outcomes of the endoscopic insertion of a selfexpandable metal stent （SEMS） for the palliation of a malignant anastomotic stricture caused by recurrent gastric cancer. METHODS： The medical records of patients, who had obstructive symptoms caused by a malignant anastomotic stricture after gastric surgery and underwent endoscopic insertion of a SEMS from January 2001 to December 2007 at Kangnam St Mary＇s Hospital, were reviewed retrospectively. RESULTS： Twenty patients （15 male, mean age 63 years） were included. The operations were a total gastrectomy with esophagojejunostomy （n = 12）, subtotal gastrectomy with Billroth-Ⅰ reconstruction （n = 2） and subtotal gastrectomy with Billroth- Ⅱ reconstruction （n = 8）. The technical and clinical success rates were 100% and 70%, respectively. A small bowel or colon stricture was the reason for a lack of improvement in symptoms in 4 patients. Two of these patients showed improvement in symptoms after another stent was placed. Stent reobstruction caused by tumor ingrowth or overgrowth occurred in 3 patients （15%） within 1 mo after stenting. Stent migration occurred with a covered stent in 3 patients who underwent a subtotal gastrectomy with Billroth-Ⅱ reconstruction. Two cases of partial stent migration were easily treated with a second stent or stent repositioning. The median stent patency was 56 d （range, 5-439 d）. The median survival was 83 d （range, 12-439 d）. CONCLUSION： Endoscopic insertion of a SEMS provides safe and effective palliation of a recurrent anastomotic stricture caused by gastric cancer, A meticulous evaluation of the presence of other strictures before inserting the stent is essential for symptom improvement.

Spatio-temporal change in forest cover and carbon storage considering actual and potential forest cover in South Korea

This study analyzes change in carbon storage by applying forest growth models and final cutting age to actual and potential forest cover for six major tree species in South Korea. Using National Forest Inventory data, the growth models were developed to estimate mean diameter at breast height, tree height, and number of trees for Pinus densiflora, Pinus koraiensis, Pinus rigida, Larix kaernpferi, Castanea crenata and Quercus spp. stands. We assumed that actual forest cover in a forest type map will change into potential forest covers according to the Hydrological and Thermal Analogy Groups model. When actual forest cover reaches the final cutting age, forest volume and carbon storage are estimated by changed forest cover and its growth model. Forest volume between 2010 and 2110 would increase from 126.73 to 157.33 m^3 hm^-2. Our results also show that forest cover, volume, and carbon storage could abruptly change by 2060. This is attributed to the fact that most forests are presumed to reach final cutting age. To avoid such dramatic change, a regeneration and yield control scheme should be prepared and implemented in a way that ensures balance in forest practice and yield.

Streptococcus mutans activates the AIM2, NLRP3 and NLRC4 inflammasomes in human THP-1 macrophages

Streptococcus mutans(S. mutans), a major aetiologic agent of dental caries, is involved in systemic diseases, such as bacterial endocarditis, if it enters the bloodstream through temporary bacteraemia. Interleukin(IL)-1β, a proinflammatory cytokine, is related to the host defences against pathogens, and its synthesis, maturation, and secretion are tightly regulated by the activation of the inflammasome, an inflammatory signalling complex. This study examined the signalling mechanism of IL-1β secretion and the inflammasome pathway induced by S. mutans to explain the molecular mechanism through which systemic infection by oral streptococci can occur. After infection of THP-1 cells with S. mutans, the expression of inflammasome components was detected using various methods. S. mutans induced IL-1β secretion via caspase-1 activation, and S. mutans-induced IL-1β secretion required absent in melanoma(AIM2), NLR family pyrin domain-containing 3(NLRP3) and NLR family CARD domain-containing 4(NLRC4)inflammasome activation. In particular, the S. mutans-induced NLRP3 inflammasome was mediated by adenosine triphosphate(ATP) release, potassium depletion and lysosomal damage. Our study provides novel insight into the innate immune response against S. mutans infection.

Risk factors for rebleeding after angiographically negative acute gastrointestinal bleeding

AIM:To identify possible predictive factors for rebleeding after angiographically negative findings in patients with acute non-variceal gastrointestinal ***: From January 2000 to July 2007, 128 patients with acute non-variceal gastrointestinal bleeding had negative f indings after initial angiography. Clinical and laboratory parameters were analyzed ***: Among 128 patients, 62 had no recurrent gastrointestinal bleeding and 66 had recurrent gastrointestinal bleeding within 30 d. As determined by the use of multivariate analysis, an underlying malignancy, liver cirrhosis and hematemesis were significant factors related to recurrent gastrointestinal ***: Clinical factors including underlying malignancy, liver cirrhosis, and hematemesis are important predictors for rebleeding after angiographically negative findings in patients with acute non-variceal gastrointestinal bleeding.

Efficient controller area network data compression for automobile apphcations

Controller area networks （CANs） have been designed for multiplexing communication between electronic control units （ECUs） in vehicles and many high-level industrial control applications. When a CAN bus is overloaded by a large number of ECUs connected to it, both the waiting time and the error probability of the data transmission are increased. Thus, it is desirable to reduce the CAN frame length, since the duration of data transmission is proportional to the frame length. In this paper, we present a CAN message compression method to reduce the CAN frame length. Experimental results indicate that CAN transmission data can be compressed by up to 81.06% with the proposed method. By using an embedded test board, we show that 64-bit engine management system （EMS） CAN data compression can be performed within 0.16 ms; consequently, the proposed algorithm can be successfully used in automobile applications.

While spray-drying has been widely utilized to improve the bioavailability of poorly water-soluble drugs,the outcomes often exhibit suboptimal particle size distribution and large particle sizes,limiting their *** this study,we introduce electrostatic spraying as an advanced technology tailored for poorly water-soluble drugs,enabling the fabrication of nanoparticles with fine and uniform particle size ***(1 g),as a model drug,copovidone(5 g),and sodium dodecyl sulfate(0.1 g)were dissolved in 200 ml ethanol and subjected to conventional-spray-dryer and electrostatic spray *** electrostatic spray-dried nanoparticles(ESDN)showed smaller particle sizes with better uniformity compared to conventional spray-dried nanoparticles(CSDN).ESDN demonstrated significantly enhanced solubility and rapid release in *** vitro studies revealed that ESDN induced apoptosis in HCT-116 cells to a greater extent,exhibiting superior cytotoxicity compared to ***,ESDN substantially improved oral bioavailability and antitumor efficacy compared to *** findings suggest that ESD shows potential in developing enhanced drug delivery systems for poorly water-soluble drugs,effectively addressing the limitations associated with CSD methods.

AIM To determine adiponectin expression in colonic tissue of murine colitis and systemic cytokine expression after melatonin treatments and sleep *** The following five groups of C57BL/6 mice were used in this study:(1) group Ⅰ,control;(2) group Ⅱ,2% DSS induced colitis for 7 d;(3) group Ⅲ,2% DSS induced colitis and melatonin treatment;(4) group Ⅳ,2% DSS induced colitis with sleep deprivation(SD) using specially designed and modified multiple platform water baths; and(5) group V,2% DSS induced colitis with SD and melatonin treatment. Melatonin(10 mg/kg) or saline was intraperitoneally injected daily to mice for 4 d. The body weight was monitored daily. The degree of colitis was evaluated histologically after sacrificing the mice. Immunohistochemical staining and Western blot analysis was performed using anti-adiponectin antibody. After sampling by intracardiac punctures,levels of serum cytokines were measured by ELISA. RESULTS Sleep deprivation in water bath exacerbated DSS induced colitis and worsened weight loss. Melatonin injection not only alleviated the severity of mucosal injury,but also helped survival during stressful condition. The expression level of adiponectin in mucosa was decreased in colitis,with the lowest level observed in colitis combined with sleep deprivation. Melatonin injection significantly(P < 0.05) recovered the expression of adiponectin. The expression levels of IL-6 and IL-17 were increased in the serum of mice with DSS colitis but decreased after melatonin injection. CONCLUSION This study suggested that melatonin modulated adiponectin expression in colonic tissue and melatonin and adiponectin synergistically potentiated antiinflammatory effects on colitis with sleep deprivation.

Background:The rapid antibiotics treatment targeted to a specific pathogen can improve clinical outcomes of *** aimed to evaluate the clinical characteristics and outcomes of biliary septicemia caused by cholangitis or cholecystitis according to causative ***:We performed a retrospective cohort study in 151 patients diagnosed with cholangitis or cholecystitis with bacterial septicemia from January 2013 to December *** patients showed clinical evidence of biliary tract infection and had blood isolates that demonstrated ***:Gram-negative,gram-positive,and both types of bacteria caused 84.1%(127/151),13.2%(20/151),and 2.6%(4/151)episodes of septicemia,*** most common infecting organisms were Escherichia coli among gram-negative bacteria and Enterococcus species(Enterococcus casseliflavus and Enterococcus faecalis)among gram-positive *** were no differences in mortality,re-admission rate,and need for emergency decompression procedures between the gram-positive and gram-negative septicemia *** univariate analysis,previous gastrectomy history was associated with gram-positive *** analysis also showed that previous gastrectomy history was strongly associated with gram-positive septicemia(Odds ratio=5.47,95%CI:1.19–25.23;P=0.029).Conclusions:Previous gastrectomy history was related to biliary septicemia induced by gram-positive *** information would aid the choice of empirical antibiotics.

In this paper, we present a waypoint tracking scheme for collision avoidance using artificial potential field（APF）. It is shown that using APF is effective in avoiding collision for path planning but it is not reported yet using APF for waypoint tracking. Since waypoints are given in discrete and even irregularly, smoothly joining the local detour path due to an obstacle and the global path given by waypoints is necessary. Otherwise, severe yawing of the heading angle of the vehicle may happen. In this paper, to resolve the problem, we propose a new path planning method based on waypoints using APF. This proposed method was validated with the empirical kinematics vehicle model using MATLAB/Simulink and waypoints given by a leading vehicle driven by a human driver. The simulation results show that the proposed path planning algorithm based on APF is effective for the waypoint tracking scenario.
