Immunoglobulin G4-related spinal pachymeningitis:A case report

BACKGROUND Immunoglobulin G4-related disease(IgG4-RD)is a complex immune-mediated condition that causes fibrotic inflammation in several organs.A significant clinical feature of IgG4-RD is hypertrophic pachymeningitis,which manifests as inflammation of the dura mater in intracranial or spinal *** IgG4-RD can affect multiple areas,the spine is a relatively rare site compared to the more frequent involvement of intracranial *** SUMMARY A 70-year-old male presented to our hospital with a two-day history of fever,altered mental status,and generalized *** initial brain magnetic resonance imaging(MRI)revealed multiple small infarcts across various cerebral *** the second day after admission,a physical examination revealed motor weakness in both lower extremities and diminished sensation in the right lower *** evaluation revealed findings consistent with acute motor sensory *** initial management with intravenous immunoglobulin for presumed Guillain-Barrésyndrome,the patient exhibited progressive worsening of motor *** the 45th day of hospitalization,an enhanced MRI of the entire spine,focusing specifically on the thoracic 9 to lumbar 1 vertebral level,raised the suspicion of IgG4-related spinal ***,the patient was administered oral prednisolone and participated in a comprehensive rehabilitation program that included gait training and lower extremity strengthening *** IgG4-related spinal pachymeningitis,diagnosed on MRI,was treated with corticosteroids and a structured rehabilitation regimen,leading to significant improvement.

Enhancement of the Electromagnetic Wave Shielding Effectiveness by Geometry-Controlled Carbon Coils

Carbon microcoils were deposited onto Al2O3 substrates using C2H2/H2 as source gases and SF6 as an incorporated additive gas in a thermal chemical vapor deposition system. At as-grown state, the carbon coils (d-CCs) show the diverse geometry. The geometry-controlled carbon microcoils (g-CMCs) could be obtained by manipulating the injection time of SF6 in C2H2 source gas. The d-CCs with polyurethane (PU) composite (d-CC@PU) and the g-CMCs with PU composite (g-CMC@PU) were obtained by dispersing d-CCs and g-CMCs in PU, respectively. The electromagnetic wave shielding properties of d-CC@PU and g-CMC@PU composites were investigated in the frequency range of 0.25 - 4.0 GHz. The shielding effectiveness (SE) of d-CC@PU and g-CMC@PU composites were measured and discussed according to the weight percent of d-CCs and g-CMCs in the composites with the thickness of the composites layers. On the whole frequency range in this work, the SE of g-CMC@PU composites was higher than those of d-CC@PU composites, irrespective of the weight percent of carbon coils in the composites and the layer thickness. Furthermore, we confirmed that the absorption mechanism, instead of the reflection mechanism, seemed to play the critical role to shield the EMI for not only the g-CMC@PU composites but also the d-CC@PU composites.

Thyroid cartilage elevation maneuver: a novel and easy-to-perform method to insert a supraglottic airway device

To the Editor:A supraglottic airway device(SAD)can be used instead of an endotracheal tube(ET)in emergency *** are two types of SAD,cuffed and non-cuffed,the ease of use and success rates of which are *** to ET insertion,SAD insertion is quick and simple,while providing comparable *** 2019 American Heart Association guideline recommends SAD insertion in emergency situations,such as out-of-hospital cardiac arrest.[1,2]However,the success of SAD insertion is not easily *** to one study,when a SAD was used in an emergency medical service(EMS)setting,airway failure was not recognized in 13.8%of patients.[3]Incorrect positioning of a SAD is evidenced by bite block malalignment with the incisors and ineffective gas exchange(characterized by thoracic and“front of neck”excursions,inadequate tidal volume,low arterial oxygen saturation[SpO_(2)],poor capnograph trace,high airway pressure,and air leak).[1,4]Failure to insert a SAD to the proper depth typically results from the resistance of structures around the hypopharynx.[5]In this study,we introduce the thyroid cartilage elevation(TCE)maneuver,a simple and novel method that facilitates correct insertion of a SAD.

Enhancement of Tumor Regression by Coulomb Nanoradiator Effect in Proton Treatment of Iron-Oxide Nanoparticle-Loaded Orthotopic Rat Glioma Model: Implication of Novel Particle Induced Radiation Therapy

Background: Proton-impact metallic nanoparticles, inducing low-energy electrons emission and characteristic X-rays termed as Coulomb nanoradiator effect (CNR), are known to produce therapeutic enhancement in proton treatment on experimental tumors. The purpose of this pilot study was to investigate the effect of CNR-based dose enhancement on tumor growth inhibition in an iron-oxide nanoparticle (FeONP)-loaded orthotopic rat glioma model. Methods: Proton-induced CNR was exploited to treat glioma-bearing SD rat loaded with FeONP by either fully-absorbed single pristine Bragg peak (APBP) or spread-out Bragg peak (SOBP) 45-MeV proton beam. A selected number of rats were examined by MRI before and after treatment to obtain the size and position information for adjusting irradiation field. Tumor regression assay was performed by histological analysis of residual tumor in the sacrificed rats 7 days after treatment. The results of CNR-treated groups were compared with the proton alone control. Results: Intravenous injection of FeONP (300 mg/kg) elevated the tumor concentration of iron up to 37 μg of Fe/g tissue, with a tumor-to-normal ratio of 5, 24 hours after injection. The group receiving FeONP and proton beam showed 65% - 79% smaller tumor volume dose-dependently compared with the proton alone group. The rats receiving FeONP and controlled irradiation field by MR imaging demonstrated more than 95% - 99% tumor regression compared with MRI-determined initial tumor size. Conclusions: Proton-impact FeONP produced therapeutic enhancement compared with proton alone in an orthotopic rat glioma model at a selected temporal point after treatment. Single BP proton beam could induce CNR- based dose enhancement and produce enhanced tumor regression that was comparable to SOBP treatment despite inhomogeneous tumor dose in the APBP-treated tumor. These results may suggest emergence of novel Particle Induced Radiation Therapy (PIRT) on malignant glioma.

AIM： To determine if the fraction of Nardostachysjata- mansi （N J） has the potential to ameliorate the severity of acute pancreatitis （AP）. METHODS： Mice were administered the biologically active fraction of N J, i.e., the 4th fraction （N J4）, intra- peritoneally, and then injected with the stable chole- cystokinin analogue cerulein hourly for 6 h. Six hours after the last cerulein injection, the pancreas, lung, and blood were harvested for morphological examination,measurement of cytokine expression, and examination of neutrophil infiltration. RESULTS： N J4 administration attenuated the sever- ity of AP and lung injury associated with AP. It also reduced cytokine production and neutrophil infiltration and resulted in the in vivo up-regulation of heine oxy- genase-1 （HO-1）. Furthermore, NJ4 and its biologically active fraction, N J4-2 inhibited the cerulein-induced death of acinar cells by inducing HO-1 in isolated pan- creatic acinar cells. CONCLUSION： These results suggest that N J4 may be a candidate fraction offering protection in AP and N J4 might ameliorate the severity of pancreatitis by induc- ing HO-1 expression.

AIM: To investigate the performance of Gadofluorine P-enhanced magnetic resonance imaging(MRI) on the diagnosis of diabetes in a streptozotocin(STZ)-induced diabetic rat ***: Fischer 344 rats were treated with *** not treated with STZ served as controls. T1-weighted MRI was performed using a 3T scanner before and after the injection of Gd-DOTA or Gadofluorine P(6 diabetic rats, 5 controls). The normalized signal intensity(SI) and the enhancement ratio(ER) of the pancreas were measured at each time point, and the values were compared between the normal and diabetic rats using the Mann-Whitney test. In addition,the values were correlated with the mean islet *** cut-off values were calculated using a positive test based on receiver operating *** Gd concentration after the injection of each contrast media was measured using laser ablation-inductively coupled plasma-mass spectrometry in a separate set of rats(4 diabetic rats, 4 controls for Gadofluorine P; 2, 2 for Gd-DOTA).RESULTS: The normalized SI and ER of the pancreas using Gd-DOTA were not significantly different between diabetic rats and controls. With Gadofluorine P, the values were significantly higher in the diabetic rats than in the control rats 30 min after injection(P < 0.05). The area under the receiver operating characteristic curve that differentiated diabetic rats from the control group was greater for Gadofluorine P than for Gd-DOTA(0.967 vs 0.667, P = 0.085). An increase in normalized SI 30 min after Gadofluorine P was correlated with a decrease in the mean number of islets(r 2 = 0.510, P = 0.014). Intra-pancreatic Gd was higher in rats with Gadofluorine P injection than Gd-DOTA injection(Gadofluorine P vs Gd-DOTA, 7.37 vs 0.00, P < 0.01). A feasible route to produce 1.1 GPa ferritic-based low-Mn lightweight steels with ductility of 47%

High- and medium-Mn (H/M-Mn) base lightweight steels are a class of ultrastrong structural materials with high ductility compared to their low-Mn counterparts with low strength and poor ***, producing these H/M-Mn materials requires the advanced or high-tech manufacturing techniques, which can unavoidably provoke labor and cost concerns. Herein, we have developed a facilestrategy that circumvents the strength–ductility trade-off in low-Mn ferritic lightweight steels, by employing low-temperature tempering-induced partitioning (LTP). This LTP treatment affords a typical Fe-2.8Mn-5.7Al-0.3C (wt.%) steel with a heterogeneous size-distribution of metastable austenite embeddedin a ferrite matrix for partitioning more carbon into smaller austenite grains than into the larger austenite ones. This size-dependent partitioning results in slip plane spacing modification and lattice strain,which act through dislocation engineering. We ascribe the simultaneous improvement in strength andtotal elongation to both the size-dependent dislocation movement in austenite grains and the controlleddeformation-induced martensitic transformation. The low-carbon-partitioned large austenite grains increase the strength and ductility as a consequence of the combined martensitic transformation andhigh dislocation density-induced hardening and by interface strengthening. Additionally, high-carbonpartitioned small austenite grains enhance the strength and ductility by planar dislocation glide (inthe low strain regime) and by cross-slipping and delayed martensitic transformation (in the high strainregime). The concept of size-dependent dislocation engineering may provide different pathways for developing a wide range of heterogeneous-structured low-Mn lightweight steels, suggesting that LTP maybe desirable for broad industrial applications at an economic cost.

Primary large cell neuroendocrine carcinoma in the common bile duct:First Asian case report

Large cell neuroendocrine carcinoma (LCNEC) in the biliary system is a poorly differentiated, high-grade neuroendocrine tumor. These tumors exhibit aggressive behavior and an increased tendency for early nodal and distant metastases. Herein, we report an unusual case of a pure primary LCNEC of the common bile duct (CBD). A 75-year-old female presented with nausea and jaundice. The patient underwent a CBD excision with lymph node dissection. Upon histological and immunohistochemical examination, the tumor exhibited pure large cell-type neuroendocrine features. Metastases were noted in two of the eight lymph nodes. The patient was administered adjuvant chemotherapy. The patient’s cancer recurred 7 mo after surgery, and the patient died from liver failure 5 mo after recurrence. The prognosis of LCNEC of CBD remains poor despite curative resection and adjuvant chemotherapy. The role of additional therapies, such as multimodal treatment including radiation therapy, must be further studied to improve the prognoses of patients.
