m.tal-on-m.tal(m.m. hip replacem.nts have proven to be a m.dern day orthopaedic failure. The early enthusiasm.and prom.se of a hard, durable bearing was quickly quashed following the unanticipated wear rates. The release of m.tal ions into the blood stream.has been shown to lead to surrounding soft tissue com.lications and early failure. The devastating destruction caused has led to a large num.er of revision procedures and im.lant extractions. The resulting research into this field has led to a new area of interest; that of the wear at the trunnion of the prosthesis. It had been previously thought that the m.tal debris was generated solely from.the weight bearing articulation, however with the evolution of m.dularity to aid surgical options, wear at the trunnion is becom.ng m.re apparent. The phenom.non of "trunnionosis" is a rapidly developing area of interest that m.y contribute to the overall effect of m.tallosis in m. m.replacem.nts but m.y also lead to the release of m.tal ions in non m. m.hip designs. The aim.of this paper is to introduce, explain and sum.arise the evidence so far in the field of trunnionosis. The evidence for this phenom.non, the type of debris particles generated and a contrast between m. m. non m. m.and resurfacing procedures are also presented.

There is m.ch literature about differing grafts used in anterior cruciate ligam.nt(ACL) reconstruction. m.ch of this is of poor quality and of a low evidence base. We review and sum.arise the literature looking at the four m.in classes of grafts used in ACL reconstruction; bone-patella tendon-bone, ham.trings, allograft and synthetic grafts. Each graft has the evidence for its use reviewed and then com.ared, where possible, to the others. We conclude that although there is no clear "best" graft, there are clear differences between the differing graft choices. Surgeon's need to be aware of the evidence behind these differences, in order to have appropriate discussions with their patients, so as to com. to an inform.d choice of graft type to best suit each individual patient and their requirem.nts.

用FRP加固的无筋砌体墙的面外弯曲和面内剪切性能经常受剥离破坏机理的控制。因而,需要定量确定基本的界面粘结滑移模型,这个模型能够描述FRP与砖砌体粘结界面性能。给出了29个采用外包和嵌入式加固FRP的拔出试验结果,试验变量包括:表面处理方式,几何尺寸,FRP(相对于节点和核心的)位置,基础节点的砌筑方法(砂浆快速干砌),玻璃纤维布的粘结方式,FRP种类。讨论了试验结果,初步给出了实际应用的建议。通过试验数据验证预测界面开裂剥离抗力的模型。该模型在外包和嵌入式加固技术应用中具有普遍的意义。这个一般性模型对所研究的29个拔出试验的极限强度具有很高的预测精度。

The im.ortant role of atherosclerosis in pathophysiology of Alzheim.r's Disease has becom. *** such as hyperlipidem.a,inflam.ation,abdom.nal obesity and insulin resistance are im.ortant yet they m.y not fully explain the specific involvem.nt of the Circle of willis in these *** Circle of Wills is a com.lex geom.trical structure which has several areas with different curvature as well as various branching angles of vessels com.osing the *** hem.dynam.cs in this region should take into account the Dean num.er which indicates the influence of curvature on the resistance to blood ***,areas with various curvature and angles m.y have different hem.dynam.cs and there are certain areas in the Circle of willis that are m.re likely to develop atherosclerotic ***,this could suggest the novel pathophysiological pathway resulting from.the geom.tric peculiarities of the Circle of *** of the directions of future research is to exam.ne whether specific areas of the Circle of willis are m.re likely to develop atherosclerotic changes com.ared to other *** areas of the Circle of willis affected by atherosclerotic changes could indicate the prim.ry role of atherosclerosis prom.ting Alzheim.r's disease although other pathophysiological m.chanism. suggesting the opposite direction should be also exam.ned in prospective studies.

BACKGROUND: Intestinal endom.triosis m.y be com.licated by bowel obstruction, colonic rupture, sepsis, and rarely, m.lignant transform.tion. Fistula form.tion is extrem.ly rare. CASE: A 26-year-old wom.n presented at 16 weeks of gestation with an acute abdom.n suggestive of ruptured appendicitis. Blood cultures were positive for Bacteroides fragilis. At laparotom., she was found to have a colouterine fistula with pelvic sepsis. The resected specim.ns dem.nstrated extensive uterine adenom.osis and endom.triosis of the cecum. with a fistulous tract lined by endom.triosis and suppurative inflam.ation extending from.the cecum.to the uterine endom.trial cavity associated with severe chorioam.ionitis and endom.om.tritis. CONCLUSION: This case illustrates a rare com.lication of colouterine fistula secondary to intestinal endom.triosis.

Background and Purpose - Concom.tant disease of the supraaortic arteries can influence the outcom. of surgical treatm.nt of carotid artery stenosis. However, sensitivity and specificity data of noninvasive contrast- enhanced 3- dim.nsional (3D) m.gnetic resonance angiography (CE m.A) for the detection of stenoocclusive disease of the entire supra- aortic arteries including the circle of willis rem.in unclear. We aim.d to intra- individually com.are high- spatial- resolution CE 3D m.A and digital subtraction angiography (DSA) for the assessm.nt of steno- occlusive vascular disease of the supra- aortic arteries. m.thods - CE m.A and DSA of the supra- aortic arteries were prospectively perform.d in 50 consecutive patients. Intra- individual com.arison of CE m.A and DSA was available in 833 arteries. High- spatial- resolution CE m.A com.rised a m.asured voxel size of 0.81 m. × 0.81 m. × 1 m. (0.66 m.3). Steno- occlusive vascular disease of the 833 arteries was assessed independently by 2 radiologists according to the NASCET criteria. Results - CE m.A had a sensitivity of 100% (73/73), a specificity of 99.3% (760/765), a positive predictive value of 93.6% (73/78), and a negative predictive value of 100% (760/760) by using a 70% to 99% threshold of arterial diam.ter stenosis. For detection of occlusion, sensitivity, specificity, PPV, and NPV value of CE m.A were 100% , respectively. Conclusions - Noninvasive high- spatial- resolution CE m.A is suited to replace diagnostic DSA for the detection of stenoocclusive disease of the supra- aortic arteries.
