Immune checkpoint inhibitors targeting the programmed cell death-1(PD-1)protein significantly improve survival in patients with advanced non-small-cell lung cancer(NSCLC),but its impact on early-stage ground-glass opacity(GGO)lesions remains *** is a single-arm,phase II trial(NCT04026841)using Simon’s optimal two-stage design,of which 4 doses of sintilimab(200 mg per 3 weeks)were administrated in 36 enrolled multiple primary lung cancer(MPLC)patients with persistent high-risk(Lung-RADS category 4 or had progressed within 6 months)*** primary endpoint was objective response rate(ORR).T/B/NK-cell subpopulations,TCR-seq,cytokines,exosomal RNA,and multiplexed immunohistochemistry(mIHC)were monitored and compared between responders and ***,two intent-to-treat(ITT)lesions(pure-GGO or GGO-predominant)showed responses(ORR:5.6%,2/36),and no patients had progressive disease(PD).No grade 3-5 TRAEs *** total response rate considering two ITT lesions and three non-intent-to-treat(NITT)lesions(pure-solid or solid-predominant)was 13.9%(5/36).The proportion of CD8^(+)T cells,the ratio of CD8^(+)/CD4^(+),and the TCR clonality value were significantly higher in the peripheral blood of responders before treatment and decreased over ***,the mIHC analysis showed more CD8^(+)T cells infiltrated in ***,responders’cytokine concentrations of EGF and CTLA-4 increased during *** exosomal expression of fatty acid metabolism and oxidative phosphorylation gene signatures were down-regulated among ***,PD-1 inhibitor showed certain activity on high-risk pulmonary GGO lesions without safety *** effects were associated with specific T-cell re-distribution,EGF/CTLA-4 cytokine compensation,and regulation of metabolism pathways.

支气管哮喘是临床常见的一种以气道慢性炎症为特征的呼吸系统疾病,其发病机制尚不明确。近年来,微生物学和分子生物学的快速发展让“肺-肠轴”这一概念成为新的热点。肺-肠轴是指存在于呼吸系统与肠道微生物之间的相互作用和调节机制,这个观点与中医“肺与大肠相表里”理论所见略同。随着哮喘疾病的不断增多,肺-肠轴成为一个备受关注的课题,深入研究肺-肠轴或有助于寻找治疗哮喘的新靶点。临床经验表明,中西医结合治疗哮喘可显著改善肺功能指标和临床评估标准,并能够降低哮喘的复发率和控制药物的使用量。其中,西医以抗生素药物为治疗哮喘的主要手段,具有见效快的优势,但也容易引起不良反应。中药治疗具有减轻哮喘症状、不良发应少和改善生活质量等显著优势,其中的针灸和推拿等中医技术均可增强防御力和调节免疫功能。中西医结合干预哮喘有着明显的临床疗效和广阔的应用前景。故该研究通过肺-肠轴理论阐述中西医干预哮喘的研究进展,以细致深入的进行机制探究,旨在为临床通过肺-肠轴理论采取合理的中西医干预哮喘措施提供依据。

印尼煤在配煤炼焦中的应用

背景与目的肺癌高居中国恶性肿瘤发病率和死亡率之首。既往肺癌筛查试验大多为针对吸烟者等高危人群的选择性筛查,然而亚洲肺癌病例非吸烟女性占相当比例。本研究旨在评估广州市社区女性的肺癌患病率并探究肺癌高危因素。方法采用胸部低剂量计算机断层扫描(low-dose computed tomography,LDCT)对广州市40-74岁女性居民行肺癌筛查,并根据2018版《中国肺癌低剂量螺旋CT筛查指南》对肺结节进行分类管理及计算阳性结节和肺癌检出率。筛查对象在行检查前均须完成“肺癌危险因素调研问卷”,后对问卷危险因素进行最小绝对收缩和选择算子惩罚Logistic回归分析。结果分析共纳入6256名女性,发现阳性结节1228例(19.63%),确诊肺癌117例(1.87%),其中原位癌6例,I期103例(占肺癌比例88.03%)。根据问卷变量行最小绝对收缩和选择算子(Least absolute shrinkage and selection operator,LASSO)惩罚Logistic回归分析发现,年龄≥50岁(OR=1.07,95%CI:1.06-1.07)、个人恶性肿瘤史(OR=3.29,95%CI:3.22-3.37)、纺织职业(OR=1.10,95%CI:1.08-1.13)、小时候使用煤为燃料(OR=1.14,95%CI:1.13-1.16)以及食物过敏史(OR=1.10,95%CI:1.07-1.13)是该地区女性肺癌的高危因素。结论胸部LDCT可以发现大量早期肺癌,可用于女性肺癌筛查。年龄≥50岁、个人恶性肿瘤史、纺织职业、小时候使用煤为燃料和食物过敏史是该地区女性人群肺癌的高危因素,需提高该群体对肺癌早筛的重视程度。

以同时含有低直链淀粉含量基因Wx-mp、香味fgr基因、稻瘟病抗性基因Pi-b的优良食味粳稻中间品系‘宁7020’为母本,携带稻瘟病抗性基因Pi-ta的江苏抗病、高产粳稻品种‘扬粳9538’为父本,配置杂交组合进行聚合育种。利用3套自主研发的PCR检测体系稻瘟病Pi-ta和Pi-b的多重PCR体系、Wx-mp基因的四引物扩增受阻突变体系PCR检测技术、香味fgr基因的InDel标记检测技术,分别在不同的世代进行目标基因跟踪,结合抗性、香味和籽粒胚乳外观表型鉴定,成功地将上述4个基因聚合于一体,选育出优质、高产、多抗、早熟的水稻新品系‘南粳58’。本研究为选育多抗、优质水稻新品种提供了一种简单、快捷的选择技术,同时也为水稻抗病、优质育种提供新的种质资源。
