Cohort study on the treatment of BRAF V600E mutant metastatic colorectal cancer with integrated Chinese and western medicine

BACKGROUND Patients with BRAF V600E mutant metastatic colorectal cancer(mCRC)have a low incidence rate,poor biological activity,suboptimal response to conventional treatments,and a poor *** the previous cohort study on mCRC conducted by our team,it was observed that integrated Chinese and Western medicine treatment could significantly prolong the overall survival(OS)of patients with colorectal ***,we further explored the survival benefits in the population with BRAF V600E mutant *** To evaluate the efficacy of integrated Chinese and Western medicine in the treatment of BRAF V600E mutant metastatic colorectal *** A cohort study was conducted on patients with BRAF V600E mutant metastatic colorectal cancer admitted to Xiyuan Hospital of China Academy of Chinese Medical Sciences and Traditional Chinese Medicine Hospital of Xinjiang Uygur Autonomous Region from January 2016 to December *** patients were divided into two *** A total of 34 cases were included,with 23 in Chinese-Western medicine cohort(cohort A)and 11 in Western medicine cohort(cohort B).The median overall survival was 19.9 months in cohort A and 14.2 months in cohort B,with a statistically significant difference(P=0.038,hazard ratio=0.46).The 1-3-year survival rates were 95.65%(22/23),39.13%(9/23),and 26.09%(6/23)in cohort A,and 63.64%(7/11),18.18%(2/11),and 9.09%(1/11)in cohort B,*** analysis showed statistically significant differences in median OS between the two cohorts in the right colon,liver metastasis,chemotherapy,and first-line treatment subgroups(P<0.05).CONCLUSION Integrated Chinese and Western medicine can prolong the survival and reduce the risk of death in patients with BRAF V600E mutant metastatic colorectal cancer,with more pronounced benefits observed in patients with right colon involvement,liver metastasis,combined chemotherapy,and first-line treatment.

Infiltration by monocytes of the central nervous system and its role in multiple sclerosis: reflections on therapeutic strategies

Mononuclear macrophage infiltration in the central nervous system is a prominent feature of neuroinflammation. Recent studies on the pathogenesis and progression of multiple sclerosis have highlighted the multiple roles of mononuclear macrophages in the neuroinflammatory process. Monocytes play a significant role in neuroinflammation, and managing neuroinflammation by manipulating peripheral monocytes stands out as an effective strategy for the treatment of multiple sclerosis, leading to improved patient outcomes. This review outlines the steps involved in the entry of myeloid monocytes into the central nervous system that are targets for effective intervention: the activation of bone marrow hematopoiesis, migration of monocytes in the blood, and penetration of the blood–brain barrier by monocytes. Finally, we summarize the different monocyte subpopulations and their effects on the central nervous system based on phenotypic differences. As activated microglia resemble monocyte-derived macrophages, it is important to accurately identify the role of monocyte-derived macrophages in disease. Depending on the roles played by monocyte-derived macrophages at different stages of the disease, several of these processes can be interrupted to limit neuroinflammation and improve patient prognosis. Here, we discuss possible strategies to target monocytes in neurological diseases, focusing on three key aspects of monocyte infiltration into the central nervous system, to provide new ideas for the treatment of neurodegenerative diseases.

Whole-lifetime Coordinated Service Strategy for Battery Energy Storage System Considering Multi-stage Battery Aging Characteristics

One battery energy storage system(BESS)can be used to provide different services,such as energy arbitrage(EA)and frequency regulation(FR)support,etc.,which have different revenues and lead to different battery degradation *** paper proposes a whole-lifetime coordinated service strategy to maximize the total operation profit of BESS.A multi-stage battery aging model is developed to characterize the battery aging rates during the whole *** the uncertainty of electricity price in EA service and frequency deviation in FR service,the whole problem is formulated as a twostage stochastic programming *** the first stage,the optimal service switching scheme between the EA and FR services are formulated to maximize the expected value of the whole-lifetime operation *** the second stage,the output power of BESS in EA service is optimized according to the electricity price in the hourly timescale,whereas the output power of BESS in FR service is directly determined according to the frequency deviation in the second *** above optimization problem is then converted as a deterministic mixed-integer nonlinear programming(MINLP)model with bilinear *** Cormick envelopes and a bound tightening algorithm are used to solve *** simulation is carried out to validate the effectiveness and advantages of the proposed strategy.

The effect of different high pressure die casting parameters on 3D microstructure and mechanical properties of AE44 magnesium alloy

To understand the relationship between the process-microstructure-mechanical properties of the high-pressure die-casting(HPDC) AE44 magnesium alloy, 3D reconstruction and 2D characterization were carried out on the HPDC castings produced with different process parameters(low slow-shot speed, fast slow-shot speed, solidification pressure). Microstructural characterization revealed that the formation of shrinkage pores are closely related to ESCs, which were mainly controlled by the low slow-shot speed in shot sleeve(ESCs growth time) and fast slow-shot speed into the die cavity(distribution of ESCs). In addition, solidification pressure can significantly reduce the shrinkage porosity in the center by improving the feeding capacity of liquid metal. Tensile fracture revealed that the tearing ridge is mainly evolved from the slip band of ESCs. The quantity and distribution of ESCs determine the fracture mode of castings. The relationship between mechanical properties of castings and the morphology of ESCs and porosity is also statistically discussed.

This paper develops a fully data-driven,missingdata tolerant method for post-fault short-term voltage stability(STVS)assessment of power systems against the incomplete PMU *** super-resolution perception(SRP),based on a deep residual learning convolutional neural network,is employed to cope with the missing PMU *** incremental broad learning(BL)is used to rapidly update the model to maintain and enhance the online application *** different from the state-of-the-art methods,the proposed method is fully data-driven and can fill up missing data under any PMU placement information loss and network topology change *** results demonstrate that the proposed method has the best performance in terms of STVS assessment accuracy and missing-data tolerance among the existing methods on the benchmark testing system.

PGT or ICSI?The impression of NGS-based PGT outcomes in nonmosaic Klinefelter syndrome

This retrospective study demonstrates the clinical outcomes of patients with nonmosaic Klinefelter's syndrome(KS)who underwent preimplantation genetic testing(PGT)with frozen-thawed testicular *** testicular sperm extraction(micro-TESE)was performed for sperm ***-generation sequencing(NGS)was conducted for embryo analysis.A total of 18 couples aged≤35 years were included,and 22 oocyte retrieval cycles were *** was detected in 29 of 45(64.4%)***,the numbers of aneuploid and mosaic embryos detected were 8(17.8%)and 8(17.8%),respectively,regardless of a lack of sex chromosome ***,13 couples with euploid embryos completed 14 frozen embryo transfer(FET)*** couples had clinical pregnancies,and 6 of them had already delivered 5 healthy babies and 1 monozygotic *** were also 4 ongoing pregnancies and 2 biochemical pregnancies,but no early pregnancy loss was *** on our results,we speculate that for KS patients,when sperm can be obtained by micro-TESE,the cryopreservation strategy makes the ovarian stimulation procedure more favorable for female *** paternal genetic risk of sex chromosome abnormalities in their offspring is extremely low in men with *** addition to PGT,the intracytoplasmic sperm injection(ICSI)procedure is comparably effective but more economical for young nonmosaic KS *** should be offered as an option for such couples,but monitoring by prenatal genetic diagnosis is recommended.

Effect of Ca and Ti contents on characteristics of inclusions in Fe-Si-Cr-Mn-Al-Ti-Ca-O melts

The inclusion characteristics in 55SiCr spring steel with different contents of titanium and calcium were *** chemical compositions of steel samples were detected by inductively coupled plasma optical emission spectrometer,and the inclusion characteristics was determined by field emission scanning electron microscopy(FE-SEM)and energy-dispersive *** results show that the Ti/Al ratio should be kept at less than 1,and the content of calcium should be controlled between 0.0015 and 0.0025 wt.%in Si-Mn-Al deoxidized steel,so that more solid inclusions can be converted to liquid ***,the high Ti content in melts is the direct cause of the high proportion of Ti_(3)O_(5) in the inclusions,which involves[Ti]to reduce SiO_(2) and A1_(2)O_(3) in *** addition,calcium treatment can reduce the content of Ti_(3)O_(5) in inclusions,and the degree of reduction is closely related to the content of[O].The thermodynamic calculation of Fe-Si-Mn-Cr-Al-Ti-Ca-O molten steel system during solidification process was performed by FactSage software,taking all types of inclusions into account,such as titanium oxide,calcium oxide,aluminum oxide,silicon oxide,manganese oxide,calcium titanate,mullite,calcium aluminate,and liquid *** inclusion type of calculation results was in accordance with the experimental results at 1550℃,and TiO_(x) aggregation behavior was consistent with the Ti_(2)O_(3)-containing precipitation phase.
