Background and Aim: Hepatic hydrothorax is one of the complications encountered in end stage liver disease. Pleural drainage carries the risk of massive protein and electrolyte depletion as well as the risk of bleeding and hepatic encephalopathy. Pleurodesis following pleural aspiration decreases the chance of pleural effusion recurrence, and has been a widely used long-standing method of controlling recurrent pleural effusions. The aim of this study is to evaluate the effect of pleurodesis using ultrasound-guided iodopovidone sclerotherapy in hepatic hydrothorax. Patients and methods: This prospective study included 56 patients with clinical, laboratory and radiological evidence of liver cirrhosis and symptomatic right sided hepatic hydrothorax. All patients were subjected to repeated thoracentesis. Ten ml of lidocaine 2% were injected in the pleural space followed by 20 ml of iodopovidone. The follow-up was done after 3 months. Results: The sclerotherapy procedure was successful in 40 out of 56 cases (71.4%), and the success rate was 66.7% in massive effusion and reached 80% in moderate effusion. Twenty eight patients (50%) had to repeat the procedure for a second time, sixteen of which (28.6%) failed despite the second trial and twelve cases (21.4%) showed no fluid reaccumulation. Conclusion: Ultrasound-guided iodopovidone sclerotherapy is an effective approach for a successful pleurodesis in hepatic hydrothorax.

Salinity is one of the major abiotic factors that limit the growth and productivity of *** application of plant growth regulators(PGRs)may help plants ameliorate the negative impacts of ***,a field experiment was conducted at the Botanical Garden University of Balochistan,Quetta,to explore the potential role of PGRs,i.e.,moringa leaf extract(mLE;10%),proline(PRO;1μm),salicylic acid(SA;250μm),and thiourea(TU;10 mm)in ameliorating the impacts of salinity(120 mm)on Plantago ovata,an important medicinal *** hampered plant photosynthetic pigments and metabolites but elevated oxidative ***,foliar application of PGRs enhanced photosynthetic pigments,including Chl b(21.11%),carotenoids(57.87%)except Chl a,activated the defense mechanisms by restoring and enhancing the metabolites,i.e.,soluble sugars(49.68%),soluble phenolics(33.34%),and proline(31.47%),significantly under salinity ***,foliar supplementation of PGRs under salt stress led to a decrease of about 43.02%and 43.27%in hydrogen peroxide and malondialdehyde content,***,PGRs can be recommended for improved photosynthetic efficiency and metabolite content that can help to get better yield under salt stress,with the best and most effective treatments being those of PRO and mLE to predominately ameliorate the harsh impacts of salinity.

AIm:To study the hemodynamics in the immediate post transplant period and compare patients with alcoholic vs viral cirrhosis. mETHODS:Between 2000-2003,38 patients were transplanted for alcoholic cirrhosis and 28 for postviral *** rate(HR),central venous pressure(CVP), mean arterial pressure(mAP),pulmonary capillary wedge pressure(PCWP),cardiac index(CI),systemic vascular resistance index(SVRI),pulmonary artery pressure(PAP),and pulmonary vascular resistance index(PVRI)were measured immediately and 24 h post transplantation. RESULTS:Hyperdynamic circulation persisted at 24 hfollowing transplantation with an elevated CI of 5.4± 1.3 L/(min×m 2 )and 4.9±1.0 L/(min×m 2 )in the viral and alcoholic groups,respectively,and was associated with a decreased *** the first 24 h, there was a significant decrease in HR and increase in mAP;the extent of the change was similar in both *** CVP,PCWP,and SVRI increased,and CI decreased in the viral patients,but not the alcoholic *** showed a lower PVRI(119±52 dynes/(cm 5 ×m 2 )vs 166±110 dynes/(cm5×m2),PmmHg vs 24±7 mmHg,Pmpared to the viral group at 24 h. CONCLUSION:Hyperdynamic circulation persists in the immediate post-transplant period with a faster improvement in the viral *** patients have a more pronounced pulmonary vasodilatation.

Wind power is not only pollution free and renewable but is becoming more economical with technology development. The countries around the Arabian Gulf region are investing for wind powers both the land and in their marine space. A detailed study on wind power resource in the Arabian Gulf is not available. This paper is on the wind energy resource availability over Arabian Gulf waters, covering 2300 grid points. The data available with Coastal Information System data base at Kuwait Institute for Scientific Research, Kuwait, from 1979 to 2015 is used. The probability density and power density were derived from these data source. Weibull probability density function has been fitted to the wind speed data and the wind power density was evaluated. The study is carried out at 10 m, 30 m and 50 m elevations. The central location of the Arabian Gulf has higher annual average wind speed, ranging from 6 to 8 m/s at 10 m elevation, 7 to 8 m/s at 30 m elevation and 8 to 9 m/s at 50 m elevation. The scale parameter “c” at central location of Arabian Gulf is found to range from 6 - 8 m/s for 10 m elevation, 7 - 8 m/s for 30 m elevation and 8 - 9 m/s for 50 m above sea level. The Weibull shape parameter k varies from 2.5 to 3 at the north and central of the Arabian Gulf. The annual mean wind power density over Arabian Gulf Waters is the highest in the central region of the Gulf. The power density at 10 m, 30 m and 50 m hub height varies between 200 to 300 w/m2, 200 to 300 w/m2 and more than 300 w/m2 respectively. It is attractive to install large scale wind power generation at the central region of the Arabian Gulf and at elevations of 30 m or 50 m, since this region lies in Class 2 category of the Wind Energy Resource Atlas of the United States. The wind power density is attractive especially in summer season around the central region location in Arabian Gulf (Saudi Arabia, Bahrain and Qatar), since high rate of power is used for air conditioning systems in summer.

Background: The treatment of breast cancer in large breast patients represents a great challenge to both surgical oncologist and radiation oncologist. The aim of this study is to evaluate the outcome of inferior pedicle therapeutic mammoplasty in large-breasted patients with upper quadrants early breast cancer. methods: Thirty five large-breasted patients with early breast cancer were included in this study. Simultaneous bilateral inferior pedicle therapeutic mammoplasty was performed. Results: The age of the patients is ranged from 36 to 61 (median 46) years and tumour size is ranged from one to three and half cm. The weight of tissue removed is ranged from 350 gm to 780 gm and the tumour safety margins are ranged from three to eight cm. Wound dehiscence was the commonest post operative complications and six patients were affected (17.6%). The cosmetic outcome was excellent in 22 patients (64.5%), nine patients (26.5%) showed good results, two patients (6%) were satisfactory and one patient (3%) showed poor result. The follow up period is ranged from 6 to 42 months with one case (3%) of systemic metastasis. Conclusion: Inferior pedicle therapeutic reduction mammoplasty for upper quadrants early breast cancer in large breasted women is a surgically and oncologically safe procedure, and it carries a satisfactory aesthetic outcome.

Background: Controversy exists over the extent of surgical treatment of papillary thyroid *** may carry a significant risk of surgical complications. The aim of this study is to investigate the complications following surgical treatment of thyroid cancer and the association between the?extent of surgery and complication rates. methods: A total of 196 patients with papillary thyroid cancer were retrospectively reviewed to identify extent of surgery and associated complications, between 2009 and 2018 atSouth Egypt Cancer Institute Assiut University. Results: Primary procedures included less-than-total thyroidectomy (near total thyroidectomy in 5.1%, subtotal thyroidectomy in 4.1%, thyroid lobectomy in 3.1%) and total thyroidectomy in 87.8% (18.9% of them are completion thyroidectomy). No lymph nodes dissection was done in 10.7%, Berry picking in 6.1%, central compartment neck dissection in 41.8%, central compartment neck dissection with modified ipsilateral radical neck dissection in 12.2%, central compartment neck dissection with modified bilateral radical neck dissection in 26.0%, central compartment dissection with both modified bilateral radical neck dissection and superior mediastinal lymph node dissection 3.1%. The most common surgical complication were transient hypoparathyroidism (16.7% - 45.8%) and 2.0% permanent, transient vocal cord palsy (7.3% - 16.7%), and 3.9% permanent, hematoma 1.5%, seroma 7.1% and chyle fistula in 2.6% of patients. Conclusion: Thyroid cancer surgeries are safe. The transient complication rates of hypoparathyroidism increased significantly with increasing the extent of surgery.

AIm:To assess sustained virological response(SVR) rates in a predominantly hepatitis C virus(HCV) genotype 4 infected population. mETHODS:Between 2003-2007,240 patients who were treated for chronic hepatitis C infection at our center were *** data,viral genotypes,and treatment outcomes were evaluated in all treated *** with chronic renal failure, previous non-responders,and those who relapsed after previous treatment were excluded from the *** all patients,57%were treated with PEG-interferon(IFN)α-2a and 43%patients were treated with PEG-IFNα-2b;both groups received a standard dose of ribavirin. RESULTS:89.6%of patients completed the treatment with an overall SVR rate of 58%.The SVR rate was 54%in genotype 1,44%in genotype 2,73%in genotype 3,and 59%in genotype 4 *** was no statistical difference in the SVR rate between patients treated with PEG-IFNα-2a and PEG-IFNα-2b (61.5%vs 53%).Patients younger than 40 years had higher SVR rates than older patients(75%vs 51%,P =0.001).SVR was also statistically significantly higher when the HCV RNA load(pretreatment)was below 800.000(64%vs 50%,P=0.023),in patients with a body mass index(BmI)less than 28(65%vs 49%,P =0.01),and in patients who completed the treatment duration(64%vs 8%,P≤0.00001).CONCLUSION:The SVR rate in our study is higher than in previous *** with the standard duration of treatment,higher ribavirin dose,younger age,lower BmI,and low pretreatment RNA levels were associated with a higher virological response.

HIV-1 capsid protein(CA) has emerged as a promising target for antiviral treatment considering its structural and regulatory roles in HIV-1 replication. Here, we disclose the design, synthesis, biological assessment, and mechanism investigation of a novel series of phenylalanine derivatives gained by further structural modification of PF74. The newly synthesized compounds demonstrated potent anti-HIV activity, represented by 7n displayed anti-HIV-1 activity 6.25-fold better than PF74, and 7h showed anti-HIV-2activity with nearly 139 times improved efficacy over PF74. Surface plasmon resonance(SPR) studies of representative compounds proved that HIV-1 CA was the binding target. Competitive SPR studies using CPSF6 and NUP153 peptides identified that 7n binds to a vital CA assembly interface between the Nterminal and C-terminal domain(NTD-CTD interface). Action stage determination assay revealed that the newly synthesized compounds were antiviral with a dual-stage inhibitory profile. molecular dynamics(mD) simulations offered the crucial foundation for the hopeful antiviral potency of 7n. Besides, 7m and7n modestly increased metabolic stabilities in human liver microsome(HLm) and human plasma compared to PF74. Overall, these studies offer valuable insights and can regard as the beginning for succedent medicinal chemistry endeavors to discover promising HIV capsid inhibitors with improved efficacy and better drug-like characteristics.

Alumina silicate powder which is extracted from the obsolete spark plug NGK (insulator part as a filler) has been used to produce epoxy/alumina silicate composite. The dielectric behavior of the composite materials (epoxy resin-alumina silicate NGK) is analyzed as a function of the filler content, temperature and frequency. AC conductivity and impedance are also studied. The results show that the permittivity, dielectric loss and loss tangent for all composites increase with increasing alumina silicate NGK filler content.
