Objective: The aim of this trial was to determine whether endometrial scratch-suction caused by insertion tube of (IUCD) with suction improved the probability of pregnancy in the subsequent IVF cycle in patients who had previous failed IVF outcome by removing small polyps and any hazardous materials. Design: Observational prospective cohort experimental study. setting: Zagazig University Hospital, Obstetrics and Gynecology department. From period between september 2011 till september 2012. Population: 30 patients with previous failed IVF despite transfer of goodquality embryos were allocated to endometrial scratch-suction in the same the cycle of embryo transfer. Methods: From day 5 to day 7 of same ICsI cycle, the patients in study underwent endometrial scratch-suction technique. After preparation of insertion tube of (IUCD) and infant feeding tube 8 F, moving the insertion tube of IUCD from below upwards with scratching anterior uterine wall, then posterior, then left lateral uterine wall, then right lateral uterine wall, then fundus, all these steps are associated with suction by attaching proximal end of infant feeding tube 8 F to suction apparatus. Main Outcome Measures: Implantation and clinical pregnancy rates. Results: The implantation occurred in 24/30 (80%). The clinical pregnancy occurred in 20/30 patients (66.66%). 12/30 patients (40%) during scratch-suction technique had small fleshy masses obstructing suction tube which were small endometrial polyps. Conclusions: Endometrial scratch-suction improves clinical pregnancy and implantation rates in the same IVF-ET cycle in patients with previous unsuccessful IVF cycles.

为了推动中国与伊斯兰世界关系的进一步发展,满足广大穆斯林在中国使用清真寺的文化需求,研究了多元文化背景下的清真寺庭院设计.清真寺由于其独特的设计形式,成为"中国-伊斯兰"多元文化融合中的重要代表,庭院作为清真寺中的一个基本构成元素,在多元文化融合背景下表现出来的形式亦各不相同.为了提升设计质量,且用户参与设计过程的需求也在不断增长,研究旨在总结传统和现代中国清真寺庭院设计在阿拉伯穆斯林使用者中现状评价状况,以评估其是否符合阿拉伯穆斯林使用者的需求.通过对居住在南京的阿拉伯穆斯林人群进行问卷调查,分析传统与现代清真寺庭院的设计要点,并对其设计方案的质量进行评价.通过清真寺庭院设计中的7个自然和人工元素,分析了清真寺庭院的功能及其结构特征.研究结果可应用于对未来清真寺庭院设计的改进提升,使其满足中国和伊斯兰穆斯林用户在使用以及审美方面的需求.

Introduction: This study aimed to evaluate, discuss and illustrate the role of fetoscopy diagnostically and therapeutically. Material and Methods: This study was conducted in private center in under the supervision of the professors of Azhar University, Egypt from Dec-2012 to Mar-2017. Women with confirmed fetal congenital malformations, and willing to do fetoscopy were recruited. Fetoscopy was attempted in all cases to treat the underlying fetal conditions. Follow up was made until delivery. Results: Twenty patients with 22 fetuses were included in this study with different congenital anomalies. Therapeutic drainage or coagulation was made in all cases. In cases of lower urinary tract obstruction, fetoscopy confirmed pre-suspected urethral atresia and changed the diagnosis from complete PUV to urethral atresia in some cases. The procedure succeeded in all cases. However, the on-going success rate was 17 (77.3%) of fetuses. The overall number of live birth was 11 fetuses (50%). Overall, 15 (75%) of the 20 patients experienced some complication withpreterm labor the most prevalent 15 (75%). Conclusion: Fetoscopy can diagnose and differentiate between posterior urethral valve (PUV) and atresia, and manage well-selected cases of twin to twin transfusion syndrome (TTTs), and have a reasonable survival rate in lower urinary tract obstruction (LUTO).

In this paper, an internal multiband antenna is proposed for LTE-A/WWAN wireless applications in tablet computer. The proposed antenna is configured to have two branch radiators. These two branch radiators are a U-shaped driven monopole and a nonuniform wrapped inverted U-shaped monopole. The impedance bandwidths across dual operating bands are 89.7 MHz and 4185 MHz at the LTE-A/WWAN bands. Various techniques, such as branching and parasitic element are used to enhance the antenna’s bandwidth, the matching, and the size of the proposed antenna. The antenna is presented on an area of 50 × 15 mm2. Experimental results of this antenna show nearly omni-directional coverage and stable gain variation across the LTE-A/WWAN bands.

AIM: To investigate the relationship between low molecular polypeptide-7 (LMP-7) gene polymorphism and response to interferon (IFN) therapy in chronic hepatitis C virus (HCV) patients. METHODs: LMP-7 polymorphism at codon 49 with nucleotide substitution from A to C was amplified in 104 chronic HCV patients of genotype 4. The amplicons were digested with restriction endonuclease Bsm I and the produced restriction fragment length polymorphism was analyzed. Patients received IFN + regional blood volume therapy for 48 wk and the frequency of thissingle nucleotide polymorphism (sNP) was statistically correlated with treatment response. The exclusion criteria for these patients were stated by the national health program for treating viral hepatitis. Main exclusion criteria included co-infection with hepatitis B virus or schistosomiasis, thyroid dysfunction, uncontrolled diabetes mellitus, history of long term drug or alcohol intake and autoimmune hepatitis. Multivariate analyses were done to correlate LMP-7 sNP plus several factors such as age, gender, weight, serum alpha-fetoprotein (AFP) and alanine aminotransferase levels, liver activity, fibrosis score and viral load with response to therapy. REsULTs: The data presented in this study clearly demonstrated statistically significant differences between sustained virological response (sVR) (defined as the absence of HCV RNA levels in the patient's sera at least 6 mo after discontinuation of treatment) and non-response (NR) (where HCV RNA levels in the patient's sera never become undetectable for 6 mo during or after treatment). Variables were described as odds ratio with 95%CI. The data were considered significant if P values were ≤ 0.05; highly significant if P significant if P showed that 91.7% of patients carrying LMP-7 C/C allele were associated with sVR, while the other two genotypes C/A and A/A were associated with NR patients, 83.3% and 64.3% respectively, showing that g

Thiopurines are widely used for maintenance treatment of inflammatory bowel disease. Interindividual variability in clinical response to thiopurines may be attributed to several factors including genetic polymorphisms, severity and chronicity of disease, comorbidities, duration of administration, compliance issues and use of concomitant medication, environmental factors and clinician and patient preferences. The purpose of this review is to summarise the current evidence on thiopurine safety and toxicity, to describe adverse drug events and emphasise the significance of drug interactions, and to discuss the relative safety of thiopurine use in adults, elderly patients, children and pregnant women. Thiopurines are safe to use and well tolerated, however dose adjustment or discontinuation of treatment must be considered in cases of non-response, poor compliance or toxicity. Drug safety, clinical response to treatment and short to long term risks and benefits must be balanced throughout treatment duration for different categories of patients. Treatment should be individualised and stratified according to patient requirements. Enzymatic testing prior to treatment commencement is advised. surveillance with regular clinic follow-up and monitoring of laboratory markers is important. Data on long term efficacy, safety of thiopurine use and interaction with other disease modifying drugs are lacking, especially in paediatric inflammatory bowel disease. High quality, collaborative clinical research is required so as to inform clinical practice in the future.

An Experimental study on the Effect of Foundation Depth, size and shape on subgrade Reaction of Cohessionless soil

The modulus of subgrade reaction ks depends on several factors such as the size and shape of the foundation as well as the embedment depth of the foundation. The present study is an experimental analysis using plate load test to determine the effect of foundation depth, size as well as the shape on the modulus of subgrade reaction (ks) of cohesionless soils. It was carried out by using nine rigid steel plates with different sizes and shapes (circular, square and retangular). The tests were carried out on cohessionless soil with different relative densities under different applied pressures. The settlement has been measured at the surface of the plate for different depths of footings. The ultimate bearing capacity [qu] has been determined from the stress-settlement relationships. The allowable bearing capacity (qa) was determined by dividing the ultimate bearing capacity (qu) by F.s. = 3.0, after which the corresponding settlement (sa) has been obtained. However, ks was calculated based on dividing the allowable bearing capacity (qa) by the corresponding settlement (sa). From the present study it is concluded that the subgrade reaction ks of cohessionless soil increases with increasing foundation depth as well as foundation size. In addition, subgrade reaction ks of cohessionless soil under rectangular footing is higher than that under square and that under circular one with same equivalent area. An empirical formula is presented to calculate the subgrade reaction ks of cohessionless soil under square foundation taking into consideration foundation depth. Fair agreement has been obtained between values of ks from the empirical formula at depth of footing = 0.00 B and Biot (1937) as well as Meyerhof and Baike (1965).
