Outcome of repair of chronic tear of the pectoralis major using corkscrew suture anchors by box suture sliding technique

AIM To assess the functional and clinical results of repair of chronic tears of pectoralis major using corkscrew and sliding suture technique. METHODS In this retrospective study, we reviewed the results of pectoralis major repair in 11 chronic cases(> 6 wk) done between September 2011 and December 2014 at our institute. In all cases repair was done by same surgeon using corkscrew suture anchors and box suture sliding technique. At 6 mo, after surgery magnetic resonance imaging was done to see the integrity of the repair. Functional evaluation was done using Penn and ASES scores. Pre and postoperative Isokinetic strength was *** Average follow-up was 48.27 ± 21.0 mo. The Wilcoxon signed rank test was used to evaluate the outcome scores. The average ASES score increased from an average of 54.63 ± 13.0 preoperatively to 95.09 ± 2.60 after surgery at their last follow-up. The average Penn score also increased from 5.72 ± 0.78, 2.81 ± 1.32 and 45.81 ± 1.72 to 9.36 ± 0.80, 8.27 ± 0.90 and 59 ± 1.34 for pain, satisfaction and function respectively. Follow up magnetic resonance imaging(MRI)(at 6 mo) showed continuity and the bulk of pectoralis major muscle in all cases. Average isokinetic strength deficiency in horizontal adduction at 60° was 13.63% ± 6.93% and at 120° was 10.18% ± 4.93% and in flexion at 60° was 10.72% ± 5.08% and at 120° was 6.63% + 3.74%. Results showed that both ASES and Penn score improved significantly(2 tailed P value = 0.0036).CONCLUSION We could conclude from this series that pectoralis major repair even in chronic cases using 5.5 mm corkscrew anchors give excellent functional and cosmetic results. In chronic cases the repairable length of the tendon is not available and sliding suture technique allows for fixation of worn out tendomuscular junction to bone without letting cutting through the muscle.

Background:Chikungunya virus(CHIKV)is an infectious agent that caused several outbreaks among different countries and affected approximately 1.3 million Indian *** is transmitted by Aedes mosquito–either *** or ***,the clinical manifestations of CHIKV infection involve high-grade fever,joint pain,skin rashes,headache,and *** present study aims to investigate the relationship between the CHIKV virus load and clinical symptoms of the CHIKV infection so that better patient management can be done in the background of the CHIKV outbreak as there is no licensed anti-viral drug and approved vaccines available against ***:CHIKV RTPCR positive samples(n=18)(Acute febrile patients having D.O.F≤7 days)were taken for the quantification of CHIKV viremia by Real-Time *** features of the febrile patients were recorded during the collection of blood ***:The log mean virus load of 18 RT-PCR-positive samples was 1.3×10^(6) copies/mL(1.21×10^(3)–2.33×10^(8) copies/mL).Among the observed clinical features,the log mean virus load(CHIKV)of the patients without skin rash is higher than in the patients with skin rash(6.61 vs.5.5,P=0.0435).Conclusion:The conclusion of the study was that the patients with skin rashes had lower viral load and those without skin rashes had higher viral load.

A neglected shoulder dislocation is a rarer entity and only few cases are reported in the literature. An anterior dislocation of the shoulder is rarely missed as patients present with limb in abduction and external rotation, an attitude very familiar to orthopaedic surgeon. Occasionally such cases are missed when they present with fracture of proximal humerus or when they receive treatment from unqualified practitioners who commonly practise in rural areas. Owing to very few reports there is paucity of literature and no standard treatment protocol exists for neglected anterior dislocation of the shoulder. though most such chronic cases are managed by open reduction. This case report describes a six months old neglected anterior dislocation with a significant Hill Sachs lesion, which was managed by closed reduction and Latarjet procedure.

Removing the smog from digital images is a challenging pre-processing tool in various imaging ***,many smog removal(i.e.,desmogging)models are proposed so far to remove the effect of smog from *** desmogging models are based upon a physical model,it means it requires efficient estimation of transmission map and atmospheric veil from a single smoggy ***,many prior based restoration models are proposed in the literature to estimate the transmission map and an atmospheric ***,these models utilized computationally extensive minimization of an energy ***,the existing restoration models suffer from various issues such as distortion of texture,edges,and ***,in this paper,a convolutional neural network(CNN)is used to estimate the physical attributes of smoggy *** gradient channel prior(OGCP)is utilized to restore the smoggy ***,a dataset of smoggy and sunny images are ***,we have trained CNN to estimate the smog gradient from smoggy ***,based upon the computed smog gradient,OGCP is utilized to restore the still smoggy *** analyses reveal that the proposed CNN-OGCP based desmogging model outperforms the existing desmogging models in terms of various performance metrics.

Colorectal cancer(CRC) is the third most common cancer diagnosed worldwide in human beings. Surgery, chemotherapy, radiotherapy and targeted therapiesare the conventional four approaches which are currently used for the treatment of CRC. The site specific delivery of chemotherapeutics to their site of action would increase effectiveness with reducing side effects. Targeted oral drug delivery systems based on polysaccharides are being investigated to target and deliver chemotherapeutic and chemopreventive agents directly to colon and rectum. Site-specific drug delivery to colon increases its concentration at the target site, and thus requires a lower dose and hence abridged side effects. Some novel therapies are also briefly discussed in article such as receptor(epidermal growth factor receptor, folate receptor, wheat germ agglutinin, VEGF receptor, hyaluronic acid receptor) based targeting therapy; colon targeted proapoptotic anticancer drug delivery system, gene therapy. Even though good treatment options are available for CRC, the ultimate therapeutic approach is to avert the incidence of CRC. It was also found that CRCs could be prevented by diet and nutrition such as calcium, vitamin D, curcumin, quercetin and fish oil supplements. Immunotherapy and vaccination are used nowadays which are showing better results against CRC.

In DNA-guided hepatitis B treatment, viral load is insufficient, and requires other viral markers for treatment of hepatitis B patients as in patients with acute exacerbation of chronic hepatitis B, end-stage renal disease on dialysis, human immunodeficiency virus co-infected patients. There are exceptions to this rule： a residual level hepatitis B virus （HBV） DNA at 24 wk predicts beneficial outcome and reduced resistance at i year. The genotypic viral resistance to antiviral agents and occult HBV infection can be determined by HBV-DNA levels.

Bioelectrochemical systems （BES） have been extensively studied ｜br resource recovery from wastewater. By taking advantage of interactions between microorganisms and electrodes, BES can accomplish wastewater treatment while simultaneously recovering various resources including nutrients, energy and water （＂NEW＂）. Despite much progress in laboratory sit, dies, BES have not been advanced to practical applications. This paper aims to provide some subjective opinions and a concise discussion of several key challenges in BES-based resource recovery and help identil- the potential application niches that may guide further technological development. In addition to further increasing recovery efficiency, it is also important to have more lbcus on the applications of the recovered resources such as how to use the harvested electricity and gaseous energy and how to separate the recovered nutrients in an energy-efficient way. A changc in mindset fur energy performance of BES is necessary to understand overall energy production and consumption. Scaling up BES can go through laboratory scale, transitional scale, and then pilot scale. Using functions as driving forces lbr BES research and development will better guide the investment of efforts.
