Int.oduct.on: While aut.graft.bone is t.e gold st.ndard for mult.level post.rolat.ral lumbar fusion, bone subst.t.t.s and graft.ext.nders such as allograft.bone, ceramics and demineralized bone mat.ix (DBM) have been used t. avoid t.e morbidit. and insufficient.quant.t. associat.d wit. harvest.ng aut.logous bone. t.e primary object.ve of t.is ret.ospect.ve st.dy was t. det.rmine whet.er, in pat.ent. wit. increased risk of operat.ve nonunion relat.d t. mult.level fusion, adding DBM fibers t. mineralized bone allograft.result.d in bet.er fusion t.an using allograft.alone. t.e secondary object.ves were t. evaluat. how adding DBM fibers affect. funct.onal disabilit., low back pain, int.aoperat.ve blood loss and t.e nonunion rat.. Met.ods: t.is ret.ospect.ve st.dy involved a chart.review of consecut.ve pat.ent. who underwent.mult.level lumbar spinal fusion and were operat.d on by a single surgeon. t.e pat.ent. were divided int. t.o groups: 14 pat.ent. received mineralized bone allograft.(cont.ol group) and 14 pat.ent. received a combinat.on of mineralized bone allograft.and DBM (experiment.l group). Pat.ent. were reviewed at.a mean of 16.4 ± 2.2 mont.s aft.r surgery at.which point.Ct.scans were analyzed t. det.rmine whet.er fusion had occurred;Oswest.y disabilit. index (ODI) and pain were also evaluat.d. On exposure t. low pH during gast.ic digest.on, phyt.n dissociat.s int. phyt.c acid and solubilized Ca2+. Even at.low gast.ic pH,phyt.c acid is negat.vely charged which forms t.e basis of it. ant.-nut.it.ve behavior. Pepsinogen has ext.nsive basic amino acids on it. act.vat.on pept.de t.at.are present.d as cat.ons at.low pH which are t.rget.d by pepsin for act.vat.on.
