Acute bleeding after argon plasma coagulation for weight regain after gastric bypass: A case report

BACKGROUND Roux-en-Y gastric bypass(RYGB) is the most commonly performed surgical procedure used to treat obesity worldwide. Despite satisfactory results in terms of weight loss, over time many patients experience weight regain. There are many factors that contribute to weight regain after RYGB, including the diameter of the gastric-jejunal anastomosis(GJA). One of the most commonly performed endoscopic procedures for weight regain after RYGB is argon plasma coagulation(APC). We report a case of hematemesis after outlet revision with APC. We highlight several treatment modalities that can be used to treat this *** sUMMARY A 45-year-old female with a history of weight regain after RYGB was referred for possible endoscopic treatment for weight regain. On endoscopic evaluation, the diameter of the GJA was 22 mm. Due to the dilated GJA, treatment with APC was performed. several months later she reported a return of poor satiety and an increased appetite. A repeat endoscopy was then performed. The GJA was approximately 15 mm and was incompetent. APC was performed. One day post procedure she had four episodes of hematemesis. An endoscopy was performed and a large ulcer with a visible arterial vessel was visualized at the *** was attempted using a Coagrasper and after initial contact with the vessel, the vessel started oozing. Due to fibrosis and the depth of ulceration in the area, clips and repeat APC could not be used. Therefore, an attempt to inject epinephrine injection was made. However, persistent oozing was noted. As a result, hemostatic powder was applied to the region of the bleeding ***, no more bleeding was observed. On follow-up, the patient remained hemodynamically stable and a second look endoscopy was not performed. Pathophysiology of cerebral oedema in acute liver failure

serum-Derived Bovine Immunoglobulin/Protein Isolate Therapy for Patients with Refractory Irritable Bowel syndrome

Background: A small double-blind study showed benefits of serum-derived bovine immunoglobulin/protein isolate (sBI), for diarrhea-predominant irritable bowel syndrome (IBs-D) [1]. The purpose of this chart review was to assess safety and clinical outcomes of sBI in refractory irritable bowel syndrome (IBs) patients. Methods: A retrospective review of 35 IBs patients with diarrhea or mixed diarrhea/constipation pattern (IBs-M) who were administered sBI 5 grams twice daily was performed. Clinical response (“good response” or “no response”) and adverse events were determined by follow-up after four weeks of therapy. Patients were included for evaluation if a lactulose breath test (LBT) had been performed prior to sBI. All patients were refractory to common IBs therapies. The response rate to the inclusion of sBI was calculated in three separate groups: dividing patients based on their LBT results (positive or negative), dividing patients by their IBs diagnosis (IBs-D or IBs-M) and grouping all patients together. Results: Analysis was carried out on 26 IBs-D/-M patients with LBT results. Two patients were lost to follow-up and were excluded from data analysis. The positive LBT group (N = 11) had a 73% (p = 0.117) positive response rate to sBI. The negative LBT group (N = 13) had a significant response rate of 77% (p = 0.040). If patients were divided by IBs diagnosis or grouped together, the response rate to sBI was similarly ranging from 69% - 88%. Adverse events leading to cessation of sBI occurred in 3 of 24 patients. Conclusion: sBI appeared to be a safe and effective nutritional moiety in refractory IBs-D and IBs-M patients. Larger, double-blind studies are needed.

The Contribution of Nutrients and Water Properties to the Carbonate system in Three Particular Areas of the Tropical Atlantic (NE-BRAZIL)

Tropical waters show different regional aspects due to specificities in their nutrient biogeochemical cycles, which can affect the carbon system and influence their regional role as sinks or sources of COsub>2sub>. This study was performed on particular tropical areas that present a different seasonal behaviour related to the carbon cycle observed in the late rainy season (July 2013). Understanding the COsub>2sub> drawdown and outgassing potential in these areas is needed to call attention to more long-term monitoring efforts and protect understudied tropical coastal systems more efficiently. This study is focused on nutrient values, hydrological data, biogeochemical carbon behaviour linked to the carbonate system and includes estimates of COsub>2sub> fluxes in three contrasting areas off the northeastern Brazilian shelf: 1) an urbanised estuary (Recife-REC), 2) a coastal Island (Itamaracá-ITA) and 3) an oceanic archipelago (Fernando de Noronha-FN). In general, REC acted as a source, while ITA and FN as carbon sinks. In ITA, despite the high DIC and Total Alkalinity observed (mean ~2360 μmol·kgsup>-1sup>), the sink is associated with an effective cascading of atmospheric COsub>2sub> associated with turbulent shallow waters coupled with biogenic removal of and precipitation of CaCOsub>3sub> by coralline algae. FN acted as a sink, linked to minor decreases in Total Alkalinity (mean~2295 μmol·kgsup>-1sup>) influenced by ammonium-based primary production, nitrogen fixation and sporadic entrainment of nutrient rich waters in the upper thermocline. More studies in different western tropical Atlantic coastal systems can improve the knowledge of tropical shelf seas and their contribution to the ocean carbon budget under specific regional trophic regimes.

The brainstem is a major site in the central nervous system involved in the processing of the cardiovascular reflexes such as the baroreflex and the peripheral chemoreflex. The nucleus tractus solitarius and the rostral ventrolateral medulla are 2 important brainstem nuclei, and they play pivotal roles in autonomic cardiovascular regulation. Angiotensin II is one of the neurotransmitters involved in the processing of the known that one of the mechanisms by which angiotensin II exerts cardiovascular reflexes within the brainstem. It is well- its effect is via the activation of pathways that generate reactive oxygen species （ROs）. In the central nervous system, ROs are reported to be involved in several pathological dis- eases such as hypertension, heart failure and sleep apnea. However, little is known about the role of ROs in the processing of the cardiovascular reflexes within the brainstem. The present review mainly discussed some recent findings documenting a role for ROs in the processing of the baroreflex and the peripheral chemoreflex in the brainstem.

Propofol Attenuates a-synuclein Aggregation and Neuronal Damage in a Mouse Model of Ischemic stroke

a-synuclein is a soluble monomer abundant in the central nervous system. Aggregates of a-synuclein,consisting of higher-level oligomers and insoluble fibrils,have been observed in many chronic neurological diseases and are implicated in neurotoxicity and neurodegeneration.a-synuclein has recently been shown to aggregate following acute ischemic stroke, exacerbating neuronal *** is an intravenous anesthetic that is commonly used during intravascular embolectomy following acute ischemic stroke. While propofol has demonstrated neuroprotective properties following brain injury, the mechanism of protection in the setting of ischemic stroke is unclear. In this study, propofol administration significantly reduced the neurotoxic aggregation of a-synuclein, decreased the infarct area, and attenuated the neurological deficits after ischemic stroke in a mouse model. We then demonstrated that the propofol-induced reduction of a-synuclein aggregation was associated with increased mammalian target of rapamycin/ribosomal protein s6 kinase beta-1 signaling pathway activity and reduction of the excessive autophagy occurring after acute ischemic stroke.
