Background: The objective of this pilot study was to describe clinical profile, electric, echocardiographic and angiographic caracteristics with procedural outcome of congolease patients undergoing coronarography in the first and only one cardiac catheterization center opened in Kinshasa. methods: An analytical cross-sectional study was carried out over a period from October 2019 (date of establishment of the first coronary angiography unit in DR Congo) to march 2021. We proceeded to a serial sampling of the consecutive cases of all the patients who have an angiographic exploration of coronary arteries. Clinical, ECG and cardiac ultrasound data were collected in all patients. The indications for the coronary angiography examination were set by differents cardiologists on the basis of repolarization troubles in the electrocardiogram, cinetic troubles in echocardiography, positive stress test and chest pain in patients with cardiovascular risq factors. Results: The serie (47 patients) was predominantly male with a sex ratio m/W of 2.6. The average age was 59.8 ± 10.5 years. Arterial hypertension (HBP) was the main risk factor (89.4%);followed by diabetes mellitus (14.9%). Chest pain was the main functional sign with an atypical character in 44.7%. The ECG showed ST segment depression (17%) and T wave inversion (17%), the anterior region being the most affected. Hypokinesia was the most common echocardiographic abnormality (34%), followed by akinesia (10.6%). The anteroseptal and apical territories were affected in 12.8%. Dilated myocardiopathy (DmC) was significantly predominant in the male sex (29.4% vs 7.7%;p = 0.011). With radial puncture as the main approach, coronary angiography was pathological in 44% revealing mono-truncal lesions. The left coronary network was the most affected: the middle inter ventricular artery (12.8%), the proximal interventricular artery (10.6%) and the proximal circonflex artery (10.6%). In a previous study, we showed that patients with muscular dystrophies (mDs) perceive passive movements, experience sensations of illusory movement induced b y muscle tendon vibration, and have proprioceptive- regulated sways in response to vibratory stimulation applied to the neck and ankle muscle tendons. These fi ndings argue for preserved proprioceptive functions of muscle spindles. However, it is unclear whether the function of intrafusal muscle fibers is spared, i.e., whether they retain their ability to contract when submitted to a fusimotor dri ve. To answer this question, we analyzed the effects of reinforcement maneuvers (mental computation and the Jendrassik maneuver) that are known to increase musc le spindle sensitivity via fusimotor drive in healthy subjects. Nine patients wi th different mDs participated in the study. Reinforcement maneuvers increased bo th the mean amplitude of the Achilles tendon reflex (187 ± 52.9% of the mean control amplitude) and the sensitivity of muscle spindle afferents to imposed mo vements of the ankle. The same reinforcement maneuvers failed to alter the ampli tude of the Hoffmann reflex in the triceps surae muscle. These results suggest t hat the intrafusal muscle fibers preserve their contractile abilities in slowly progressive mDs. The reasons for a differential impairment of intrafusal and ext rafusal muscle fibers and the clinical implications of the present results are d iscussed.

Objective: To compare the current non-invasive tests for Helicobacter pylori infection in children and *** design: This multicenter, multinational study investigated the sensitivity, specificity, and positive and negative predictive values of four non-invasive tests: urea breath test (UBT), stool antigen test, and antibody detection in serum and urine, in comparison with biopsy-based ***: Of 503 patients included pre-treatment, 473 fulfilled the definition of H pylori status and among those 316 had results available for the four non-invasive tests (including 133 H pylori-positive patients).The specificity was excellent for all *** UBT had the best sensitivity in all age groups, followed by serology, stool test, and antibody detection in urine.A trend for better sensitivity with an increase in age was observed except for the stool *** receiver operating characteristics (ROC) curves showed that sensitivity of serology, stool test, and urinelisa could be improved by changing the cutoff *** inadequate storage of the specimens may explain the poor results of the stool ***: The UBT appears to be an excellent test for diagnosis of H pylori infection for children and adolescents.

The aim of this study was to analyse torsional optokinetic nystagmus (tOKN) in 17 patients with menière’s disease before and after (1 week, 1 month and 3 months)-a curative unilateral vestibular neurotomy (UVN). The tOKN was investigated during optokinetic stimulations around the line of sight directed towards either the lesioned or the healthy side, at various constant angular velocities. Dynamic properties of tOKN and static ocular cyclotorsion were analysed using videonystagmography. Patients’performances were compared with those of 10 healthy subjects. The results indicate that, in the acute stage after UVN, patients exhibited drastic impairment of tOKN velocity that depended on the direction of stimulation: tOKN velocity increased for ipsilesional stimulations and decreased for contralesional stimulations. These changes were responsible for a dramatic tOKN asymmetry, with ipsilesional directional preponderance of torsional slow-phase eye velocity. The changes were associated with static ocular cyclotorsion towards the operated side. Despite progressive compensation of tOKN deficits over time, tOKN velocity still differed from that recorded preoperatively, and tOKN asymmetry remained uncompensated 3 months after UVN. A static ocular cyclotorsion remained up to 3 months after lesion. These results are the first description of tOKN deficits and recovery after unilateral vestibular loss. They show that vestibular cues contribute to gaze stabilization during optokinetic stimulation around the line of sight. They also strongly suggest that tOKN impairment could be part of the longterm asymmetrical functions reported after unilateral loss of vestibular functions.

Aims: Catheter ablation of atrial fibrillation(AF) is centred on pulmonary vein(PV) ablation with or without additional atrial substrate modification. These procedures may be prolonged with significant fluoroscopy exposure. This study evaluates a new non-fluoroscopic navigation system during ablation of AF. methods and results: Seventy-two patients undergoing catheter ablation of symptomatic drug refractory AF were prospectively randomized to ablation with(n=35; study group) or without(n=37; control group) non-fluoroscopic navigation. PV isolation was performed in all patients. In patients with persistent or inducible sustained AF after PV isolation linear ablation was performed by joining the superior PVs. PV isolation was achieved in all patients; fluoroscopy(15.4± 3.4 vs. 21.3± 6.4 min; Pmin; P=0.02) durations were significantly reduced in the study group. Linear block was achieved in 37 of the 39 patients; with a significant reduction in fluoroscopy(5.6± 2.2 vs. 9.9± 4.8 min; P=0.003) and procedural(14.7± 5.5 vs. 26.6± 16.9 min; P=0.007) durations in the study group. After a follow-up of 6.9± 2.9 months(range 3-10), 26(74% ) patients in the non-fluoroscopic navigation group and 29(78% ) patients in the control group were arrhythmia-free after the first procedure. Conclusion: This prospectively randomized study demonstrates significant reduction of fluoroscopy exposure and procedural duration using supplementary non-fluoroscopic imaging system for AF ablation.

对比聚乙烯醇(PVA)膜,采取热重分析和差示扫描量热法研究了大豆蛋白/聚乙烯醇复合纤维(以下简称sp/PVA)的热学性能,并分析了聚乙烯醇及sp/PVA热性能的差异.不同sp/PVA测试结果表明大豆蛋白含量及线密度影响纤维的热性能.热重分析中,大豆蛋白含量多少影响失重过程长短,纤维线密度越低,热性能越好.差示扫描量热结果显示,sp/PVA的第一个峰为玻璃化转化区,第二个峰主要是由于聚乙烯醇的熔融引起.
