exosomes,small tiny vesicle contains a large number of intracellular particles that employ to cause various diseases and prevent several pathological events as well in the human *** is considered a“double-edged sword”,and depending on its biological source,the action of exosomes varies under physiological ***,the isolation and characterization of the exosomes should be performed accurately and the methodology also will vary depending on the exosome ***,the uptake of exosomes from the recipients’cells is a vital and initial step for all the physiological *** are different mechanisms present in the exosomes’cellular uptake to deliver their cargo to acceptor *** the exosomal uptake takes place,it releases the intracellular particles that leads to activate the physiological *** though exosomes have lavish functions,there are some challenges associated with every step of their preparation to bring potential therapeutic ***,overcoming the pitfalls would give a desired quantity of exosomes with high purity.

Gray-Hole Attack Minimization in IoMT with 5G Based D2D Networks

Reliable transmission is vital to the success of the next generation ofcommunications technologies and Fifth Generation (5G) networks. Many sensitive applications, such as eHealth and medical services, can benefit from a 5G network. The Internet of Medical Things (IoMT) is a new field that fosters themaintenance of trust among various IoMT Device to Device (D2D) modern technologies. In IoMT the medical devices have to be connected through a wirelessnetwork and constantly needs to be self-configured to provide consistent and effi-cient data transmission. The medical devices need to be connected with sophisticated protocols and architecture to handle the synergy of the monitoring ***, one of the commonly used algorithms in D2D communication is the Optimized Link State Routing protocol (OLSR). The OLSR is considerably good ateffectively utilizing the bandwidth and reserving the paths. One of the majorattack against the OLSR is the Node isolation attack, also known as the Gray holedenial of service attack. The Gray hole attack exploits the vulnerabilities presentwith sharing the topological information of the network. The attackers may usethis topological information to maliciously disconnect the target nodes from theexisting network and stops rendering the communication services to the victimnode. Hence, considering the sensitivity and security concerns of the data usedin e-Health applications, these types of attacks must be detected and disabledproactively. In this work, a novel Node Authentication (NA) with OLSR is proposed. The simulation experiments illustrated that the proposed protocol has anexcellent Packet Delivery Ratio, minimal end-end delay, and minimal Packet losswhen compared to the Ad-hoc On-Demand Distance Victor (AODV) protocol andthe proposed authentication scheme was able to protect the OLSR protocol from anode isolation attack.

A clinicoradiological classification and a treatment algorithm for traumatic triceps tendon avulsion in adults

CD4＋FoxP3＋ regulatory T cells （Tregs） are immunosuppressive cells that are critical for immune tolerance. Several studies have demonstrated that one of the anti-inflammatory mechanisms of action of intravenous immunoglobulin （Wig） involves the expansion of Tregs. Recently, we demonstrated that IVIgmediated Treg expansion involves the cyclooxygenase-2 （COX-2）-dependent induction of prostaglandin e2 （PGe2） in human dendritic cells （DCs）. However, the validity of these findings in autoimmune patients is lacking.

SCSBe: Secured Cluster and Sleep Based energy-efficient Sensory Data Collection with Mobile Sinks

Wireless sensor networks applications involve a position of inaccessible metropolitan vicinity en-closed by wireless sensor nodes (WSNs)-monitors environmental parameters like battle field surveillance, home applications like fire alarm, health monitoring, etc. energy plays a vital role in Wireless sensor networks. So, we have to concentrate more on balanced energy consumption for maximizing the network lifetime. Minimizing the whole network overhead and vigor disbursement coupled with the multi-hop data reclamation process that ensuring balanced energy consumption among SNs which results in prolonged network lifetime. This can be achieved by forwarding the sensed data to their cluster heads and then filtering the data before sending it to their tryst nodes, which is located in proximity to MS’s trajectory. Sleep and awakening of nodes periodically helps to retain their energy for some more time. The events occurring in any part of the network should be identified by the nodes, while arrangements sleep and active among the nodes. (i.e.) the nodes should be scheduled to sleep, so that the outstanding nodes can take care of the whole network. The eXtensible Randomized Matrix Arithmetic Coding (XRMAC) Technique has been used to enhance the security among all the nodes in the network. Simulation results show that our Proposed Scheme can have better Lifetime, improved throughput, reduced delay compared to other existing methods.

Nasopharyngeal colonization of otopathogens in South Indian children with acuteotitis media -- A case control pilot study

Background: Acute otitis media (AOM) is an inflammatory disease of the middle ear causing significantmorbidity in early childhood. A pilot study was undertaken to identify the role of various risk factorsSouth Indian children with AOM, especially the role of nasopharyngeal ***: A prospective case control pilot study was conducted in children aged below six years,presenting to a single tertiary care from 2018 to 2019. Fifty cases with AOM and 45 age and gendermatched controls were recruited. Two nasopharyngeal swabs were collected, one was processed forbacterial culture. The other swab was processed according to the CDC recommended broth enrichmentmethod to identify carriage of S. pneumoniae. Subsequent serotyping was done by Quellung method andconventional sequential multiplex ***: Otalgia was the major presentation seen in 92% of the children with AOM. None of the clinical anddemographic characteristics were found to be statistically significant between the cases and controls. Themost common otopathogen was S. pneumoniae (55%) followed by H. influenza (29%). The commonS. pneumoniae serotypes encountered were 11A and *** colonization with S. pneumoniae[OR 6.57, p enzae [OR14.18, p ere significant risk factors for AOM inchildren. The risk increased with co-colonization (OR 13.89,p engthens the significant association between nasopharyngeal colonization ofotopathogens and AOM as a risk factor that is enhanced by co-colonization.S. pneumoniae was the mainotopathogen in this population, serotypes 11A and 19F being the most common.

Distal femoral physeal crush injury with metaphyseal comminution - A report of two cases and a new perspective to physeal injury

The physis of a long bone may get ＇sandwiched＇ and crushed between the metaphysis and the epiphysis if it is traumatically loaded along its long axis. Such a physeal injury may lead to complications like angular deformities and growth restrictions and hence, management of such injuries requires adequate planning and attentive execution. Two patients with distal femoral physeal crush injury were treated using a ring fixator such that one ring had the wires passing through the epiphysis and the other through the femoral shaft. On table image intensifier controlled distraction of the crushed physis was done to bring the height of the physis similar to that of the opposite limb. Patients were followed up for more than two years clinically and radio- logically. There was no clinical or radiological angular deformity of the operated limbs. MRI scans showed intact physes with no physeal bar formation in either of the two patients. The distraction obtained by the ring fixator appears to have provided ample ＇breathing space＇ to the compressed physis and that the growth potential may have been re-gained by the procedure. However, two years is a relatively short duration of follow-up and further follow-up of longer duration and in greater number of patients is needed to gauge the actual effectiveness of the technque used bv us.
