Postoperative Complications and Patient＇s Satisfaction after Harvesting of the radial Artery for Coronary Artery Bypass

Objectives： There is a paucity of data regarding wound-site complications and patient＇s satisfaction after harvesting of a radial artery. This study is conducted to evaluate the frequency complications and the level of patient＇s satisfaction after myocardial revascularization in our setting. Methods： From April 2009 to October 2013, 97 patients had radial artery （ra） used as a graft in myocardial revascularization. The graft was harvested using open technique. This was retrospective study. Telephone questionnaire was used to evaluate： arm pain, swelling, mobility, sensory changes, patient＇s contentment with a cosmetic result and the general health state. Results： Pain of limited duration was reported by 24 patients （24.7%）, none of them reported permanent pain. Some problems in performing everyday＇s activities were reported by 8 pts （8.2%）. Sensory changes were permanently present in 5 pts （5.2%）, and frequent arm fatigue was reported by 4 pts （4.1%）. Positive opinion regarding the cosmetic result was reported by 95 pts （97.9%）. State of the health after surgery, 81 pts （83.5%） described as excellent or good. Patients who had more mobility problems, also had more sensory and neurological discomfort respectively. Patients with better self-reported general health state were also more satisfied with the esthetic effect of the intervention. Conclusions： Using the open technique resulted in excellent cosmetic effect. This gives us credit to point out that exclusive need of the endoscopic as a supreme method might be under the question mark. Our study suggests that individual attitude of the particular patient plays extremely important role in his/her overall satisfaction with the end effect of the procedure.

Insulin and Leptin resistance are the hallmarks for the development of type 2 diabetes mellitus (T2DM) and obesity. Both of these conditions are associated with increased activity and idiom of Protein Tyrosine Phosphatase 1B (PTP 1B). Therefore, inhibition of PTP 1B activity or down-regulation of its expression has become a striking area of therapeutics development in order to ameliorate the phenomenon of Insulin and Leptin resistance. Lack of awareness and late diagnosis makes diabetes difficult to manage. PTP 1B inhibitory activity by Erythroxylum monogynum plant extracts in the cytosolic supernatant of goat skeletal muscle and the liver was estimated. Erythroxylum monogynum plant extraction was done in hydroalcoholic solution (water–ethanol 20:80 (v/v)) and Methanol. The Polyphenols and protein content were identified in the hydroalcoholic and methanolic extracts. The greatest PTP 1B inhibitory activity in goat liver and skeletal muscle was found in the hydroalcoholic extract. Erythroxylum monogynum is a potent natural source of PTP 1B inhibition and may become cost-effective therapeutics in correcting metabolic disturbances of T2DM, obesity and cardiovascular disorders.

＜正＞A two-stage mixing approach（TSMA）was developed to improve the quality of recycled aggregate concrete（raC）.It has been established that this new method can produce concrete with strength of up to 70 MPa. This paper examines compressive strength of raC manufactured using ra from local centralized recycling plant in *** properties are tested and compared between ra and natural aggregate（NA）.The ra sample collected has a high water absorption rate of about 5.67%,while water absorption of NA is relating low of about 0.59%.Due to weak mortar surrounding ra,its aggregate crushing value（ACv）is about 42.8% larger than that of NA.A comparison with the strength data from concrete made of the same ingredients but produced using the standard mixing approach（$MA）confirms the superiority of the *** is conclusive that ra manufactures inferior concrete strength to NA concrete.A 100% replacement of NA with ra deteriorated the 28-day strength of concrete by up to 10.8% in compressive *** optimal raC mixture design would be about 20% of ra replacement,since it produced superior 28-day strengths compared to other raC designs in most concrete strength departments,and achieved these results for both the SMA and *** strength of raC can be improved by implementing the TSMA,with overall 28-day strength improvements of up to 4.3%.

Objective. To study the effects of two kinds of Chinese herbal medicine, radix angelicae sinensis(raS)(当归)and Shuanghuanglian(SHL)(双黄连) on chronic Pseudomonas aeruginosa(PA)lung infection in a rat model mimicking cystic fibrosis(CF). *** were divided into raS, SHL and control groups. All rats were challenged intratracheally with alginate embedded PA and the treatments with herbal medicine started on the same day of challenge. The drugs were administered subcutaneously once a day for ten days and the control group was treated with sterile *** rats were sacrificed two weeks after challenge. Results. Significantly improved lung bacterial clearance(Pral blood leukocytes(Pvel in serum (PraS treatment reduced fever(Pvel in serum(Pvity test in SHL was weakly positive whereas in raS it was negative. *** treatment with both herbal medicines could increase the resistance of the rats against PA lung infection and they therefore might be potential promising drugs for stimulation of the immune system in CF patients with chronic PA lung infection.

New Statistical Methods for Systematizing the Nuclei Fission Fragments: Post-Scission Approach

A new statistical method for systematizing the nuclei fission fragments, investigation of their mass and charge spectra and neutron fission parameters has been suggested. In proposed method, the mass and charge yields of nucleons are determined from the equilibrium conditions of the nuclear fragments post-fission ensemble. Here we consider protons and neutrons of different fragments as statistically non-equivalent. The abilities of the proposed statistical method for explanation of the U, Pa, Th, Ac and ra post-fission fragments ordering and neutron emission function calculation have been demonstrated.

Experimentation on Tool Wear and Surface Roughness in AISI D2 Steel Turning with WC Insert

The Taguchi method, based on an orthogonal arrangement (L9, 33), the vari-ance analysis, the signal-to-noise ratios and the response surface methodol-ogy have been used to optimize maximum flank wear (vBmax) and surface roughness (ra) of the cutting tool when turning a hardened steel AISI D2 (65 HRC) with PvD—TiAlN coated WC insert upon dry environment. By em-ploying regression models;cutting speed, cutting depth and feed rate, which optimize maximum flank wear and surface roughness were validated. Results of relation signal-to-noise ratios, showed that with cutting speed of 200 m/min, cutting depth of 0.2 mm and feed rate of 0.20 mm/rev, ra is opti-mized. With cutting speed of 150 m/min, cutting depth of 0.4 mm and feed rate of 0.3 mm/rev, vBmax is optimized. Through the variance analysis it was concluded that the depth of cut was the main parameter that affected on the surface roughness;whereas, the feed rate was the most influential parameter on the flank wear. Confirmation test results showed that the Taguchi method was very successful in the optimization of machining parameters for mini-mum surface roughness and flank wear in the turning of the D2 steel.

Investigation of Surface Free Energy for PTFE Polymer by Bipolar Argon Plasma Treatment

The low ion energy argon plasma was used for surface modification of Poly tetra fluoroethylene (PTFE) polymer. The plasma was generated between two plane metal electrode by using 50 kHz bipolar power supply. The plasma treated surface was characterized by atomic force microscopy (AFM) and X-ray photoelectron spectroscopy (XPS). The surface free energy (SFE) of plasma treated surface was calculated from contact angle measurement. SFE increases from 33.39 mJ/m2 to 41.40 mJ/m2 with the increase in plasma treatment time and the corresponding contact angle changed from 76? to 60?. XPS study shows that F/C ratio change from 1.8 (untreated) to 1.3 (treated) and O/C ratio changes from 0.094 (untreated) to 0.148 (treated). The XPS analysis shows that both F1s and the C1s spectra for PTFE are marginally modified by plasma treatment. AFM study shows that the average surface roughness (ra) increased from 8.5 nm to 22.8 nm after plasma treatment. vicker’s micro hardness of the film increases upon plasma treatment. The increase in SFE after plasma treatment is attributed to the functionalization of the polymer surface with hydrophilic groups as supported from the above observations.

To compare the subjective visual experience of cataract patients during phacoe mulsification and intraocular lens implantation under topical anesthesia (TA) vs retrobulbar anesthesia (ra). Prospective, randomized, controlled trial. Three h undred six cataract patients eligible for phacoemulsification and intraocular le ns implantation were randomized to receive either TA or ra during surgery by one of three surgeons. The surgeons were familiar with both anesthetic techniques a nd operated on the patients using the technique to which the patients were rando mized. A masked interviewer conducted in person interviews with the patients us ing a standardized questionnaire about their intraoperative visual experience an d their reaction to their visual experience between 30 minutes and 4 hours after the surgery. Two patients (one in each group) had intraoperative posterior caps ule rupture and were excluded from analysis. There was no statistically signific ant difference between TA (n=154) and ra (n=150) groups, except that more males compared with females had TA (P=. 03). More patients who had TA reported percept ion of light (P vements, flashes, instruments, or the surgeon or medical staff during the o peration and who found their visual sensations frightening in the two groups. Ho wever, 10.4%in the TA group and 9.3%in the ra group found their visual experie nce frightening. More patients undergoing cataract surgery under TA compared wit h ra reported perception of light, colors, and a change in light brightness. A s ignificant proportion of patients in both groups found the visual experience fri ghtening.

电热流体力学是流体力学、电磁学和传热学交叉学科,涉及电场、温度场以及流场之间的多物理场耦合作用。本文从经典的两无限大平行平板间的rayleigh-Bénard对流出发,通过数值模拟方法研究了电荷传导机制下考虑电场强化解离的昂萨格-韦恩效应(Onsager-Wien effect)的电热对流问题,重点研究不同参数对流动启动和分叉规律的影响。数值方法基于开源有限体积算法框架OpenFOAM实现,并通过一系列验证算例验证了数值方法的可靠性。本文特别考虑了随温度线性变化的离子迁移率下的电热对流流动,重点分析了传导数Co对流动临界瑞利数ra的影响以及努塞尔数Nu随瑞利数ra的变化关系。
