Despite the gradual professionalization of the construction sector as well as the abundance of sand mining sites offered by the North Kivu, Democratic Republic of Congo Region, ignorance of materials by local builders persists. This is the case of quarries extracting river sand used to make concrete and mortar. However, the dosages of the various constituents are most often chosen on the basis of experience without any prior characterization of this material. This paper presents a comprehensive review of the characterization of river sand for its use in concrete in DRC. The origin and global use of river sand in construction are presented in percentage terms to highlight the importance of river sand as a construction material. The physical properties of river sand, including particle size distribution, bulk density, absolute density, and cleanliness are discussed in detail. The paper examines the effect of variations in river sand properties on concrete behavior, including density and compressive strength. Overall, this paper emphasizes the need to properly characterize river sand before using it in construction to ensure durable, high-quality structures. This will avoid the problems that are observed in particular a bad behavior of the coating on the walls;cracks and crumbling of the beams, lintels, posts and even the ruin of the structures.

Xenobiotic effects in cancer related pathways-high throughput screening and proof of concept in animal models

Xenobiotic drugs and chemicals directly interact with DNA,proteins,or other biomolecules in cells. These direct interactions with molecular targets may trigger a series of downstream effects on metabolic-biochemical and regulatory-signaling networks that can invoke cellular consequences leading to adaptive homeostatic or adverse pathological responses. Regulators for drug and chemicals safety have therefore since long required the testing of toxicity in animal models before drugs and pesticides can enter the market. The US National Research Council of the National Academy of Sciences,in its report,Toxicity Testing in the 21st Century: a Vision and a Strategy [1] ,proposed that toxicity testing should become less reliant on apical endpoints from whole animal tests and eventually rely instead on quantitative,doseresponse models based on information from in vitro assays and in vivo biomarkers,which can be used to screen large numbers of chemicals. The present paper reports about a combination of HTS in vitro assays that can be used to study the potential tumorigenic effect of xenobiotics ( drug targets,environmental chemicals) via a set of"sentinel"genes [2] that are functionally interrelated based on evidence weighted functional linkage network ( FLN ) log-likelihood scores ( Linghu et al [3] ) .

Broadband mid-infrared light sources are highly desired for wide-ranging applications that span free-space communications to *** recent years,silicon has attracted great interest as a platform for nonlinear optical wavelength conversion in this region,owing to its low losses(linear and nonlinear)and high ***,most research in this area has made use of small core waveguides fabricated from silicon-on-insulator platforms,which suffer from high absorption losses of the use of silica cladding,limiting their ability to generate light beyond 3μ***,we design and demonstrate a compact silicon core,silica-clad waveguide platform that has low losses across the entire silicon transparency *** waveguides are fabricated from a silicon core fibre that is tapered to engineer mode properties to ensure efficient nonlinear propagation in the core with minimal interaction of the mid-infrared light with the *** waveguides exhibit many of the benefits of fibre platforms,such as a high coupling efficiency and power handling capability,allowing for the generation of mid-infrared supercontinuum spectra with high brightness and coherence spanning almost two octaves(1.6-5.3μm).

Visiting Patients at Home: Reasons and Efficiency of Dental House Calls

This retrospective study analyzes indications for house calls by dentists in a domiciliary setting. Its database consists in 69 visits made by a dentist office in Munich from 1998 to 2013 at the homes of 29 non-institutionalized seniors (≥62 years old, 15 females, 14 males, average age 83.7 years old). The analysis focused on the patients’ general state of health, their dental and oral health, the kinds of dental treatments administered, as well as economic aspects and the quantitative development in the frequency of house calls during the study period. The most frequent pre-existing conditions that made a house call necessary were cardiovascular illnesses (60%) and dementia diseases (36%). Multi-morbidity, dependency on care, accessibility issues, and frailty because of advanced age played a central role. The patients’ oral health was acceptable, with the exception of those with dementia diseases, who were more frequently toothless (40% versus 5.6%) and had a higher percentage of carious and decayed teeth (26.3% versus 10.3%). The most frequent procedures were treatments for denture sores and the repair of removable dentures (53.6%). The main focus was on reparative dentistry. The average house call took 37 minutes including travel time. Thus, the time needed was a multiple of the time necessary for treatment in the dental office. The lack of economic efficiency may be the reason why many dentists in Germany hesitate to make house calls. Despite the introduction of additional fees for the treatment of patients in need of long-term care on April 1, 2013, the number of house calls is low and increases only slowly. At the same time, the need for house calls will rise in the future due to an increasing proportion of elderly persons in the population and a decline in the percentage of toothless elderly.

Battle against Aging and Folds: Benefit and Risks of the Semi-Permanent Fillers Polylactic Acid and Calcium Hydroxylapatite

Background: Semi-permanent fillers are among the most favorable fillers on the market. Through their unique mode of action and its associated lasting aesthetic effect, they take an exceptional position. Objective: To compare the two semi-permanent fillers Poly-L-Lactic Acid (PLLA) and calcium hydroxylapatite (CaHA) in reference to the aesthetic result, patient satisfaction and side effects. Methods: Studies on side effects, patient satisfaction and aesthetic results after augmentation with semi-permanent fillers were analyzed. Results: Semi-permanent fillers seem excellently suited for the augmentation of very deep wrinkles particularly in the lower half of the face. In general, high patient satisfaction can be determined with both fillers. Here, the effect from the polylactic acid can be verified for up to two years while no effect could be verified already after one year in a majority of the patients augmented with CaHA. Short-term side effects such as bleedings or erythema in the region of the augmented area have been observed in both fillers during augmentation. The incidence of nodules and granulomas seems significantly higher in augmentations with PLLA compared to CaHA. Rare side effects such as an embolization of a blood vessel caused by the implant have been described for both fillers in case reports. Conclusion: Semi-permanent fillers are superbly suited for wrinkle augmentation. Which filler is the preferred one in what case depends strongly on the individual needs of the patient and the therapist’s experience.

Methylphenidate Misuse in Adults: Survey of 414 Primary Care Physicians in Germany and Comparison with the Literature

Objectives: This paper deals with the methylphenidate (MPH) misuse by adults in Germany. Results of a survey among primary care physicians/internists have been supplemented by a comparison with the literature. Methods: In the period from October 5-20, 2015, a survey was sent to 414 primary care physicians/internists in four German cities (n = 10 were undeliverable). The response rate was 58% (n = 235). 34 original works on MPH abuse worldwide were found in the literature and are used in the analysis of the present data situation. The literature published before November 9, 2015 is considered in this paper. Results: 14% of the doctors who took part in the survey said that they had been asked for MPH prescriptions without any medical indication. The most frequent reason given for the request (42%) was adult attention deficit/hyperactivity disorder (ADHD) not verified by documents. According to the comparison with the literature, university students had a lifetime prevalence of MPH misuse ranging from 0.8% to 16.6% and school children had a misuse rate of 4.0%. In the civilian US population, the misuse rate was 4.2%. Among patients in possession of a current MPH prescription due to a diagnosis, the lifetime prevalence was 29% and among adolescents with suspected alcohol and/or drug problems 20%. Conclusions: MPH misuse is a major problem which has not been studied sufficiently. MPH misuse not only plays a role in the field of psychiatry, but also in other disciplines. Misuse particularly following a therapeutic prescription should be taken into account.

Indolent B-cell lymphomas are a group of incurable cancers of the elderly that encompass various entities,such as chronic lymphocytic leukemia(CLL),follicular lymphoma(FL),and marginal zone lymphoma(MZL).All indolent lymphomas may evolve towards aggressive transformation with an increased proliferation index and decreased tumor doubling *** transformation,which is called Richter’s syndrome in CLL,is associated with an aggressive clinical course and poor *** least in CLL,transformation of an indolent B-cell clone is molecularly different from that of de novo diffuse large B-cell lymphomas.1 Aging is known to be associated with immune decline,with increased inflammation,decreased immune surveillance and increased onset of malignancies among the consequences.2 Progression of indolent B-cell lymphomas is likely to be associated with escape from immune surveillance.
