A prospective randomized study to define the role of low dose continuous prophylactic antibiotics and anti-adherence agents in altering the microbial colonization related to indwelling double‐J stents

Objective:Despite conflicting evidence,it is common practice to use continuous antibiotic prophylaxis(CAP)in patients with indwelling double-J(DJ)*** extracts and d-mannose have been shown to prevent colonization of the urinary *** evaluated their role in this ***:We conducted a prospective randomized study to evaluate patients with indwelling DJ stents following urological *** were randomized into three *** A(n=46)received CAP(nitrofurantoin 100 mg once daily[OD]).Group B(n=48)received cranberry extract 300 mg and d-mannose 600 mg twice daily(BD).Group C(n=40)received no *** stents were removed between 15 days and 45 days after *** groups were compared in terms of colonization of stent and urine,stent related symptoms and febrile urinary tract infections(UTIs)during the period of indwelling stent and until 1 week after ***:In Group A,9(19.5%)patients had significant bacterial growth on the *** was 8(16.7%)in the Group B and 5(12.5%)in Group C(p-value:0.743).However,the culture positivity rate of urine specimens showed a significant difference(p-value:0.023)with Group B showing least colonization of urine compared to groups A and *** was no statistically significant difference in the frequency of stent related symptoms(p-value:0.242)or febrile UTIs(p-value:0.399)among the ***:Prophylactic agents have no role in altering bacterial growth on temporary indwelling DJ stent,stent related symptoms or febrile *** extract may reduce the colonization of urinary tract,but its clinical significance needs further evaluation.

Clinicohistopathological implications of phosphoserine 9 glycogen synthase kinase-3β/β-catenin in urinary bladder cancer patients

BACKGROUND Aberrant activation of phosphorylated form of glycogen synthase kinase-3β[pS9 GSK-3β(Serine 9 phosphorylation)] is known to trigger Wnt/β-catenin signal cascade but its clinicohistopathological implications in bladder carcinogenesis remain *** To investigate the diagnostic and prognostic relevance of expressions of pS9 GSK-3β, β-catenin and its target genes in the pathobiology of bladder *** Bladder tumor tissues from ninety patients were analyzed for quantitative expression and cellular localization of pS9 GSK-3β by immunohistochemical(IHC) staining. Real time-quantitative polymerase chain reaction and IHC were done to check the expression of β-catenin, Cyclin D1, Snail and Slug at transcriptome and protein level respectively. Clinicohistopathological variables were obtained from histology reports, follow up and OPD visits of *** of the markers were statistically correlated with these variables to determine their significance in clinical setting. Results were analysed using SPSS20.0 *** Aberrant(low or no membranous/high nuclear/high cytoplasmic) expression of pS9 GSK-3β was noted in 51% patients and found to be significantly associated with tumor stage and tumor grade(P = 0.01 and 0.04; Mann Whitney U test).Thirty one percent tumors exhibited aberrant co-expression of pS9 GSK-3β andβ–catenin proteins and showed strong statistical association with tumor stage,tumor type, smoking/tobacco chewing status(P = 0.01, 0.02 and 0.04, MannWhitney U test) and shorter overall survival probabilities of patients(P = 0.02;Kaplan Meier test). Nuclear immunostaining of Cyclin D1 in tumors with altered pS9 GSK-3β/β–catenin showed relevance with tumor stage, grade and ***β–catenin and pS9 GSK-3β proteins are identified as markers of diagnostic/prognostic significance in disease pathogenesis. 下一代电子产品的飞速发展对热管理提出了更高的要求。初始石墨烯的导热性是铜的13倍。本文通过快速热处理化学气相沉积(RTP-CVD)法制备了具有大sp^(2)结构域的单层、双层和多层石墨烯(SLG、BLG、FLG),进一步通过低浓度H_(2)还原制备了高导热石墨烯。在1000℃下生长25 min制备出SLG,利用拉曼光谱和透射电子显微镜(TEM)研究了石墨烯的品质。为了验证RTP-CVD法生成的石墨烯的散热能力,将其作为2TB固态硬盘的散热器,通过红外热成像仪进行了研究。结果证明,RTP-CVD生长的石墨烯用于消费电子产品的热管理测试时性能表现优异。SLG显示温度(最高)比商用铜散热器低5℃,SLG的散热能力比商用铜散热器快200倍左右。综上,利用RTP-CVD法制备的轻质的柔性石墨烯可以成为下一代5G设备和消费电子产品热管理的更好选择。

Pathogenesis of achalasia cardia

Achalasia cardia is one of the common causes of motor *** the disease was first described more than 300 years ago,exact pathogenesis of this condition still remains ***,achalasia cardia is caused by loss of inhibitory ganglion in the myenteric plexus of the *** the initial stage,degeneration of inhibitory nerves in the esophagus results in unopposed action of excitatory neurotransmitters such as acetylcholine,resulting in high amplitude non-peristaltic contractions(vigorous achalasia);progressive loss of cholinergic neurons over time results in dilation and low amplitude simultaneous contractions in the esophageal body(classic achalasia).Since the initial description,several studies have attempted to explore initiating agents that may cause the disease,such as viral infection,other environmental factors,autoimmunity,and genetic *** Chagas disease,which mimics achalasia,is caused by an infective agent,available evidence suggests that infection may not be an independent cause of primary achalasia.A genetic basis for achalasia is supported by reports showing occurrence of disease in monozygotic twins,siblings and other first-degree relatives and occurrence in association with other genetic diseases such as Down's syndrome and Parkinson's *** in genes encoding for nitric oxide synthase,receptors for vasoactive intestinal peptide,interleukin 23 and the ALADIN gene have been ***,studies on larger numbers of patients and controls from different ethnic groups are needed before definite conclusions can be ***,the disease is believed to be multi-factorial,with autoimmune mechanisms triggered by infection in a genetically predisposed individual leading to degeneration of inhibitory ganglia in the wall of the esophagus.

Geomorphometric Characterization of Upper South Koel Basin, Jharkhand: A Remote Sensing &GIS Approach

The quantitative analysis of drainage system is an important aspect of characterization of watersheds. Morphometry is measurement and mathematical analysis of landforms. The present study is an attempt to evaluate the drainage morphometrics of Upper South Koel Basin using Remote Sensing and GIS approach. A morphometric analysis was carried out to describe the topography and drainage characteristics of Upper South Koel watershed. The stream numbers, orders, lengths and other morphometric parameters like bifurcation ratio, drainage density, stream frequency, shape parameters etc. were measured. The drainage area of Upper South Koel watershed is 942.4 sq km and the drainage pattern is dentritic. The watershed was classified as 6th order drainage basin. The low values of bifurcation ratio and drainage density suggest that the area has not been much affected by structural disturbances. The study reveals that the different geomorphic units in the study area i.e. Structural hills, Pediments, Valley fills, Pediplains formed under the influence of permeable geology, are moderate to nearly level plains, with medium to low drainage density (<2.0) & low cumulative length of higher order streams . Such studies can be of immense help in planning and management of river basins.

An Unusual Case of Right Transmesocolic Hernia with Ureter at the Neck of Sac

The purpose of the present article is to present an unusual case of internal herniation. In this case small bowl obstruction was due to congenital right trans-mesocolic hernia which had the right ureter at the neck of the sac forming a constriction band. This requires careful widening of neck of hernia, so as to avoid iatrogenic trauma to ureter. This unusual presentation needs dissemination as this type of internal hernia is extremely rare.

Safety of gastric lavage using nasogastric ryle’s tube in pesticide poisoning

Objective: Gastric lavage is mandatory irrespective of nature in all patients with acute poisoning in India. Present study was undertaken with aim whether lavage done using nasogastric Ryle’s tube and small aliquots of water or normal saline is safe. Patients and Methods: All the patients above 12 years of age admitted consecutively with pesticide ingestion or exposure between July 2004 to June 2005 were studied with respect to complications associated with lavage using Ryle’s tube. Forty five patients were admitted directly to our hospital and lavage was undertaken using Ryle’s tube (16F ) with 100 - 200 mL of aliquots till 1 - 1.5 liters of fluid was lavaged, with prophylactic endotracheal intubation in patients with Glasgow coma scale ( GCS) < 10 (group I). The incidence of complications related to lavage in group I was compared to that in 53 patients admitted during same period with pesticide poisoning but lavaged outside using nasogastric Ryle’s tube and referred to our institute (group II). Results: The significant complications observed in group I were significant drop in SaO2 (6 patients) laryngospasm, tachycardia, electrolyte imbalance and tube getting struck in throat (one each). In one patient in group I (had no prophylactic intubation though GCS 3) In group II, 7 had aspiration pneumonia (no prophylactic intubation). Other significant complication was drop in SaO2 during lavage. None of them had any serious life threatening complication. Conclusion: Gastric lavage carried out using nasogastric Ryle’s tube and small aliquots of water or normal saline is relatively safe in patients with pesticide poisoning when combined with prophylactic endotracheal intubation in patients with GCS < 10. In absence of prophylactic intubation, risk of aspiration is there. However aspiration pneumonia is generally mild and not life threatening.

Ovipositional preference and larval performance of poplar defoliator,Clostera restitura on different poplar clones in north-western India

We evaluated ten poplar clones （G-3, G-48, L-50/88, L- 154/84, L-156/89, S7C8, S7C15 WSL-22, WSL-29 and uday） for ovi- positional preference and larval performance of Clostera restitura. Fe- male moths did not show any preference with respect to clones for ovi- position. Significant differences were observed for number of eggs laid on different plant parts. C. restitura laid eggs in clusters, preferably on upper surface of leaf and size of the egg cluster varied from 15 to 167 eggs. Clones varied for their relative resistance and susceptibility to C. restitura. L-50/88; L-156/89 were identified as most resistant clone based on minimum leaf consumption, whereas S7C15 was found to be most susceptible clone to C. restitura. In multiple choice experiments, no feeding preference by C. restitura larvae was detected amongst different poplar clones. After initial settlement of larvae on a particular clone, the larvae remained confined to that clone and negligible inter-clonal move- ment was noticed subsequently. The fresh pupal weight was correlated negatively （r = -0.37） with percentage surface leaf area eaten and positively （r = 0.47） with length of larval period, measured on different clones. Relationship between percentage leaf area eaten and length of larval period was negative （r = -0.23）. Owing to relative resistance of L- 50/88 and L-156/89 against C. restitura, these clones can be recom- mended for plantation in defoliator prone areas in north-western India.
