Maternal Group B Streptococcus Infection Correlates Inversely With Preeclampsia in Pregnant African American Women

Objective:To determine whether an association exists between group B Streptococcus(GBS)colonization and preeclampsia among pregnant Black ***:This retrospective cross-sectional study involved Black women who gave birth at State University of New York Downstate Hospital between January 2010 and December *** were collected from the Obstetric Department,including delivery date,time,mode of delivery,age of the mother,weeks of gestation at delivery,and antepartum *** GBS test results were originally determined using the eSwab transport *** was defined based on the American College of Obstetricians and Gynecologists criteria for the periods 2010–2012 and 2013–*** primary outcome was whether GBS was associated with the outcome of preeclampsia in the population of Black ***,including smoking status,gestational age,parity,body mass index,maternal age,and presence of herpes simplex virus(HSV)and human immunodeficiency virus(HIV)were examined as potential ***-squared test and logistic regression model were used,presenting odds ratios with 95%confidence intervals(P<0.050),analyzed with SAS on Demand for Academics(SAS Institute,Inc.,NY).Results:Among the 8,019 Black women included in this study,GBS-positive women(n=977)had a 53%reduction in the likelihood of being diagnosed with preeclampsia compared to GBS-negative women(adjusted odds ratio,0.47;95%confidence interval,0.32–0.70).We did not find evidence of differences in the distribution of smoking habits(P=0.783)or maternal age(P=0.107)between GBS-positive and GBS-negative ***,the GBS-positive women tended to be less likely to have a preterm delivery(9.62%(94/977)vs.24.24%(1707/7042),P<0.001),less likely to be nulliparous(33.37%(326/977)vs.37.87%(2667/7042),P=0.006),and less likely to be obese(51.38%(502/977)vs.55.30%(3894/7042),P<0.001)compared with GBS-negative *** contrast,GBS-positive women were more likely to have a comorbid infecti

高山寺古钞本是唐五代一度流行的《庄子》抄本或其传抄本,在判定敦煌写本真伪及厘清其版本系统、梳理《庄子》文本演变、推求成玄英疏本原貌等方面具有重要价值。有学者怀疑钞本底本是成玄英疏原本,出自刘宋元嘉本,乃抄手只抄经注,节略疏文而成。通过异文分析、对比可知,钞本跟元嘉本、成疏本存在一些共同异文,但有更多异文相左,钞本并非元嘉本,更非成疏原本。成玄英疏本早期并非经注疏合抄本,没有确凿证据证明钞本有疏文窜入,抄手只抄经注,节略疏文的观点不能成立。钞本是吸收《经典释文·庄子音义》底本、或本、元嘉本、成疏本等众多郭注本异文而形成的新版本。钞本与几种敦煌写本高度一致,它们有共同的版本来源——唐五代时曾经通行的一个《庄子》写本。该本可能是玄宗朝形成的“奉诏”新校本,但后来亡佚。就钞本抄写和祖本形成时间来说,其上限在陆德明《经典释文》成书之后,下限在陈景元《庄子阙误》成书之前。

Glutamine-supplemented total parenteral nutrition attenuates plasma interleukin-6 in surgical patients with lower disease severity

AIM： To evaluate whether the effect of Gin dipeptideenriched total parenteral nutrition （TPN） on postoperative cytokine alteration depended on the disease severity of surgical patients. METHODS： Forty-eight patients with major abdominal surgery were allocated to two groups to receive isonitrogenous （0.228 g nitrogen/kg per d） and isocaloric （30 kcal/kg per d） TPN for 6 d. Control group （Cony） using conventional TPN solution received 1.5 g amino acids/kg per day, whereas the test group received 0.972 g amino acids/kg per day and 0.417 g L-alanyI-L-glutamine （Ala-GIn）/kg per day. Blood samples were collected on d 1 and d 6 postoperatively for plasma interleukin （IL）-2, IL-6, IL-8, and interferon （IFN）-γ analysis. RESULTS： Plasma IL-2 and IFN-γ were not detectable. IL-6 concentrations were significantly lower on the 6^th postoperative day in the Ala-GIn group than those in the Cony group in patients with APACHE Ⅱ≤6, whereas no difference was noted in patients with APACHE Ⅱ〉6. There was no difference in IL-8 levels between the two groups. No difference in cumulative nitrogen balance was observed on d 2-5 after the operation between the two groups （Ala-GIn -3.2±1.6 g vs Cony -6.5±2.7 g）. A significant inverse correlation was noted between plasma IL-6 levels and cumulative nitrogen balance postoperatively in the Ala-GIn group, whereas no such correlation was observed in the Conv group. CONCLUSION： TPN supplemented with Gin dipeptide had no effect on plasma IL-8 levels after surgery. However, Gin supplementation had a beneficial effect on decreasing systemic IL-6 production after surgery in patients with low admission illness severity, and lower plasma IL-6 may improve nitrogen balance in patients with abdominal surgery when Gin was administered.

The title compound rac-11-t-butoxy-l,4-dihydro-l,4-methanoanthracene （C19H200, Mr = 264.35） has been synthesized and characterized by FT-IR, 1H NMR, HRMS spectra and single-crystal X-ray diffraction. The crystal belongs to monoclinic, space group P21/n, with a = 13.5240（10）, b = 8.3453（6）, c = 13.9604（9） A,β = 100.0190（10）°. The structure of the title compound comprises a norbornene unit with a t-butoxy g, roup, having a naphthalene ring fused on one side. The naphthalene is essentially planar with a maximum deviation of 0.032（2） A for atom C（3）. In the crystal, inversion-related molecules are linked by pairs of C- H...O hydrogen bonds, forming a cyclic dimer with R2z （16） graph-set motif. The C-H-..~r interactions are also observed, linking the molecules into a continuous two-dimensional framework structure.

AIM: Malignant gastrointestinal stromal tumors (GISTs)are rare. Tumors larger than 10 cm tend to recur earlier:the larger the volume of the tumor, the worse the *** hypothesized that treatment with imatinib mesylate (Gleevec; STI-571), a c-kittyrosine kinase inhibitor, as palliative therapy would prolong the survival of patients with recurrent giant malignant GISTs after ***: We performed a retrospective analysis of the effects of resection on patients with giant GISTs (＞10 cm in diameter) to determine the overall survival and recurrence rates. Twenty-three patients diagnosed with giant GISTs were included from June 1996 to December 2003. STI571 was not available until January 2000. After that time,9 patients received this drug. The factors of age, sex, tumor location, histological surgical margin, and STI-571, tumor size changes and drug side effects were reviewed. We compared the survival rate to determine the prognostic factors and the effects of STI-571 on patients with recurrent malignant gastrointestinal stromal ***: The positive surgical margin group had a significantly higher recurrence rate than the negative margin group (P = 0.012). A negative surgical margin and palliative treatment with STI-571 were significant prognostic variables (Log-rank test,P＜0.05). Age, sex and tumor location were not significant prognostic variables. The 5-year survival rate of the surgical margin free patients was 80%and the 2-year survival rate of the surgical margin positive patients was 28%. The 5-year survival rate was 80% for the patients given STI-571 and 30% for the patients not given STI-571. The use of STI-571 gave a significant tumor shrinkage (6/9) rate in patients with giant GIST recurrence after ***: A negative surgical margin and the use of STI-571 after surgical resection were good prognostic indicators.
