Difficulties in Recruitment for a Randomised Controlled Trial of Lifestyle Intervention for Type 2 Diabetes: Implications for Diabetes Management

Obj.ctive: To report our experience of attempting a randomised controlled trial of an intensive lifestyle intervention for early type 2 diabetes delivered in a residential setting. Methods: We established a trial requiring 84 participants (46 standard care and 38 intervention) to detect a 1% difference in HbA1c between intervention and control groups at 12 months, allowing for attrition. Ethics approval was obtained from Monash University. Results: The study was abandoned after five months of consistent promotion due to recruitment failure (four subj.cts recruited). Conclusion: It appears to be difficult for patients with diabetes to commit to a live-in period of education regarding lifestyle modification as a means of treating the illness. We recommend better education of patients and their doctors about the potential health benefits of lifestyle change to manage type 2 diabetes, and further research into novel methods of delivering lifestyle advice which are both effective and sustainable.

Competition and body size in plants:the between-species tradeoff for maximum potential versus minimum reproductive threshold size

Aims According to traditional theory,superior competitive ability in plants generally requires relatively large plant body *** even within the most crowded vegetation,most resident species are relatively small;species size distributions are right-skewed at virtually every *** examine a potential explanation for this paradox:small species coexist with and outnumber large species because they have greater‘reproductive economy’,*** are better equipped—and hence more likely—to produce offspring despite severe size suppression from intense *** Randomly placed plots within old-field vegetation were surveyed across the growing *** each plot,the largest(MAX)and smallest(MIN)reproductive individuals of each resident species were collected for above-ground dry mass *** tested three hypotheses:(i)smaller resident species(with smaller MAX size)have generally smaller reproductive threshold sizes;(ii)smaller resident species have greater‘reproductive economy’,i.e.a smaller MIN relative to MAX reproductive plant size;and(iii)MIN size predicts plot occupancy(species abundance within the community)better than MAX *** Findings The results supported the first and third,but not the second ***,we could not rej.ct the hypothesis that smaller species have greater reproductive economy,as it was not possible to record data for the largest potential plant size for each species—since even the largest(MAX)plants collected from our sampled plots were subj.cted to competition from neighbours under these natural field ***,contrary to conventional competition theory,more successful species(in terms of greater plot occupancy)had smaller minimum not larger(or smaller)maximum reproductive *** results suggest that a small reproductive threshold size,commonly associated with relatively small potential body size,is generally more effective in transmitting genes into future generations when selection

Informal Caregiving and Disaster Risk Reduction:A Scoping Review

Informal caregivers are a population currently in the shadows of disaster risk reduction(DRR),and yet essential to the provision of healthcare *** scoping review explored the literature to understand issues related to informal caregiving and promising practices to support resilience for *** guidelines for scoping review as outlined by Tricco et al.(2016),relevant publications were identified from five maj.r databases—Medline,Embase,PubMed,Web of Science,and *** studies referenced informal caregiving and disasters for a variety of population groups including children,people with disabilities or chronic illnesses,and older *** were excluded if they discussed formal caregiving services(for example,nursing),lacked relevance to disasters,or had insufficient discussion of informal ***,21 articles met the inclusion criteria and were fully *** themes were identified:(1)the need for education and training in DRR;(2)stressors around medication and supply issues;(3)factors affecting the decision-making process in a disaster;(4)barriers leading to disaster-related problems;and(5)factors promoting *** areas of strategic action and knowledge gaps are *** informal caregivers do not feel adequately prepared for *** the important role of informal caregivers in healthcare provision,preparedness strategies are essential to support community resilience for those requiring personal care *** understanding and mobilizing assets to support the resilience of informal caregivers,we also support the resilience of the greater healthcare system and the community,in disaster contexts.

High mobility group box 1(HMGB1)is an evolutionarily conserved non-histone chromatin-binding *** infection or inj.ry,activated immune cells and damaged cells release HMGB1 into the extracellular space,where HMGB1 functions as a proinflammatory mediator and contributes importantly to the pathogenesis of inflammatory *** studies reveal that infl ammasomes,intracellular protein complexes,critically regulate HMGB1 release from activated immune cells in response to a variety of exogenous and endogenous danger *** stranded RNA dependent kinase(PKR),an intracellular danger-sensing molecule,physically interacts with inflammasome components and is important for infl ammasome activation and HMGB1 ***,these studies not only unravel novel mechanisms of HMGB1 release during infl ammation,but also provide potential therapeutic targets to treat HMGB1-related infl ammatory diseases.

Characterization of inflammation and insulin resistance in high-fat diet-induced male C57BL/6j.mouse model of obesity

Background: Animal models of diet-induced obesity(DIO) are commonly used in medical research for mimicking human diseases. There is no universal animal model, and careful evaluation of variety of factors needs to be considered when designing new experiments. Here, we investigated the effect of 9 weeks high-fat diet(HFD) intervention, providing 60% energy from fat, on parameters of inflammation and insulin resistance in male C57 BL/6 j.***: Six weeks old mice were initiated on regular diet(RD) or HFD providing 60 kcal energy from fat for 9 weeks. Fasting blood glucose levels were measured by glucometer, and fasting plasma levels of insulin and proinflammatory cytokines by Luminex assay. Insulin sensitivity was evaluated by using QUICKI and HOMA2 ***: HFD mice showed ~ 40% higher body weight and ~ 20% larger abdominal circumference, due to an increase in the white adipose tissue mass. Liver examination revealed increased size and higher hepatic lipid accumulation in livers from HFD mice compared to their RD counterparts. Animals from the HFD group were characterized with significantly higher presence of crown-like structures(CLS) in WAT and higher plasma levels of proinflammatory cytokines(TNF-α, IL-6, leptin, MCP-1, PAI-1, and resistin). HFD-fed mice also demonstrated impaired insulin sensitivity(lower QUICKI, higher HOMA-insulin resistance(HOMA-IR), and lower HOMA-percent sensitivity(HOMA-%S)) index ***: Male C57 BL/6 j.mice on 9 weeks HFD providing 60 kcal energy from fat display impaired insulin sensitivity and chronic inflammation, thus making this DIO mouse model appropriate for studies of early stages of obesity-related pathology.

P2X7 receptor activation causes phosphatidylserine exposure in canine erythrocytes

AIM To determine if activation of the ATP-gated P2X7 receptor channel induces phosphatidylserine(PS) exposure in erythrocytes from multiple dog *** Peripheral blood was collected from 25 dogs representing 13 pedigrees and seven crossbreeds. ATP-induced PS exposure on canine erythrocytes in vitro was assessed using a flow cytometric Annexin V binding *** ATP induced PS exposure in erythrocytes from all dogs studied. ATP caused PS exposure in a concentrationdependent manner with an EC50 value of 395 μmol/L. The non-P2X7 agonists, ADP or AMP, did not cause PS exposure. The P2X7 antagonist, AZ10606120, but not the P2X1 antagonist, NF449, blocked ATP-induced PS *** The results indicate that ATP induces PS exposure in erythrocytes from various dog breeds and that this process is mediated by P2X7 activation.

Striatins and STRIPAK complex partners in clinical outcomes of patients with breast cancer and responses to drug treatment

Obj.ctive: Striatins(STRNs) family, which contains three multi-domain scaffolding proteins, are cornerstones of the striatins interacting phosphatase and kinase(STRIPAK) complex. Although the role of the STRIPAK complex in cancer has become recognized in recent years, its clinical significance in breast cancer has not been fully ***: Using a freshly frozen breast cancer tissue cohort containing both cancerous and adj.cent normal mammary tissues, we quantitatively evaluated the transcript-level expression of all members within the STRIPAK complex along with some key interacting and regulatory proteins of STRNs. The expression profile of each molecule and the integrated pattern of the complex members were assessed against the clinical-pathological factors of the patients. The Cancer Genome Atlas(TCGA) dataset was used to evaluate the breast cancer patients’ response to chemotherapies. Four human breast cancer cell lines, MDA-MB-231, MDA-MB-361, MCF-7, and SKBR-3, were subsequently adopted for in vitro ***: Here we found that high-level expressions of STRIP2, calmodulin, CCM3, MINK1 and SLMAP were respectively associated with shorter overall survival(OS) of patients. Although the similar pattern observed for STRN3, STRN4 and a contrary pattern observed for PPP2CA, PPP2CB and PPPR1A were not significant, the integrated expression profile of STRNs group and PPP2 group members constitutes a highly significant prognostic indicator for OS [P<0.001, hazard ratio(HR)=2.04, 95% confidence interval(95% CI), 1.36-3.07] and disease-free survival(DFS)(P=0.003, HR=1.40, 95% CI, 1.12-1.75). Reduced expression of STRN3 has an influence on the biological functions including adhesiveness and migration. In line with our clinical findings, the breast cancer cells responded to STRN3 knockdown with changes in their chemo-sensitivity, of which the response is also breast cancer subtype ***: Our results suggest a possible role of the STRIPAK complex
