Clinical and molecular characteristics of East Asian patients with von Hippel–Lindau syndrome

Background: Von Hippel–Lindau(VHL) syndrome is a dominantly inherited multisystem cancer syndrome caused by a heterozygous mutation in the VHL tumor suppressor gene. Previous studies suggested that similar populations of Caucasian and Japanese patients have similar genotype or phenotype characteristics. In this comprehensive study of East Asian patients, we investigated the genetic and clinical characteristics of patients with VHL ***: To create a registry of clinical characteristics and mutations reported in East Asian patients with VHL syndrome, we conducted a comprehensive review of English language and non?English language articles identi?fied through a literature search. Publications in Japanese or Chinese language were read by native speakers of the language, who then performed the data ***: Of 237 East Asian patients with VHL syndrome, 154 unique kindreds were identified for analysis. Analyzed by kindred, missense mutations were the most common(40.9%, 63/154), followed by large/complete deletions(32.5%, 50/154) and nonsense mutations(11.7%, 18/154). Compared with a previously reported study of both East Asian and non?East Asian patients, we found several key differences. First, missense and frameshift mutations in the VHL gene occurred less commonly in our population of East Asian patients(40.9% vs. 52.0%; P = 0.012 and 8.4% vs. 13.0%; P < 0.001, respectively). Second, large/complete deletions were more common in our population of East Asian patients(32.5% vs. 10.5%; P < 0.001). Third, phenotypically, we observed that, in our population of East Asian patients with VHL syndrome, the incidence of retinal capillary hemangioblastoma was lower, whereas the incidence of renal cell carcinoma was ***: Evidence suggests that the genotypic and phenotypic characteristics of East Asian patients with VHL syndrome differ from other populations. Nasal metastases from renal cell carcinoma are associated with Memorial Sloan-Kettering Cancer Center poor-prognosis classification

Unusual sites of metastases are recognized in patients with renal cell carcinoma (RCC). However, the prognostic implications of these sites are not well understood. We used the Memorial Sloan-Kettering Cancer Center (MSKCC) risk classification for metastatic RCC to evaluate 912 consecutive patients with RCC managed at the Singapore General Hospital between 1990 and 2009. Among these patients, 301 had metastases either at diagnosis or during the course of illness. Nasal metastases, all arising from clear cell RCC, were identified histologically in 4 patients (1.3% of those with metastasis). All 4 patients were classified as MSKCC poor prognosis by current risk criteria. Nasal metastases were significantly associated with lung and bone metastases. The frequency of nasal metastases in patients with metastatic RCC is about 1%, occurring predominantly in patients with clear cell RCC. Nasal metastases are associated with poor prognosis as estimated by the MSKCC risk classification, with attendant implications for selection of targeted therapy, and are usually associated with multi-organ dissemination, including concurrent lung and bone involvement.

Evaluating Nebraska’s local comprehensive plans to achieve the national wetland conservation missions in the USA

The *** government’s requirements and guidance for wetland conservation are reflected in a series of federal laws,regulations,and ***,the degree to which these requirements are reflected in local planning efforts is *** study using five components with 56 indicators evaluates how well wetland conservation efforts are integrated into 112 local comprehensive plans in Nebraska,*** results show that the majority of local comprehensive plans pay little direct attention to wetland conservation,although many conservation efforts are conducted under the umbrella of environmental protection *** indicators include water resource protection received the highest score and was the descriptor of natural or environmental resources and resulted in the highest level of local awareness on natural *** indicator of setting goals for no net loss of wetlands received the lowest score among all indicators,demonstrating a clear gap between the national vision and the local reality of wetland *** suggest local governments need more direct and proactive inputs to improve wetland *** findings from this research provide a practical roadmap for planners globally,particularly for the developing countries,to integrate wetland conservation into the local planning systems.

Fabrication of all-transparent polymer-based and encapsulated nanofluidic devices using nano-indentation lithography

In this paper,we describe a novel and simple process for the fabrication of all-transparent and encapsulated polymeric nanofluidic devices using nano-indentation ***,a nanomechanical probe is used to‘scratch’nanoscale channels on polymethylmethacrylate(PMMA)substrates with sufficiently high ***,polydimethylsiloxane(PDMS)is used twice to duplicate the nanochannels onto PDMS substrates from the‘nano-scratched’PMMA substrates.A number of experiments are conducted to explore the relationships between the nano-indentation parameters and the nanochannel dimensions and to control the aspect ratio of the fabricated *** addition,traditional photolithography combined with soft lithography is employed to fabricate microchannels on another PDMS‘cap’*** manually aligning the substrates,all uncovered channels on two separate PDMS substrates are bonded to achieve a sealed and transparent nanofluidic device,which makes the dimensional transition from microscale to nanoscale *** smallest dimensions of the achievable nanochannels that we have demonstrated thus far are of~20 nm depth and~800 nm width,with lengths extendable beyond 100μ*** flow experiments are performed to verify the reliability of the *** types of colloidal solution are used to visualize the fluid flow through the nanochannels,that is,ethanol is mixed with gold colloid or fluorescent dye(fluorescein isothiocyanate),and the flow rate and filling time of liquid in the nanochannels are estimated based on time-lapsed image *** simplicity of the fabrication process,bio-compatibility of the polymer substrates,and optical transparency of the nanochannels for flow visualization are key characteristics of this approach that will be very useful for nanofluidic and biomolecular research applications in the future.

AIM To critically appraise short-term outcomes in patientstreated in a new Pelvic Exenteration(PE) *** This retrospective observational study was conducted by analysing prospectively collected data for the first 25 patients(16 males, 9 females) who underwent PE for advanced pelvic tumours in our PE Unit between January 2012 and October 2016. Data evaluated included age, co-morbidities, American Society of Anesthesiologists(ASA) score, Eastern Cooperative Oncology Group(ECOG) status, preoperative adjuvant treatment, intra-operative blood loss, procedural duration, perioperative adverse event, lengths of intensive care unit(ICU) stay and hospital stay, and oncological outcome. Quantitative data were summarized as percentage or median and range, and statistically assessed by the χ~2 test or Fisher's exact test, as *** All 25 patients received comprehensive preoperative assessment via our dedicated multidisciplinary team approach. Long-course neoadjuvant chemoradiotherapy was provided, if indicated. The median age of the patients was 61.9-year-old. The median ASA and ECOG scores were 2 and 0, respectively. The indications for PE were local y invasive rectal adenocarcinoma(n = 13), advanced colonic adenocarcinoma(n = 5), recurrent cervical carcinoma(n = 3) and malignant sacral chordoma(n = 3). The procedures comprised 10 total PEs, 4 anterior PEs, 7 posterior PEs and 4 isolated lateral PEs. The median follow-up period was 17.6 mo. The median operative time was 11.5 h. The median volume of blood loss was 3306 mL, and the median volume of red cell transfusion was 1475 mL. The median lengths of ICU stay and of hospital stay were 1 d and 21 d, respectively. There was no case of mortality related to surgery. There were a total of 20 surgical morbidities, which occurred in 12 patients. The majority of the complications were grade 2 Clavien-Dindo. Only 2 patients experienced grade 3 Clavien-Dindo complications, and both required procedural interventions.
