BACKGROUND Hepatic infarctions(HI)are ischemic events of the liver in which a disruption in the blood flow to the hepatocytes leads to focal ischemia and *** HI are due to occlusive events in the liver’s blood vessels,but non-occlusive HI may *** are associated with disruption of microvasculature,such as in diabetic *** HI usually presents as peripheral lesions with clear borders,irregular nodular lesions may occur,indistinguishable from liver neoplasms and presenting a diagnostic *** SUMMARY We report a case of multiple extensive HI in a patient with poorly controlled diabetes mellitus,who first presented to the emergency room with diabetic *** then developed jaundice,thrombocytopenia,and a marked elevation of serum *** ultrasound of the liver showed the presence of multiple irregular *** investigation with a computerized tomography scan confirmed the presence of multiple hypoattenuating nodules with irregular borders and heterogeneous *** lesions were considered highly suggestive of a primary neoplasm of the *** the patient was clinically stable,his bilirubin levels remained persistently elevated,and he underwent an ultrasound-guided percutaneous biopsy of the largest *** results revealed extensive ischemic necrosis of hepatocytes,with no signs of associated *** months after the symptoms,the patient showed great improvement in all clinical and laboratory parameters and extensive regression of the lesions on imaging *** This case highlights that diabetic ketoacidosis can cause non-occlusive HI,possibly presenting as nodular lesions indistinguishable from neoplasms.

Genome-Wide Identification and Characterization of the DofTranscription Factor Gene Family in Phaseolus vulgarisL.

The Dof (DNA-binding with one finger) proteins are a class of plant-specific transcription factors that can trigger several processes involved in plant growth and development, as well as in stress responses. Here, we performed a systematic bioinformatics analysis to characterize all Dof genes in common bean, which included analysis of the genome sequence, conserved protein domains, chromosomal locations, subcellular locations, phylogenetic relationships, gene duplications, and gene expression profiles in different tissues. Bioinformatics analysis revealed 36 putative genes related to PvDof that were classified into seven subfamilies (A, B1, B2, C1, C2, D1, and, D2) by comparative phylogenetic analysis. Based on our genome duplication analysis, a total of 36 genes were found to be distributed on all 11 chromosomes, and they expanded through gene duplication in tandem, suggesting the involvement of segmental duplication events in the evolutionary process. Synteny events and phylogenetic comparisons of the Dof proteins of common bean with those of A. thaliana, O. sativa, and G. max L. led to the identification of several orthologous and paralogous genes, which provided further insight into the diversity of the evolutionary characteristics of genes of this family in other plant species. Expression profiles revealed that most of the PvDof genes were expressed in different tissues, indicating that PvDof genes may be involved in various physiological functions during plant development. The results of this study provide additional information and potential biotechnological resources for further understanding the molecular basis of this gene family and consequently improvement of common bean crops.

Taxifolin stability: In silico prediction and in vitro degradation with HPLC-UV/UPLCe ESI-MS monitoring

Taxifolin has a plethora of therapeutic activities and is currently isolated from the stem bark of the tree Larix gmelinni(Dahurian larch). It is a flavonoid of high commercial interest for its use in supplements or in antioxidant-rich functional foods. However, its poor stability and low bioavailability hinder the use of flavonoid in nutritional or pharmaceutical formulations. In this work, taxifolin isolated from the seeds of Mimusops balata, was evaluated by in silico stability prediction studies and in vitro forced degradation studies(acid and alkaline hydrolysis, oxidation, visible/UV radiation, dry/humid heating) monitored by high performance liquid chromatography with ultraviolet detection(HPLC-UV) and ultrahigh performance liquid chromatography-electrospray ionization-mass spectrometry(UPLC-ESI-MS). The in silico stability prediction studies indicated the most susceptible regions in the molecule to nucleophilic and electrophilic attacks, as well as the sites susceptible to oxidation. The in vitro forced degradation tests were in agreement with the in silico stability prediction, indicating that taxifolin is extremely unstable(class 1) under alkaline hydrolysis. In addition, taxifolin thermal degradation was increased by *** the other hand, with respect to photosensitivity, taxifolin can be classified as class 4(stable).Moreover, the alkaline degradation products were characterized by UPLC-ESI-MS/MS as dimers of taxifolin. These results enabled an understanding of the intrinsic lability of taxifolin, contributing to the development of stability-indicating methods, and of appropriate drug release systems, with the aims of preserving its stability and improving its bioavailability.

Longitudinal Care Evaluation in Child Healthcare

Objective: To evaluate the essential attribute of Primary Health Care, longitudinal care, care directed at children from birth to two years old. Methods: This is a descriptive and exploratory study of evaluative character and quantitative approach, conducted with parents/caregivers of 186 children, younger than two years old, patients of primary health care services in the city of Santa Cruz/RN, Brazil. For data collection, the instrument Primary Care Assessment Tools (PCA Tools) was used, and the results of the questions dealing with longitudinal attribute were evaluated. The data were stored and processed in Statistical Package for Social Sciences (SPSS). The study was approved by the Research Ethics Committee of the Health Sciences School of Trairí, under number 348896. Results: The mean age of children in months was 8.21;84.4% (n = 157) were assisted by the same doctor/nurse every time;in 81.7% (n = 152) of cases the doctor/nurse know the full medical history of the child;73.7% (n = 137) answered that the professional know their child more as a person than just as someone with a health problem;48.9% (n = 91) stated that the doctor/nurse do not know their family very well;86.6% (n = 161) reported finding the doctor/nurse understands what is saying or questioning;96.2 (n = 179) of respondents said the doctor/nurse answers the questions so that they understand;96.2% (n = 179) of the interviewed said they feel comfortable telling the concerns or problems of their child to the doctor/nurse;66.7% (n = 124) claimed that they would not change the service/doctor/nurse to another health service. Conclusion: It is concluded that the attribute was well rated by the mothers of children seen in primary health care services and that they can establish good communication and relationship with the health professionals who treat their children.

Assessment of the Accessibility of Users in Primary Health Care

Objectives: The goal of this project was to evaluate the quality of the accessibility of the adult population to services in Primary Health Care, with a view to contribute to the development of measures that will propose improvement in the offered assistance. Methods: This is a quantitative and evaluative study made in the municipality of Santa Cruz, State of Rio Grande do Norte, Brazil, with a sample of 180 people. The study was approved by the Ethics and Research Committee of the Federal University of Rio Grande do Norte under Opinion number 152/2012. Results: It was found that the adult population classified the quality of care from regular to good, showing an association with the waiting time, time spent from the unit to one’s house and the reception. Conclusions: It could be concluded that this study contributes to the development of strategies able to provide a full and equitable care to the adult population in the primary health care network, since this is the gateway to other levels of care, because it aims to promote adult health and prevent diseases.

Tracheal Intubation Difficulties: Systematic Literature Review

Background: Tracheal intubation is a common procedure in medical practice, being performed mainly in surgical centers and emergencies in trauma patients. Objective: This systematic literature review aims to understand the main difficulties and complications related to tracheal intubation. Methods: Systematic literature review, conducted in the Virtual Health Library databases between 2016 and 2021, restricted to articles in English and Portuguese with the descriptors: Airway, Difficult intubation, and Complications. After reading the abstracts, 34 articles were found, and, after reading them, 9 articles were selected. Results: This review analyzed 9 scientific studies that rigorously met the previously established characteristics in the sample selection. The synthesis included the following aspects (author/year of publication, article title, objective, type of study, and database. Discussion: Several factors that influence the success of intubation were identified, such as technical, professional, and patient issues. In addition, measures to minimize the risks involving the procedure were highlighted. Conclusion: Tracheal intubation is a common procedure in surgical centers and emergencies. However, its risks, while low, should not be ignored. Unsuccessful procedures can occur and have consequences for the patient, such as trauma to the upper airway.

Background： This study evaluated the impact of hormonal modulation at the onset of proestrus on ovarian response and uterine gene expression of beef ***： A total of 172 anestrous beef cows were assigned to one of four groups according to the treatment with estradiol cypionate（ECP） and/or equine chorionic gonadotropin（eCG） [CON（n = 43）, ECP（n = 43）, eCG（n = 44）and ECP ＋ eCG（n = 42）].Results： ECP-treated cows（ECP and ECP ＋ eCG groups） presented greater occurrence of estrus（44.6% vs. 65.4%; P = 0.01） and pregnancy per AI [47.1% vs. 33.3%; P = 0.07], but similar progesterone（P4） concentration at subsequent diestrus than cows not treated with ECP（CON and eCG groups）. Nonetheless, eCG-treated cows（eCG and ECP ＋ eCG groups） presented larger follicle at timed AI（12.6 ± 0.3 vs. 13.5 ± 0.3 mm; P = 0.03）, greater ovulation rate（96.5%vs. 82.6%; P = 0.008） and greater P4 concentration at d 6（3.9 ± 0.2 vs. 4.8 ± 0.2 ng/mL; P = 0.001） than cows not treated with eCG（CON and ECP groups）. Next, cows with a new corpus luteum 6 d after TAI were submitted to uterine biopsy procedure. Uterine fragments [CON（n = 6）, ECP（n = 6）] were analyzed by RNA-Seq and a total of 135 transcripts were differentially expressed between groups（73 genes up-regulated by ECP treatment）. Subsequently,uterine samples were analyzed by q PCR（genes associated with cell proliferation）. ECP treatment induced greater abundance of PTCH2（P = 0.07） and COL4 A1（P = 0.02）, whereas suppressed EGFR（P = 0.09） expression. Conversely, eCG treatment increased abundance of HB-EGF（P = 0.06）, ESR2（P = 0.09）, and ITGB3（P = 0.05）, whereas it reduced transcription of ESR1（P = 0.05）. Collectively, supplementation with ECP or eCG at the onset of proestrous of anestrous beef cows influenced ovarian responses, global and specific endometrial gene ***： Proestrus estradiol regulate the endometrial transcriptome, particularly stimulating proliferative activity in the endometrium.

Candida albicans isolated from urine:Phenotypic and molecular identification,virulence factors and antifungal susceptibility

Objective: To isolate Candida albicans(C. albicans) from the urine of hospitalized patients and assess the virulence factors and antifungal susceptibility profiles of the ***: Yeasts were identified using the chromogenic medium CHROMagar?, the VITEK~?2 system, hypertonic Sabouraud broth, tobacco agar, polymerase chain reaction,and DNA sequencing. The evaluated virulence factors were proteinase production,phospholipase production, and biofilm production on polystyrene. The broth microdilution technique was used to determine the minimum inhibitory ***: All yeasts isolated from urine were identified as C. albicans using both classical and molecular methods. Although 91.3% of the isolates showed no phospholipase activity, 56.5% showed strong proteinase activity and 91.7% produced biofilm. All microorganisms were sensitive to fluconazole, voriconazole and amphotericin B, but 56.5% of the yeasts showed resistance to ***: C. albicans isolates from urine have a high capacity for virulence and can be associated with infectious processes. Furthermore, the high percentage of isolates resistant to itraconazole is important because this antifungal agent is commonly used to treat fungal infections in the hospital environment.

Prevalence of Methicillin-Resistant Staphylococcus aureusin Raw Hamburgers and Ready-to-Eat Sandwiches Commercialized in Supermarkets and Fast Food Outlets in Brazil

Although well known, Staphylococcus aureus is a bacterium that remains widely studied because of its high pathogenic potential and its ability to develop resistance to antibiotics routinely used in clinical practice. The present study investigated the occurrence of methicillin-resistant Staphylococcus aureus (MRSA) in hamburgers and sandwiches sold in supermarkets and fast food outlets in Salvador, BA, Brazil. Fifty samples of frozen raw hamburgers (25: beef and 25: chicken) and 50 samples of ready-to-eat sandwiches (25: beef and 25: chicken) were collected and investigated for the presence of MRSA. MRSA was present in 32% of the hamburgers and 8% of the sandwiches. The frequency of MRSA was higher in the samples containing chicken meat. However, the statistical analysis showed no association between MRSA presence and the type of meat investigated (P > 0.05). The high prevalence of MRSA in hamburgers and the presence of the microorganism in ready-to-eat sandwiches are worrying and indicate the need for better control during food preparation to prevent the spread of bacteria.

Postural Balance in the Elderly with Mild Cognitive Impairment: Relationship to Accidental Falls

This study analyzed postural balance in the elderly with mild cognitive impairment and its relationship to accidental falls. A quantitative and quasi-experimental method was used in a sample of 43 elderlies between 64 and 88 years old, mostly females. Data collection was performed in two Basic Health Units in the city of Rio de Janeiro in 2014. The instruments used were the Mini-Mental State Examination (MMSE), Tinetti scale, Motor Scale for the Elderly (EMTI) scale, and Elderly Falls Diary. The data were analyzed with the SAS statistical software version 9.3.1. The scales were applied before and after the psychomotor interventions. A significant difference was observed between the evaluating moments. Balance improvements were observed in all age groups, suggesting that all elderlies, regardless of age, showed satisfactory responses to the implemented psychomotor activities. The elderlies who fell more frequently were those between 60 and 69 years old. It is noteworthy that the age group with the lowest incidence of falls was that of elderlies between 70 and 79 years old. It was concluded that psychomotor activities are beneficial, regardless of age, proving their effectiveness when continued stimuli with cognitive and psychomotor activities are carried out.
